When a dear friend left our house after a holiday party thiswinter, I had no idea that the next time I would see her shewould be a patient at Columbia Memorial Hospital. Shehad been battling cancer for more than a decade, but noone expected that on her way home she would suffer amajor setback and need to be rushed to ColumbiaMemorial.I became a regular visitor to Memorial 4A, where

she was a patient, and as the Executive Director of theFoundation, I cast a keen eye on the staff. What I saw were capable, caring professionals who really believe in our hospital’s “patients first” philosophy. I witnessed team-work, collaboration and commitment. The quality of carethey delivered was impressive – achieved through kindness,compassion and skill. And in speaking with people visitingother patients, I quickly realized that this level of care isnot an aberration – it happens every day at ColumbiaMemorial Hospital. After nearly a month in the hospital, I lost that dear

friend. Finally, even the best medicine and the best carecouldn’t beat her cancer. But what I gained was a profound respect for the

amazing care that is delivered at our hospital. I renewed my promise to “dream big,” because at the Foundation,that’s what we need to do. Every day we need to renew ourefforts to raise the funds that make it possible for our staffto deliver the care that our patients need and deserve.“Patients First!”In order to succeed, we need your help and support.

Our hospital is continuing to grow, but even more important, we are getting better and better. Our quality of care indicatorswhich are documented by the federal government demonstratethat we are at 100% with regard to our treatment of heartattack, congestive heart failure, and post-operative surgicalpatient care. In fact, our surgical site infection rate is 0.3%which is far below the national average. In addition our patientsafety program has enabled us to reduce serious safety events by80% over the past two years. We went almost a full year withouta single serious safety event, but in January one of our patientsfell and sustained a fracture. The fact that both our entire hospital staff and our entire medical staff, have undertaken education in patient safety techniques is paying huge dividendsfor our patients. In order to provide greater access to the community for

essential and urgent care, we will be opening an urgent carecenter later this year. We have also recruited a new pediatricianwho will join us in May; a new internist who will join us in June, and an obstetrician as well as an endocrinologist and apulmonologist/critical care specialist, all joining us in August. If you are wondering what the construction is at the

ambulance entrance to The Kellner Wing, it is an expansion for our increasing patient volume. We are adding space to usefor treating psychiatric patients. The space will be tailored totheir specific needs and in turn it will free up space within theexisting Emergency Department for our medical surgicalpatients. This project is scheduled to be completed in June,before the summer influx. We also continue to receive letters from very satisfied patients

which is the best motivation there is for our staff to continue toprovide such a high level of care. A “new”hospitalLegacy images of the “old” Columbia Memorial Hospital linger insome minds. Renowned sculptor John Cross, a recent patient, advises people to use Columbia Memorial where he received the very best of care.”

Back in the beginning of January, noted wood-carver and sculptor John Cross was at a partywhen he felt faint. He left the woman withwhom he was speaking and went to find thehost. Shortly thereafter, Cross said, “I went intoa full nod. The host called for emergency help.There was a doctor and a nurse at the party,and they said, ‘you’d better go to the hospital.’”Cross was rushed to the Emergency

Department at Columbia Memorial Hospital.Fortunately, after an overnight stay and a series of tests, he was found to be sufferingfrom nothing serious, and was sent home.As a result of his experience at Columbia

Memorial, Cross wrote to the Register Star tosay: “That Saturday night I was delighted to find that things have changed very much forthe better at Columbia Memorial. I was giventhe very best of care by conscientious, highlycapable and caring doctors and medical staff,and the facilities are as clean and as modern as you could wish.”

Cross noted that going to the ER is not like going to the spa. At best it’s a frighteningtime for the patient, yet at Columbia Memorial“I appreciated how I was treated.”Cross, who keeps his studio in Elizaville,

where he lives with his wife, Linda, said, “We’vebeen here 40 years. You get to know people. I remember when we first came here theHudson hospital was looked on with dread. Old legends die slowly. People get a fix onsomething, it’s hard to get them to change. If you can get the word out, I think it mightchange things a bit.”Cross’s work has appeared in exhibitions

from Chicago to New York City to London, and countless places in between. It is part ofthe private collections of, among others, theSmithsonian, Absolut Vodka, Garrison Keillor,and Ali McGraw.His woodcarvings are of a highly eclectic

nature, encompassing such subjects as NegroLeague baseball players Josh Gibson and

Smokey Joe Williams; comedians Woody Allenand Groucho Marx; artist Pablo Picasso; Jackie Robinson and Larry Doby, who broke the major league baseball race barrier; and a full depiction of Bobby Thomson’s 1951 pennant clinching “Shot Heard ‘Round TheWorld.”“There is a simplicity, there is a directness,

there’s a sense of America” in what Cross does,he said. “All my pieces seem to have someAmericana to them. Baseball, circus figures,sideshow people, people playing cards.”Meanwhile, Cross is grateful for the care

he received at Columbia Memorial, and looksforward to being part of the 20th Anniversaryexhibit at the Carrie Haddad Gallery in Hud-son. There are also plans for an upcomingshow at the Visitors Center at Art Omi.

