Chronic Hepatitis C
• A leading cause of cirrhosis in the US.• 8,000-10,000 deaths/yr
– This number expected to triple in the next 10 to 20 years
• Associated with an increased risk of liver cancer• Most common reason for liver transplantation in
the US

Diagnosis of Chronic Hepatitis C
• Ask patients about risk factors– Remember that many infected patients deny
risk factors• Obtain liver enzymes as part of routine
blood work• Order serologic tests in all patients with
viral risk factors or elevated ALT levels
![Page 14: CHRONIC HEPATITIS C: UPDATE - Department of Medicine · • Incidence, acute hepatitis C – 1980’s: 180,000 cases/yr – 1990’s: 30,000- 35,000 cases/yr • 1.8% of Americans](https://reader035.fdocuments.net/reader035/viewer/2022081522/5ee1437fad6a402d666c3495/html5/thumbnails/14.jpg)

New CDC Recommendation for HCV ScreeningThe “Birth Cohort”
CDC is recommending that everyone born during 1945 through 1965, also known as
baby boomers, get a blood test for Hepatitis C. This recommendation calls for one-time
testing of baby boomers.
![Page 16: CHRONIC HEPATITIS C: UPDATE - Department of Medicine · • Incidence, acute hepatitis C – 1980’s: 180,000 cases/yr – 1990’s: 30,000- 35,000 cases/yr • 1.8% of Americans](https://reader035.fdocuments.net/reader035/viewer/2022081522/5ee1437fad6a402d666c3495/html5/thumbnails/16.jpg)
Hepatitis C Antibody (ELISA)
• EIA-2 test for detection of hepatitis C Ab– Assay detects 2 epitopes of Ab
• Sensitivity 92%-95%• Detection of anti-HCV averages 12 weeks
following infection• Positive test is diagnostic in patients with
elevated ALT and viral risk factorsAlter MJ. Semin Liver Dis, 1995
![Page 17: CHRONIC HEPATITIS C: UPDATE - Department of Medicine · • Incidence, acute hepatitis C – 1980’s: 180,000 cases/yr – 1990’s: 30,000- 35,000 cases/yr • 1.8% of Americans](https://reader035.fdocuments.net/reader035/viewer/2022081522/5ee1437fad6a402d666c3495/html5/thumbnails/17.jpg)
Hepatitis C Antibody (ELISA)
• False positives– Autoimmune diseases– Hypergammaglobulinemia– Suspect if positive in patient with normal ALT and no
viral risk factors• False negative
– Immunosuppressed patients (eg. transplant patients)– Chronic dialysis patients

Recommendations to Avoid Transmission of Hepatitis C
• Adherence to universal healthcare precautions• No donation of blood, organs, tissues, or semen of
individuals who are hepatitis C positive• Use of condoms by individuals who have multiple
sexual partners• NO changes recommended in sexual practices in
monogamous long-term relationships• Testing for HCV recommended for sexual partners
of infected patientsNIH Consensus Committee, 1997

Natural History of Chronic Hepatitis CSummary
• Asymptomatic in early stages– Recognition of high-risk groups– Diagnosis requires high index of suspicion– Early diagnosis critical for early treatment
• During first 2 decades, modest rates of morbidity and mortality
• After second decade, morbidity and mortality more aggressive
• Often progressive, leading to cirrhosis, hepatic failure, and HCC
![Page 29: CHRONIC HEPATITIS C: UPDATE - Department of Medicine · • Incidence, acute hepatitis C – 1980’s: 180,000 cases/yr – 1990’s: 30,000- 35,000 cases/yr • 1.8% of Americans](https://reader035.fdocuments.net/reader035/viewer/2022081522/5ee1437fad6a402d666c3495/html5/thumbnails/29.jpg)
Clinical Presentation of Chronic Hepatitis C
• Typical patient is asymptomatic• ALT (SGPT) may be normal or have mild-
to-moderate elevations– As many as 45% have normal ALT at initial
evaluation• Any degree of ALT abnormality must be
evaluated further!
![Page 30: CHRONIC HEPATITIS C: UPDATE - Department of Medicine · • Incidence, acute hepatitis C – 1980’s: 180,000 cases/yr – 1990’s: 30,000- 35,000 cases/yr • 1.8% of Americans](https://reader035.fdocuments.net/reader035/viewer/2022081522/5ee1437fad6a402d666c3495/html5/thumbnails/30.jpg)

Spontaneous Post-Marketing Cases of HBV ReactivationWith 2nd Generation DAA Regimens
• FDA adverse event reporting system database (2013-2015)• HBV reactivation with DAA therapy (n=29, 5 in the US)
– Median age: 58 years– Male: 45%– Median onset: 46 days– Heterogenous in HCV genotype, DAA received, and baseline
HBV parameters– Anti-HBc data were available in 6/29 cases
• All 6 were anti-HBc positive and experienced reactivation

Bersoff-Matcha SJ, et al. Ann Intern Med. 2017;166:792-798.
• Outcomes of HBV reactivation– Decompensation (n=3)
• Death in 2 and liver transplantation in 1
• HBV treatment given (n=16)– Entecavir (n=9), tenofovir DF (n=6), emtricitabine/tenofovir DF
(n=1), not reported (n=6) – Treatment was usually delayed (≥7 cases)– HBV treatment improved HBV DNA levels and other signs and
symptoms
• No consistency with regard to DAA received– Suggests a drug-class effect

Chronic Hepatitis C
Major Points
• Chronic hepatitis C progresses in a large number of patients
• Hepatitis C should be suspected on the basis of risk factors or elevated ALT levels
• Severity of disease is best assessed by histologic exam of liver tissue
• Curative treatment is available
![Page 64: CHRONIC HEPATITIS C: UPDATE - Department of Medicine · • Incidence, acute hepatitis C – 1980’s: 180,000 cases/yr – 1990’s: 30,000- 35,000 cases/yr • 1.8% of Americans](https://reader035.fdocuments.net/reader035/viewer/2022081522/5ee1437fad6a402d666c3495/html5/thumbnails/64.jpg)
