    IntroductionThis report summarises the findings of a health design event run by Action Caerau Ely and Spice to

    engage community members in Ely, Cardiff, in the design of a healthy community. People shared th

    thoughts, feelings and ideas about things that they feel affect their health, barriers to accessing hea

    related services and activities and ideas for improvements or innovations around the delivery of heservices. Many of these are things which the community felt they can lead or play an active role in

    delivering. Since this event community members have become involved in a number of health initia

    including with working with the NHS to design a new NHS direct phone line service for Wales.

    Anne-Marie Lawrence, who co-ordinates the

    Timeplace project in Cardiff West organised the

    event;

    I recently attended a conference about the

    coproduction of health services. It struck me that

    there was a gap, that I might be able to help fill.

    Many of the attendees were high level managers of

    health services, they all made the right noises

    about wanting to co-produce their services but

    didntreally know where to start. I could see that by

    working with ACE and the local community wecould begin to have some useful conversations with

    residents about health.

    Spice

    Spice programmes are based on a belief that

    everybody has something of value to contribute

    and that services work better when they are

    shaped and delivered by the people who use

    them, in partnership with and supported by

    agencies and professionals. Time Credits are a

    way to support individuals to recognise and realisetheir potential, and enable everyone to be part of a

    their community.

    Spice are a national organisation who develop local Time Credit programmes as a means

    of achieving co-production and community engagement. People receive Time Credits as a

    thank you for contributing their time to a community or service. They can then spend time

    credits to access events, activities and services, or to thank others in turn.

    Time Credits are demonstrating very positive impacts for communities, individuals and

    organisations. 64% of programme participants report improved quality of life, 73% feel they

    can contribute more and 49% feel more confident. 67% of people have made new friends,

    65% now spend more time with others in their community and 64% know people in their

    community better. 74% of organisations agreed Time Credits had helped them make better

    use of the skills and resources in their communities.

    ACE - Action in Caerau and Ely

    Action in Caerau and Ely (ACE) host the Cardiff West Communities First programme and

    have partnered with Spice to develop the Timeplace Time Credits programme in Cardiff. ACE

    was formed in 2011 to work with the people of Ely & Caerau towards the successful social,

    economic and environmental regeneration of their community.

    Communities First Health & Wellbeing Team

    The Health and Wellbeing Team deliver the Healthier Communities strand of th

    Communities First programme for ACE in Cardiff Westcovering the wards of

    Caerau, Fairwater and Pentrebane. The team works in partnership with the loca

    community, developing activities and initiatives that help people to improve their

    and wellbeing. Activities are planned that provide opportunities for people to en

    healthy behaviours such as walking or cycling and increase knowledge and pracskills in areas such as healthy eating and mental wellbeing. Activities are desig

    provide an element of social support and take a holistic approach so that an indiwider needs and barriers, such as financial difficulty, isolation or childrens behav

    issues are also addressed, either on the spot or through support to access othe

    linked Communities First activities.

    Community Ambassador programme

    The community ambassador project is all about helping local people know about

    access to benefit from and be able to influence services which could improve phand mental health. Ambassadors can get involved in the project in a variety of w

    Co-production in Health We have a problem of rising demand and increasing stress on public

    finances.

    There's a growing recognition that the system is paternalistic, based on

    the medical model rather than the social model, and only designed to dealwith illness and crisis.

    This approach means that people remain passive recipients rather than

    active agents of their own health and wellbeing.

    Acceptance of need to change and to support communities to work with

    service providers to share responsibility for their own health and care

    particularly in relation to well-being. For example it is increasingly

    accepted that social issues such as loneliness and isolation impact

    directly on health, increasing the strain on both GPs and hospitals. In policy we are seeing emerging consensus in health and social services

    through the Prudent Health Care agenda and the Social Services &

    Wellbeing Act 2014. The emphasis is on partnerships between providers

    and citizens, with local authorities required to involve people in design and

    delivery of services.

    There's also a recognition that co-production and asset based community

    development can play a crucial role in health & well-being through building

    sustainable neighbourhoods. These approaches have the capacity to bring together partners from

    education, public health, community safety and volunteering to create

    powerful, citizen-led networks which support well-being and health.