“I know of a number of people who havebeen to Columbia Memorial and been treatedwell. To think that we can trust in the hospitalis wonderful,” said Cross. �

Columbia Memorial Hospital anesthesiologist John Mathews providescare worldwide as one of the physicians in Medicins Sans Frontiers.

Dr. John Mathews’ first mission for MedicinsSans Frontiers, known in this country asDoctors Without Borders, was in 2008 inAmman, Jordan. For two months he worked with a team of Iraqi surgeons on bomb blastvictims from Iraq, the occupied areas of theWest Bank, and Gaza Strip. These patients had survived their initial trauma but needed“post-blast” reconstructive work. “These werenot acute injuries, which is unusual for MSF.There were both adults and children. It wasvery intense work, but was even more emotion-ally draining – it was hard to see kids with burnscars as well as severe facial and limb disfigure-ments,” said Mathews.To those who would question the wisdom

of willingly putting oneself in harm’s way in theMiddle East, Mathews would say that was notthe case at all. Amman, Jordan was perfectlysafe.Nigeria, however, was an altogether

different experience. In 2010 he was sent by MSF to Teme Hospital in Port HarcourtNigeria in western sub-Saharan Africa, on afour week assignment. The team saw three to five acute traumas per day. These were mostly gunshot wounds from automaticweapons and machete attacks stemming frompolitical unrest. These created an enormousworkload that often extended late into thenight. While he never felt “particularly unsafe”in Nigeria, thanks to tight security protocols,there were definite security concerns. There was constant radio communication between your location and the hospital. The routes to and from the hospital were changed daily toeither avoid areas of gunfire or avoid potential kidnapping. People with machine guns were a very common site in the Niger Delta. However,Mathews said, keep in mind that “all politicalfactions and combatants need the help MSF is providing. MSF has one single agenda – toprovide medical care to those most in need.That included all combatants, as long as theyleft their guns at the door.”In Port Harcourt, resources we take for

granted here were often not available requiring

the team to rely heavily on clinical judgment.One thing that Teme Hospital does to treatmassive hemorrhagic shock is to stock “whole”blood and not separate it into various compo-nents as is done here in the United States.“They have an extensive blood bank, andbelieve me, it’s needed,” Mathews said. In the true spirit of MSF, the medical team

in Port Harcourt was multinational from fivecountries representing four continents includ-ing many local Nigerians. “There is truly noth-ing like responding to what is an obvious, deepneed. You realize how blessed we are in thiscountry. It puts you back in touch with yourpriorities,” Mathews said.The core principles of MSF’s humani-

tarian work are Neutrality, Impartiality, andIndependence. An extensive interview processis conducted before accepting an applicant. If accepted for a mission all travel, lodging(very austere), and meals (also very simple butnourishing) are provided. You have to speakeither English, French, or Spanish. It also really helps to speak more than one of thoselanguages. “I speak French fluently, which was a big

plus because I can therefore accept assign-ments where the team’s primary language isFrench.” The five major crises that are thefocus of MSF work are armed conflict, malnutrition, epidemics, natural disasters, and exclusion from health care (as in the cases of migrant workers and street children,for example). MSF is a global effort with missions in approximately 80 countries.Briefing and debriefing at the start and end of an assignment takes place at the headquarters in Paris and again in New York.Columbia Memorial has been very sup-

portive of his efforts, he said. “The hospital embraces it. And when I

return from a mission, I give talks to staff and personnel.” Mathews’ MSF missions “put me back in touch with all the reasons why I became a doctor.” Some aspects of themissions are fun (i.e. Paris) but the work isoften emotionally draining, physically exhaust-ing, and gut wrenching. It is also unbelievablyrewarding. �

Global medicine

Dr. John Mathews, theChief of the Anesthesi-ology Department atColumbia MemorialHospital since 1998, has been at ColumbiaMemorial for nearly two decades. A generous supporter of local charitable organizations, Dr.Mathews joined theBoard of the Columbia-Greene HospitalFoundation in 2010.