    He described the importance of, and

    challenges around, healthy eating,

    physical activity, housing, and having

    the time and opportunity to do

    activities that relax you and give you

    enjoyment, for improving andmaintaining health and wellbeing.

    The political perspective

    Mark Drakeford, Health Minister for Wales, spoke at the ev

    the importance of coproducing health.

    We have to consider everyone as equal partners because ev

    counts. Both parties are experts, of course the professionals

    in providing care, and the users are expert about their life, wh

    have to face every day, and they have to be listened to.

    By co-producing the health system together we can make a b

    future. We all need to take responsibility for our health, we n

    aware about what we can do and what we can avoid for our c

    and future generations. Changing things at a small scale as i

    and as a community can enable us to share in better global wFor me being here today is a great chance to be a part of cre

    Event overview

    The event was designed not only to be a place forexchange of ideas about the health system andpublic services, but also to enable communitymembers to express different points of view abouthealth issues they face in their daily lives andunderstand the factors that impact peopleshealthin Ely.

    Attendees were invited to take part in a number ofdifferent activities that explored their experiencesand opinions about their health and the servicesthat impact on it. The event will be the start of alonger term process in which ACE, Spice and thecommunity will look at how we can address health

    needs in new ways and how we can bring togetherservice providers and service users to transformthe way that services are delivered.

    Key themes that came out of this activity wpeople feel a lot of their stress at home an

    struggle to find a balance, sometimes from

    Time Credits

    Established in 2012 the Timeplace time credits scheme

    which operates in Ely, Caerau and Fairwater now has

    Over 85 community groups signed up

    Approx. 1000 individual members

    Over 30,000 hours of time donated to the community

    Over 100 time credit events, activities and trips

    organised by the community

    Last year we went on so many family trips during the

    six week holidays than I would have before as money is

    very tight because of not being able to work due to

    medical reasons. The children they enjoyed it so much,

    then Xmas I did some thing they have always wanted to

    do I was able to take them on a holiday with timecredits, it might of only been down the road to porthcawl

    but it was a holiday and kids myself and husband

    enjoyed it so much thanks to time credits!!

    In one year alone my family and I have earned over 300

    time credits which has enabled me to take the children on

    many trips, go weekly to orchestra practice, stay in

    Porthcawl for the weekend, visit cinema monthly at Chapterand many other activities that I may not have been able to

    do without time credits! I have also been on courses for first

    aid, food hygiene, laughter yoga, mental health first aid and

    now Ive even got a job out of it. I have also made many new

    friends along the way!

    Time CreditsTime credits have had a huge impact in

    the local community. People donate an

    hour of their time to help the communityin return for one credit that can then be

    spent on a range of services, activities

    and events.

    People can already use their time credits to access a wide

    health related activities and services, wed like to see more

    organisations get on board with time credits to widen the of

    deepen the benefits for community members. Here are just

    the health related activities people can spend their hard ear

    credits to access

    Spa and pamper day events

    Without time credits I would never of been able to afford to

    take my kids on trips, sometimes its just what you & your kids

    need, a break from the stress & strain & much needed bonding

    time. I also use time credits as a gateway, from going on litterpicks to doing courses to earn time credits & realising I do

    have skills & now having the confidence to change my life, bygoing to university in the near future. I am a great believer in

    timecredits, I believe they can do great things not just for the

    individual but for the whole community

    NHS phoneline co-designJust before the health design event in March we were contacted by Co-

    production Wales to see if any community members might be interested in

    taking part in a project around helping input into the design of a new NHS directphone line for Wales.

    We invited them along to our health design event to run an activity.

    Since then they have come back to run a workshop with community members,

    some of whom then participated in an NHS urgent care event working with

    health professionals to share their experiences and ideas as service users.

    Community members are now in the process of forming a steering group that

    will work with the NHS over several months to inform the design of the newservice.

    Whats next?

    Ace are training up community members to

    deliver training to professionals and

    communities across Wales and have

    community members who are keen to take

    part in more co-design and consultation

    events. ACE are also developing the nextphase of the Timeplace project which will

    focus on co-production and how to use time

    credits to tackle the main issues affecting the

    community.

    Currencies in Action (CCIA) collaboration project.