He is also very passionate about hisinternational humani-tarian work withMedicins Sans Frontiers(MSF), also known asDoctors Without Borders.He hopes to continue missions with MSF every other year until his retirement, then continue practicing medicine in the ThirdWorld throughout mostof the year. Thus far, Dr. Mathews has been in Amman, Jordan, and Port Harcourt, Nigeria,and although eachinvolved victims ofarmed conflict, heencountered two vastlydifferent situations. �

Economic impactColumbia Memorial pumps $193 million annually into the community.

Columbia Memorial has become the engine of the local economy, pumping in more than$193,442,000 annually, according to figures compiled in 2008 and 2009 (2010 figures arenot yet available) by the Healthcare Associationof New York State (HANYS) and the hospital.Columbia Memorial increases commerce

through the 1,400 people it employs, purchaseof supplies, and hiring local contractors. Inaddition, employees spend their salaries andpay state and local sales taxes, further impact-ing the economy.This is in addition to the positive impact

the hospital has providing expert medicine for the whole family in Greene, Columbia and Northern Dutchess counties: birthingbabies, caring for the aged, ensuring publicsafety during disasters, promoting healthylifestyles, addressing emergencies, and pro-viding end of life care and comfort.“The hospital treats more than 34,000

patients in our emergency department everyyear, delivers 500 babies, and provides morethan 180,000 physician office visits. Our annualoperating budget is over $130,000,000 and weemploy more than 1,400 individuals who reside in our area,” said CEO Jane Ehrlich. Ehrlich said that the hospital has more than

200 physicians on staff, and provides more than$8,000,000 in free care every year to those inneed.“We’ve invested more than $60 million in

new facilities and equipment in the last tenyears including new surgical instrumentation, patient information systems, and medical imaging technologies,” she said. The hospital’s capital spending in 2009

generated approximately $10,557,000 for thelocal economy.Columbia Memorial has a total payroll of

close to $84,726,600. “Hospital employees use their wages to

purchase goods and services, creating jobs andincome for other businesses,” said Ehrlich.

The employees of the hospital and the jobs supported indirectly by the facility paid$3,884,000 in state and local income taxes forthe periods examined in the HANYS/hospitalreport. Locally, the sales tax impact of spendinglocally was $2,251,000; and $2,373,000 was statesales tax.“As the hospital continues to grow, refurbish-

ing older areas of the hospital and buildingnew facilities, we affect everyone from the local convenience store down the street, to the insurance agents whose policies back thecontractors who work on these capital projects,”said Ehrlich.Construction projects to improve health-

care delivery currently underway (refurbishingthe 5th and 6th floors of the hospital) total approximately $4.5 million dollars.The reportalso cited the impact that hospital visitors have on the local economy when they use local community amenities such as restaurantsand motels.HANYS calculated these figures with the

Regional Input-Output Modeling System II(RIMS II) developed by the U.S. Department of Commerce, Bureau of Economic Analysis,and acquired other data from hospital annualreports. �

Thanks to an $80,000 donation from the Hudson River Bank & TrustCompany Foundation, a new rehab center in Hudson completes theorthopedic pathway from diagnosis through surgery and rehab thatcan get you back in action a lot faster than you think.

A helping hand

An $80,000 donation from the Hudson RiverBank & Trust Company Foundation for a newrehabilitation center at Fish and Game Road inHudson completes the hospital’s orthopedicpathway – a continuum of care that helpsrestore you to your former self when you incur an injury to bone or joint.“We are very excited to award Columbia-

Greene Hospital Foundation our grant,” saidCarl Florio, who serves on the Board of theHudson River Bank & Trust CompanyFoundation. “This project holds the promise of better

serving the area residents and we commend the hospital for its success in completing theexpansion of the rehabilitation facility.”Orthopedic surgeon Louis DiGiovanni

says the combined goal of Columbia Memorial Bone & Joint and ColumbiaMemorial Rehabilitation Services is to help the patient “get his or her life back through asuccessful diagnosis, surgical treatment andpost-operative rehabilitation program.” “Do you remember the old days when

people sat in a rocking chair on the porch?Often they were limited to minimal activity dueto injury or age related degenerative processes.You don’t see them anymore. We make lifeworth living. It’s about the quality of life.”Over the last twenty-five years, DiGiovanni

said he has seen “tremendous innovations in orthopedics. Back in 1985, we didn’t have techniques and procedures we now have. We’ve moved ahead. It’s been fun to watch and to be a part of such dramatic improve-ments in orthopedic care.”DiGiovanni said that when he started on

Union Street, Hudson, “we had 2,000 squarefeet of office space and two orthopedic sur-geons. Now, we have 17,000 square feet ofoffice space, with an onsite physical therapist,four orthopedic surgeons, a hand surgeon, asports medicine physician, and four physicianassistants.”

Following orthopedic surgery, the patient willgo to Columbia Memorial Rehabilitation wherephysical therapist Anna Marie Zietsman andher team put on the finishing touches. Theidea is to “improve the patient’s functions forhis or her day-to-day activities,” she said.“We’re very fortunate to have occupational

therapists who handle upper extremity cases,particularly elbows and hands.”“Our goal is that our patients get back to

what they love doing and have the best qualityof life possible.”“We work with everyone from the high

school football player to the recreational golfer, but also with those just getting intoshape to do gardening.”Zietsman said “it’s exciting to work with

the state-of-the-art equipment we have. We’revery lucky to have this new space thanks to adonation from the Hudson River Bank & TrustCompany Foundation.”The circle thus completed, Columbia

Memorial Bone and Joint Center and ColumbiaMemorial Rehabilitation have accomplishedtheir mission of returning their patients to their active lives. “The Hudson River Bank & Trust Company

Our best medicine practices, state-of-the-art-equipment and managerial practices placeColumbia Memorial firmly in the sights of those who can learn from our experience. That happened recently when Ciming

Shan, Chief Nurse Executive with the HenanProvince People’s Hospital in Zheng Zhou,China, visited Columbia Memorial Hospital aspart of an exchange of scientific, cultural, andnursing managerial practice between the twohospitals.The Henan Province Hospital is the largest

in that region of China with more than 2,900beds. Nurse Shan manages more than 1,600nurses at this hospital about six hours south of Beijing.Shan is visiting other U.S. hospitals and

nursing schools as well. Three years in the making, this program is “an open exchangethat will contribute to cultural and scientificprogress,” according to the text of a full colorreport Shan brought from her hospital. Now in a major development program, Shan’s hospital will soon go from 15 ICU beds to more than 200. Yet, she remarked that she has learned a great deal from ColumbiaMemorial’s practices, even given the size difference of the two healthcare institutions.

Snow, ice and bedside codingShan remarked, through translation providedby Columbia Memorial Director of Pathology,Dr. Eric Li, that she is especially interested in nursing management techniques, nursing education and how the nurses serve theirpatients. When asked what the most remarkablething she had experienced so far, Nurse Shanlaughed: “The snow and ice.” She has beenhere through our winter storms.On the serious side, she said: “Even though

small, this is a well run hospital.”While shadowing ICU nurses, Shan was

especially impressed with the bedside codingsystem for the dispensing of drugs and the responsibility of the nurses using this system.She admitted that even though her hospital

report to the Provincial government with a formal document citing healthcare advancesshe has seen. She will then report to hospitaladministration and speak to nursing staff aboutthis exchange.According to Leitha Pierro, Vice President

of Patient and Clinical Operations, ColumbiaMemorial has entered into a sister hospital relationship with the Henan ProvincialHospital. “Hosting Ciming Shan has provided

learning experiences for our nurses as we compare both cultural and professional differ-ences in providing nursing care to our patients.It has been very exciting,” said Pierro. �

Rotary donation helps improve cardiac event recovery

For those who suffer sudden cardiac arrest, there’s approxi-mately a 95% mortality rate. Many die within minutes.But thanks to an $8,000 donation from the Hudson Rotary

You keep the patient cold for 24hours, with a core temperature of 93 degrees Fahrenheit, andthen revive them by bringing their temperature slowly back tonormal.”The success ratio has been astonishing, Weisberg said.Early on, ice was used to lower the patient’s temperature,

“but that was messy,” Weisberg said, presenting problems withwater melting all around.At the January 10 meeting of the Hudson Rotary, Dr.

Weisberg accepted the $8,000 check that will be used to purchase what amounts to a device in which to wrap the bodyfor this protocol. “It is almost like being in a sleeping bag.” �

The Columbia-Greene chapter of the National Alliance for Mental Illness (NAMI) presented Columbia Memorial Hospital with a check for$2,100 recently to use toward the planned expansion of the psych ward.

Columbia Memorial will renovate the entire fifth floor psych ward and add four beds to the unit. NAMI raised the money through a pancakebreakfast, held in August at Applebee’s on Fairview Avenue in Greenport.NAMI is a grassroots support group for family members of and peoplediagnosed with psychiatric disorders. �
