MESSAGE FROM THE EXECUTIVE DIRECTOR

Dear Readers:

Entering the Fall of 2011 reminds us of the Thanksgiving season. This is the time the staff, Board members, and volunteers get ready for the days of family union, near and far. This is also an opportunity for all of us at the Vietnamese American Cancer Foundation (VACF) to express our thanks to your generous financial contributions and manpower

resources; this is important to us because it shows the heart of solidarity for those who need your support. We would also like to extend our thanks to all of the volun-teers for your enthusiastic contributions of time and talents; VACF would not have been able to arrange and organize multiple community lectures in various topic such as breast cancer, colorectal cancer, and ovarian cancer. In addition, you have also been the integral part of the events for breast cancer, cervical cancer, and screening for Hepatitis B & C which are totally free of charge for hundreds of people. Last but not least, we thank all Board Members for their contributions in both finance and nu-merous volunteer hours, all doctors in different areas and specialties, associations, comprehensive hospitals, community hospitals, and many non-profit organizations.

To meet the needs of the Vietnamese community, VACF has constantly developed new and free services for the community. For example: researching for different short-term financial sources for cancer patients who are undergoing treatment and unable to work, navigating and/or filling out application forms for cancer and hepatitis B & C patients to apply for free medications for the whole year. Volunteers are always vital assets and resources for VACF to help make our mission, program service goals, and service activities deliverable and feasible to the public. The great advantages and benefits for many Vietnamese American communities and other communities in diversity are to gain more cancer awareness and engage their active roles in cancer prevention, early detection, and treatment options. Our volunteers have never appeared reluctant. They are joyful and compassionate, because they themselves have been undergoing the pains and agonies in life that cancers have wrestled. As a matter of fact, each moment of their agonies has built in them a sense of purposefulness and value in life. They are willing to volunteer their time and efforts to share and walk along with others through the unforeseeable battles with cancer. We are honored to present one of our volunteer: Duy Quoc Pham.

“I was supposed to intern at a non-profit organization for my upper division writing class this quarter in order to graduate from college. Therefore, I searched through the list of sites that my school offered and saw the Vietnamese American Cancer Foundation. Instantly, I felt that I should contribute something to the Vietnamese community because I am a Vietnamese. So, I decided to start volunteering before my official date started. I got a chance to meet other staff members such as Khanh Ly, Chau, and Amy. They are all friendly and hardworking people that dedicate, not

only their time, but also all their efforts and thoughts to help patients. I really enjoyed working here because the work environment is so calm and peaceful. Nevertheless, at the same time the staff and volunteers here do not force themselves to work for their own gains, but rather to help people in need.

First day at work, I was assigned to call patients and asked them about their results of recent breast and cervical screenings. Half way through the day, I already called and talked to one third of the patients on the list. The manager, Khanh Ly, told me that I was the first guy to ever take on this job of calling patients regarding their breast and cervical screening results, and she asked me if I felt awkward or weird. I didn’t feel awkward or anything because I knew the exact purpose of my job, and I thought it was meaningful and patients expressed their appreciation when I called about their health. I got a chance to interact with patients over the phone and in person to help them with their concerns related to health and financial aspects while they are undergoing treatments. I improved as a person after working at VACF, I learned to empathize with others, how to offer them emotional comfort when they need it most, and most importantly to work for greater purposes to help others rather than just for my own gains. “

Vì vậy, con đã tìm trong danh sách trên trang web của trường, con thấy Hội Ung Thư Việt Mỹ và con đã nói với bản thân mình, con là người Việt, con cảm thấy nên đóng góp một chút gì đó cho cộng đồng Việt Nam. Vì vậy, con quyết định bắt đầu làm việc trước ngày thực tập chính thức . Ở đây con có cơ hội gặp gỡ các nhân viên khác như cô Khánh Ly, cô Châu và chị Amy. Tất cả đều là những người thân thiện chăm chỉ, cống hiến tất cả nỗ lực và tâm trí vào việc giúp đỡ bệnh nhân không kể thời gian. Con thật sự rất thích công việc ở đây bởi vì môi trường làm việc rất yên bình, nhưng chủ yếu là con cảm giác được các nhân viên và tình nguyện viên làm việc không vì tiền bạc hay cá nhân nào mục đích là để giúp đỡ tất cả những người cần giúp. Ngày đầu tiên ở hội, con đã được phân nhiệm để gọi bệnh nhân và hỏi thăm về kết quả truy tầm ung thư ngực và cổ tử cung. Nửa ngày làm việc trôi qua, con đã gọi và hỏi thăm được 1/3 số bệnh nhân trong danh sách. Cô Khánh Ly nói với con là, con là người nam đầu tiên đảm nhận công việc hỏi thăm bệnh nhân về những vấn đề nhạy cảm này. Cô Khánh Ly hỏi con là có cảm thấy ngại không. Con không cảm thấy ngại ngùng hoặc khó chịu bởi vì con biết mục đích cao cả của công việc này. Con nghĩ công việc này có ý nghĩa và bệnh nhân rất thông cảm và cám ơn khi con gọi hỏi thăm về sức khỏe của họ. Con có cơ hội để tiếp xúc với các bệnh nhân qua điện thoại và ngoài đời để giúp họ về nhiều thắc mắc liên quan đến sức khoẻ và các khía cạnh tài chính trong khi họ đang điều trị bệnh. Con cảm thấy mình có nhiếu tiến bộ sau khi làm việc ở hội, con đã chia sẻ nỗi đau khổ thể chất và tinh thần của bệnh nhân. Làm thế nào cho họ sự thoải mái về tinh thần khi họ cần nhất, và quan trọng nhất là làm việc cho mục đích lớn hơn và khác hơn là nghĩ về lợi ích riêng tư của riêng mình.”

TEA PARTY On August 14, 2011 On August 14, 2011 On August 14, 2011 On August 14, 2011 VACF had our annual 9th Tea Party at the Regent West Restaurant.

There were 125 attendees including 15 volunteers, Dr. Bich Lien Nguyen, Dr. Richard Reitherman, Dr. Jay Harness, and the Compassionate Service Society group. We had a total of 62 cancer patients/survivors, of which 57 were breast cancer patients/survivors.

All three doctors who specialize in breast cancer were enthusiastically educating the audience on breast cancer treatments and answering all questions from the audience. Dr. Jay Harness from St. Joseph Hospital began the event with various types of available surgeries for breast cancer patients and how they are performed. Then, Dr. Richard Reitherman from Orange Coast Breast Center educated the audience about breast cancer screening options. Dr. Bich-Lien Nguyen lectured on medical treatment options for breast cancer, including chemotherapy, radiation therapy, and hormone therapy. Compassionate Service Society’s instructors concluded the lecture series by demonstrating Integral Taichi and breathing techniques, and explaining the benefits of improving physical and emotional wellness for breast cancer patients/survivors.

The audience’s pleasant smiles were the proof of our great achievement in organizing a successful event.

KOMEN OC & VACF On September 25, 2011 , On September 25, 2011 , On September 25, 2011 , On September 25, 2011 , VACF was able to recruit 117 friends, volunteers, and breast cancer survivors who walked together with VACF's team that day.

It was the first time that VACF’s breast cancer pat ients and surv ivors participated in the Komen Welcome Breast Cancer Survivors' Tribute. Holding a long stem rose, our cancer patients and survivors sat on a full stage along with other cancer patients. Each breast cancer survivor’s face was filled with joy and smiles. It was a very moving event for all of us. We were able to raise $4,375 which exceeded the fundraising goal of $3,000. We also surpassed our recruitment goal of 80 team members by getting 117 Vietnamese people to join the cause.

This year we had the large group b e c a u s e o f t h e n umb e r o f Compassionate Service Soc iety participants. Without CSS, we would not have had such a strong team at the race. The most important achievement was the enthusiasm and the momentum of the participants. Everyone was

walking with the motto “cure for breast cancer.”

Komen tổ chức tại quận Cam ngày 25 tháng 9 năm 2011- HUTVM đã tuyển dụng được 117 bạn bè, tình nguyện viên, và những người đã vượt qua ung thư vú cùng nhau đi bộ với Hội ngày hôm đó. Đây là lần đầu tiên, bệnh nhân đã hoặc đang bị ung thư vú tham gia chương trình của Komen: Chào Mừng, những người đã vượt qua ung thư vú cùng với sự có mặt của bác sĩ Oz. Mỗi bệnh nhân ung thư tham gia với Hội, đều được tặng một nhánh hoa hồng, đồng ngồi trên sân khấu cùng với tất cả các bệnh nhân ung thư vú khác. Lòng tự hào về chặng đường gian nan, đối phó với bệnh ung thư đã và đang vượt qua được thể hiện trên khuôn mặt mỗi người thật tràn đầy niềm vui và nụ cười. Đây là một sự kiện rất cảm động cho tất cả thành viên ngày hôm đó. Hội đã quyên góp được $4,375 và vượt qua mục tiêu gây quỹ $3,000. Chúng tôi cũng đã vượt qua mục tiêu từ 80 thành viên lên đến 117 thành viên Việt Nam ghi danh tham dự vào buổi đi bộ gây quỹ này. Năm nay, Hội đã có một nhóm lớn nhờ số lượng người tham gia của Hội Từ Bi Phụng Sự ( HTBPS ) . Nếu không có HTBPS, chúng tôi đã không thể có được một buổi đi bộ thành công như vậy. Thành tựu quan trọng nhất là sự nhiệt tình và động lực của những người tham gia trong buổi đi bộ gây quỹ nhằm chấm dứt ung thư vú này.

Many of us may think that getting cancer is equivalent to a death sentence, but many have bravely fought and survived. In this issue, we share with you the story of a survivor, Mrs. Thanh Xuan. We celebrate and support her, and encourage our fellow community members to be vigilant about getting screened.

“In 2009, I was diagnosed with

breast cancer. But due to my

residency status, I thought I

couldn’t get treatment under

insurance coverage. I quitted my

job, went back to Viet Nam for 6

months, and didn’t do anything

about my cancer. A year later, I

was luckily able to receive

treatment under Dr. Bich Lien

Nguyen’s care for a month. She

referred me to VACF for additional

assistance.

Ms. Khanh Ly at VACF has always

provided me the most caring and

kind assistance. When I first came

to VACF, I was financially struggling

with my rent and other expenses.

Immediately, I felt welcomed and

sympathetic due to her warmness.

She made me feel that there was

more hope than I could wish for. I

was surprised that there are still

such people with wonderful

goodness. I could call her at

anytime for emotional support.

With her help, I am currently

receiving financial assistance

monthly for a year under 4 different

foundations.


Thanks to Dr. Bich Lien, VACF, and

Khanh Ly, I have found good friends

who I can trust and rely on. I would

vào lần sinh họat này, chúng ta có một vị khách mời đến từ Sở Y Tế Quân Cam

đã hướng dẫn về chế độ dinh dưỡng và

cách ăn uống lành mạnh. Sau đó mọi

người thảo luận và cùng nhau chia sẻ

những cách để tiết kiệm tiền và chọn ra

những thực phẩm tốt cho sức khỏe. Tất

cả mọi nguời đã được thưởng thức một

bữa trưa ngon miệng, và mỗi thành viên

On October 2nd, 2011, we conducted a FREE Hepatitis B & C screeningFREE Hepatitis B & C screeningFREE Hepatitis B & C screeningFREE Hepatitis B & C screening at Orange Coast Memorial Medical Center for 358358358358 individuals, thanks to the generous support of foundations like Bristol-Myers Squibb, Genentech, Gilead, and Vertex. Most people were low-income or uninsured, and without this free screening, they would normally pay more than $200 per person for these tests.

Screening for Hepatitis B & C is important because having chronic hepatitis can lead to liver damage, liver failure, and liver cancer. Viral hepatitis B & C are big issues for our community because the Vietnamese are at high risk for this disease and are 8-10 times more likely than Caucas ians t o be i n f ec t ed . Vietnamese men also have one of the highest rates of liver cancer compared to any other ethnic groups due to the high hepatitis infection rate.

If you are Vietnamese and have never had a Hepatitis B or C blood test done, we highly recommend that you get screened as soon as possible! Do not wait until you have symptoms of liver cancer or damage. Early detection and treatment of Hepatitis B & C will help prevent liver cancer.

Đàn ông Việt Nam có xác suất bị ung thư gan cao nhất so với các sắc dân khác do việc bị nhiễm siêu vi viêm gan cao. Nếu quý vị là người Việt và chưa bao giờ xét nghiệm thử máu viêm gan B hay C, chúng tôi khuyên quý vị nên đi truy tầm càng sớm càng tốt. Đừng chờ đến khi có triệu chứng ung thư gan hay hư gan thì đã quá trễ. Phát hiện và chữa trị viêm gan B và C sớm sẽ ngăn ngừa được ung thư gan.

We had three doctors and a physician assistant to provide free consultations for four women, CBE screenings for 44 women, Ultrasound referrals for seven women, and mammogram screening referrals for 27 women. Dr. Thuy-Anh Nguyen, Dr. Chi Phuong Phan, Dr. Bich Lien Nguyen conducted self breast awareness workshops and distributed cancer awareness educational materials to these 44 women.

Thank you Ms. Thu Nga, a Hospital Liaison of OCMMC, for setting up the screening in an organized fashion.

We gave each pat ient b reast educational materials. Everyone was patient and happy to receive their free screenings. All patients were very pleased with the services from the doctors and staff member.

SHOULD WE USE PSA TO SCREEN FOR PROSTATE CANCER?


Prostate cancer screening with PSA

(Prostate Specific Antigen) test recently

stirred up one of the most controversial

topics among medical community. Up

until now, there are few large

randomized, controlled clinical trials

examining the absolute benefits and

risks of using PSA to screen for prostate

cancer, however, they have not reached

any consensus yet. On October 7,

2011, a panel of physicians (Family

doctors, Pediatrician, Obstetrics/

G y n e c o l o g i s t , S t a t i s t i c i a n ,

Epidemiologist) from U.S. Preventative

Services Tasks Force proposed a “draft”

recommendation of not using PSA as

screening test for healthy men1. This

statement has provoked a mixed

response and created some confusion

not only among medical community, but

also among patients and cancer

support groups themselves. The

proponents for PSA screening test

attribute this to the reduction of

prostate cancer mortality due to early

detection. The opponents, on the other

hand, criticize that PSA screening can

l e a d t o o v e r d i a g n o s i s a n d

overtreatment of prostate cancer with

associated risks of treatments. To

understand this controversial issue, we

need to understand PSA test as well as

the natural history of prostate cancer.

Prostate cancer is the second leading

cancer in men in U.S. According to

cancer statistic in 2010, there are

approximately 217,730 new cases

diagnosed and 32,050 deaths

specifically from prostate cancer2.

Majority of patients with prostate

cancer have no symptoms at all. If we

rely only on symptomatology to detect

prostate cancer, by the time it is

diagnosed the cancer is usually in an

advanced stage. Prior to 1987, when

PSA test was first invented and

In fact, after 1987 more patients with

prostate cancer were diagnosed, and

most of cancer was detected in an early

stage.

Prostate cancer can occur in any men,

even as early as 40 years of age. In

general, the older the patient, the more

likely he will harbor prostate cancer.

Most common age when prostate cancer

detected is from 60-70. As any type of

cancer, when prostate cancer is

diagnosed at an early age (<60), it tends

to progress aggressively, and mortality

from cancer will increase as compared to

those diagnosed later in life. What it

means is that patient with prostate

cancer detected when they are young

may likely succumb to the disease. As

we know, even with widespread

screening and treatment, more than

32,000 patients may die specifically from

prostate cancer. The mortality rate would

be in the range of 40,000 or more if

prostate cancer is not early diagnosed or

treated. On the contrary, not everyone

with prostate cancer will die from cancer

itself. Currently, we do not have any

perfect test which helps to differentiate

between groups of patients who should

be treated versus those who should be

observed.

PSA test does help to detect prostate

cancer at an early stage. However, if PSA

is used alone as a screening test, some

cancers will be missed. Physicians do

need to combine digital rectal exam

together with PSA test in order to help

diagnosis. PSA is a Serine Protease

produced by prostatic epithelium in male,

secreted in a small amount in serum. In

serum, PSA exists in 2 forms: Free PSA

(unbound) and Complexed PSA (bound

with another protein). Most of the

time, PSA result reported is Total PSA

(Free + Complexed). PSA test will not

tell whether or not we have prostate

cancer. Normal value of PSA is usually

less than 4ng/ml. The higher the PSA

test is, the more likely the prostate

cancer will be detected. However,

having high PSA does not mean that

you have prostate cancer. There are

many factors that might cause

elevated PSA, but not related to

cancer, such as:

UTI or bladder infection

Prostatitis

Prostatic abscess

Urinary retention or having

indwelling Foley catheter

Benign Prostatic Hyperplasia

After having Cystoscopy done

After having prostate biopsy done

On the contrary, if you use Proscar,

Avodart, Jalyn or Saw Palmetto, your

PSA will be reduced in half. So the

true PSA value if you are on these

medications will be doubled the

laboratory reported result. As a

consequence, your doctor should

factor in these when using PSA a

screening tool. Despite the fact that it

is not a perfect test, PSA together with

digital rectal exam are the only

available tools helping physician to

screen for prostate cancer at this time.

If it is suspicious, prostate biopsy is the

following test to confirm whether or not

you have prostate cancer.

The debatable topic regarding using PSA as a screening test in medical community is based on 2 large randomized, controlled trials. Both of these trials gave contradictory results. The first trial is from European

countries (The European Randomized Study of Screening for Prostate Cancer) published in 20094. In this trial, 182,160 patients from the age range from 50-74 were enrolled in the study. The second trial was done in U.S. (The Prostate, Lung, Colon, and Ovary trial of the National Cancer Institute)5. There were 76,693 patients enrolled in this study. In reality, both of these studies do have minor flaws such that the results can be interpreted differently if these contaminating factors were eliminated from the study. As we all know, screening for prostate cancer depends not only on PSA test, but also on digital rectal exam. In addition, many factors such as age, race, genetic factors, etc... need to be considered in screening and diagnosis of prostate cancer. Even with PSA, physician has to rely on other factors such as the trend of rising PSA in order to decide whether prostate biopsy needs to be done. The American Urological Association advised that men with 10 year life expectancy who wish to be screened should undergo baseline testing at age of 406. The decision to screen with PSA or not should be discussed with physician regarding the risks and benefits of screening. Diagnosis with prostate biopsy or treatments for prostate cancer should also be discussed in detail with urologist or cancer specialist. The American Cancer Soc iety, on the other hand, recommended that men with average risk of prostate cancer should discuss screening beginning at age 50 years. African American men and men with a first-degree relative diagnosed at an early age should have this discussion at age 45 years. Men with high risk for prostate cancer (several first-degree relatives diagnosed at an early age) should start talking to their physician about prostate cancer screening at age

40 years. Men with PSA < 2.5ng/ml should be tested every 2 years, and those with PSA > 2.5mg/ml should be tested annually7. Despite widely different opinions regarding recommendation for using PSA test to screen for prostate cancer or interpretations of results of trials, most will agree that: •Population based screening is not recommended.

•Before screening for prostate cancer, patient should understand its implications, risks as well as benefits.

•If having prostate cancer, treatment modalities should be discussed in detail with physician (especially Urologist, Medical Oncologist or Radiation Oncologist).

•Not everyone with prostate cancer should be treated. Decision to screen, diagnose or to treat should be individualized. At this time, effort to improve screening with better marker than PSA is under investigation. However, no single perfect test to screen for prostate cancer is detected yet.

D r . T ha i Van Ngu yen i s a gastroenterologist practicing in Los Angeles, California. She was one of the co-founders of VACF, and has been a board member since our formal beginning in 2002. She has been the leader of our extremely successful hepatitis/liver cancer program. Under her leadership, and with the capable assistance of Tanh Phan, the program has screened hundreds of people for hepatitis B and C, and has helped many to obtain very expensive drugs to treat hepatitis for free. The program also gave educational symposia as well as outreach program for the c ommu n i t y a n d f o r h e a l t h professionals, and a very successful hepatitis support group.

I am very happy that Dr. Thai Van Nguyen has accepted the responsibility of being the head of VACF. I envision great growth of this organization under her leadership, and am looking forward to continuing working with her and the rest of the board of directors to bring VACF to a new level.

một vấn đề đang tranh cãi sôi nổi

trong thế giới y khoa gần đây. Cho tới

bây giờ đã có vài nghiên cứu lớn

(trials) để xem những điều lợi hay hại

liên quan đến việc truy tầm ung thư

nhiếp hộ tuyến bằng thử nghiệm PSA,

thì kết quả cũng chưa phân biệt rõ

ràng như thế nào cả. Vào ngày 7

tháng 10, một nhóm bác sĩ của chính

phủ Hoa Kỳ (U.S. Preventative Services

Task Force) gồm các bác sĩ Gia Đình,

Nhi Khoa, Sản Phụ Khoa, Dịch Tể Học,

nhà nghiên cứu thống kê sau khi xem

xét những tài liệu y khoa đã đưa ra

một đề nghị “sơ khởi”, khuyên không

nên làm PSA để truy tầm Ung Thư

Nhiếp Hộ Tuyến1. Lời đề nghị này gây

hoang mang cả thế giới y khoa, và

ngay cả bệnh nhân. Nhóm ủng hộ thử

nghiệm PSA cho rằng PSA giúp làm

giảm tỷ lệ tử vong do tìm và chữa ung

thư sớm. Ngược lại, nhóm chống đối

thì cho rằng PSA có thể đưa đến việc

tìm quá nhiều ung thư không cần thiết

và chữa trị một số ung thư không cần

phải chữa trị. Vả lại, việc chữa trị

cũng có thể gây những hậu quả không

tốt. Để hiểu rõ những lời tranh cãi

này, trước nhất chúng ta cần hiểu về

bệnh Ung Thư Nhiếp Hộ Tuyến và thử

nghiệm PSA.

Ung Thư Nhiếp Hộ Tuyến là một loại

ung thư đứng hạng thứ hai trong nước

Mỹ. Theo xác suất thống kê vào năm

2010, có khoảng 217,730 ca ung thư

được phát hiện, và khoảng 32,050

người bị tử vong về ung thư này2. Ung

Thư Nhiếp Hộ Tuyến đa số không có

một triệu chứng gì rõ ràng cả để giúp

ta tìm bệnh sớm. Nếu chỉ dựa vào triệu

chứng không thôi thì khi phát hiện bệnh

1987, nhờ vào thử nghiệm PSA, số

người bị Ung Thư Nhiếp Hộ Tuyến được

chẩn đoán nhiều hơn và khi chẩn đoán

thì ung thư đa số ở thời kỳ vừa chớm nở

(Localized). Ung Thư Nhiếp Hộ Tuyến là một loại ung

thư có thể xảy ra ở mọi lứa tuổi, ngay cả

ở những bệnh nhân khoảng 40 tuổi.

Thông thường khi càng lớn tuổi thì xác

suất bị ung thư sẽ càng cao hơn. Lứa

tuổi hay thường bị là từ 60-70. Nhưng

cũng giống như những loại ung thư

khác, khi ung thư phát hiện ở tuổi càng

trẻ (<60) thì bệnh có khuynh hướng phát

triển càng lẹ, tử vong càng cao so với

những lứa tuổi khác. Điều đó có nghĩa là

cả khi truy tầm và chữa trị, có đến hơn

32,000 người có thể chết vì Ung Thư

Nhiếp Hộ Tuyến. Con số tử vong này có

thể sẽ lên đến 40,000 hoặc hơn nếu

không tìm và chữa bệnh sớm. Ngược

lại, không phải tất cả bệnh nhân nào bị

Ung Thư Nhiếp Hộ Tuyến đều bị tử vong

do ung thư. Cho tới bây giờ, chúng ta

vẫn chưa có một thử nghiệm nào chính

xác để giúp phân biệt nhóm bệnh nhân

nào nên theo dõi, và nhóm bệnh nhân

nào nên chữa trị khi bị ung thư.

Thử nghiệm PSA có thể giúp tìm bệnh

sớm hơn. Nhưng nếu chỉ dựa vào PSA

không thôi thì cũng không đủ, Bác Sĩ cần

phải khám nghiệm hậu môn (Digital

Rectal Exam) để so sánh và chẩn đoán.

PSA là một chất Protein (Serine Protease)

tạo ra từ nhiếp hộ tuyến, thải ra trong

máu. PSA có 2 dạng. Một là Free PSA

(dạng tự do), hai là Complexed PSA (gắn

vào một protein khác). Đa số thử

nghiệm PSA ta hay nghe nói là thử

nghiệm Total PSA (Free + Complexed).

PSA không cho ta biết là có ung thư

hay không. Trung bình PSA là dưới

4ng/ml. Nếu PSA càng cao, xác suất

ung thư càng cao. Điều đó không có

nghĩa là ai có PSA cao đều có bị Ung

Thư Nhiếp Hộ Tuyến. Có một số

trường hợp PSA cao không phải là do

ung thư như:

Nhiễm trùng đường tiểu (UTI,

Bladder infection)

Nhiễm trùng nhiếp hộ tuyến

(Prostatitis)

Áp se (túi mủ) nhiếp hộ tuyến

(Prostatic abscess)

Bị bí tiểu (Urinary Retention) phải

đeo ống thông tiểu

Bệnh nhiếp hộ tuyến nở lớn

(Benign Prostatic Hyperplasia)

Sau khi làm soi bọng đái

(Cystoscopy)

Sau khi sinh thiết nhiếp hộ tuyến

(Prostate Biopsy)

Ngược lại có một số trường hợp PSA

có thể giảm xuống như dùng thuốc

Proscar, Avodart, Jalyn, Saw Palmetto.

Đối với những trường hợp này, thì số

PSA thật sự sẽ là gấp đôi. Cho nên,

khi dùng PSA để truy tầm ung thư

trong những trường hợp này, bác sĩ

cần phải hiểu rõ tác dụng thuốc và

những sự thay đổi khi xử dụng thuốc

để giúp chẩn đoán bệnh.

Hiện giờ PSA là một thử nghiệm duy

nhất có thể giúp truy tầm Ung Thư

Nhiếp Hộ Tuyến. Mặc dù thử nghiệm

PSA không là một thử nghiệm tuyệt

đối để cho biết là có ung thư hay

không, nhưng nó vẫn giúp tìm được

ung thư sớm, khi chưa có triệu chứng

gì cả. Thử nghiệm PSA và khám

nghiệm tay là 2 yếu tố giúp bác sĩ truy

tầm ung thư.

hay không. Vấn đề bàn bạc tranh cãi lúc này trong

thế giới Y khoa là dựa trên hai cuộc

nghiên cứu lớn (large, randomized,

controlled trials). Cả hai đưa ra kết quả

hoàn toàn trái ngược với nhau. Cuộc

nghiên cứu thứ nhất làm bên những

nước Âu châu (The European

Randomized Study of Screening for

Prostate Cancer) đã ấn hành vào năm

20094. Trong cuộc nghiên cứu này có

tới 182,160 bệnh nhân tham gia (lứa

tuổi từ 50-74). Kết quả cho thấy

những bệnh nhân được truy tầm bằng

PSA, có tới 20% giảm thiểu về tử vong

do ung thư so với những bệnh nhân

không dùng PSA để tìm bệnh. Thử

nghiệm thứ hai làm bên nước Mỹ (The

Prostate, Lung, Colon and Ovary trial of

the National Cancer Institute)5. Trong

cuộc nghiên cứu này có tới 76,693

bệnh nhân tham gia. Kết quả cho thấy

không có khác biệt về tử vong trong hai

nhóm (có và không có dùng PSA để truy

tầm). Cả hai thử nghiệm lớn này thực

tế đều có một chút khuyết điểm trong

cách nghiên cứu. Cho nên nếu phân

tích kỹ ra thì chưa thể phân biệt ai

đúng ai sai.

Việc truy tầm Ung Thư Nhiếp Hộ Tuyến

phải dựa vào không những thử nghiệm

PSA, mà còn dựa vào khám nghiệm tay

(Digital rectal exam). Ngoài ra còn

nhiều yếu tố phụ thuộc khác như di

truyền, sắc tộc, tuổi tác v.v… cần được

xem xét. Ngay cả với PSA, bác sĩ

chuyên khoa cũng dựa vào nhiều yếu

tố khác như khuynh hướng (trend) gia

tăng PSA để quyết định có nên làm sinh

thiết hay không. Hội Niệu Khoa Hoa Kỳ

(American Urological Association) đưa

ra lời hướng dẫn (guidelines) khuyên ở

người đàn ông có khả năng sống hơn 10

năm, nếu muốn truy tầm ung thư nên

làm PSA từ 40 tuổi trở lên6. Quyết định

nên thử PSA hay không cần nên tham

khảo với bác sĩ của mình để hiểu rõ

những điều lợi hại. Việc chẩn đoán

bệnh bằng cách sinh thiết hay chữa trị

bệnh ung thư nếu có nên tham khảo với

bác sĩ niệu khoa. Hội Ung Thư Hoa Kỳ

(American Cancer Society) khuyên những

bệnh nhân có nguy cơ bị ung thư nên

tham khảo với bác sĩ từ 50 tuổi trở lên.

Người Mỹ gốc Phi Châu và bệnh nhân có

anh em hoặc bố bị Ung Thư Nhiếp Hộ

Tuyến nên tham khảo với bác sĩ để truy

tầm PSA từ 45 tuổi trở lên. Nếu nhiều

anh em bị ung thư, thì nên tham khảo

khi 40 tuổi trở lên. Bệnh nhân có PSA <

2.5ng/ml nên thử nghiệm 2 năm 1 lần.

Bệnh nhân có PSA > 2.5ng/ml nên thử

nghiệm PSA mỗi năm 1 lần7.

Mặc dù có những ý kiến khác nhau về

việc sử dụng PSA để truy tầm ung thư,

có một vài điều mọi người đều đồng ý là:

•Không phải tất cả bệnh nhân đều cần truy tầm Ung Thư Nhiếp Hộ Tuyến

•Trước khi truy tầm Ung Thư Nhiếp Hộ Tuyến, bệnh nhân cần hiểu rõ cái lợi hại của việc truy tầm

•Nếu bị ung thư thì việc chữa trị cũng cần được tham khảo với bác sĩ rõ ràng

•Không phải ai bị Ung Thư Nhiếp Hộ Tuyến đều phải chữa Quyết định tìm bệnh, chẩn đoán bệnh

hay chữa bệnh đều dựa vào từng trường

hợp cá nhân của mỗi người bệnh nhân.

Việc này bệnh nhân cần tham khảo với

bác sĩ chuyên khoa

Hiện tại, có rất nhiều nghiên cứu để tìm

một thử nghiệm nào đó tốt hơn PSA.

Cho tới bây giờ thì khoa học vẫn chưa

tìm ra một thử nghiệm lý tưởng để truy

tầm Ung Thư Nhiếp Hộ Tuyến.

nhu cầu liên quan đến ung thư. Chúng tôi

may mắn có được những nhân viên tận

tâm và siêng năng làm việc cùng các thiện

nguyện viên dưới sự lãnh đạo của Ban Hội

Đồng, và giám đốc điều hành đầy nhiệt

huyết Tánh Phan. Đặc biệt, đây là niềm vui

lớn của tôi để chính thức giới thiệu đến tất

cả quý vị người Chủ tịch Hội Đồng của chúng tôi, Bác sĩ Nguyễn Thái Vân.

Bác sĩ Nguyễn Thái Vân là một chuyên gia

trị liệu về đường tiêu hóa ở Quận Los

Angles, California. Cô là một trong những

người đồng sáng lập hội UTVM, và cũng là

một thành viên của ban Hội Đồng Quản Trị kể từ khi chúng tôi chính thức thành lập

Hội trong năm 2002. Cô là một nhà lãnh

đạo cực kỳ thành công về lãnh vực siêu vi

gan của chương trình viêm gan B và C,

cùng ung thư gan của Hội. Dưới sự lãnh

đạo của bác sĩ Thái Vân, với sự hỗ trợ đặc

biệt của Tánh Phan, chương trình đã sàng

lọc hàng trăm người bệnh viêm gan B và

C, và đã giúp rất nhiều bệnh nhân để có

được các loại thuốc rất đắt tiền để điều trị viêm gan miễn phí. Chương trình cung cấp

các buổi hội nghị chuyên đề giáo dục cũng

như các chương trình tiếp cận cộng đồng,

cho các chuyên gia y tế, và nhóm hỗ trợ

bệnh viêm gan rất thành công.

Tôi rất vui mừng được bác sĩ Nguyễn Thái

Vân chấp nhận trách nhiệm là người đứng

đầu của HUTVM. Tôi hình dung sự phát

triển của tổ chức này dưới sự lãnh đạo của

bác sĩ, và mong muốn được tiếp tục làm

việc với cô ấy cùng toàn thể thành viên

khác trong ban Hội Đồng Quản Trị nhằm

Due to hard times, many free state-funded programs are cutting costs and changing their requirements, leaving many of our community members without free or affordable cancer-prevention services. VACF is seeing more clients come in with cancer concerns but without the means to get screening or diagnostic services. We are thus starting a campaign devoted specifically to raise funds to help those who do not qualify for any free or existing programs or services. The Sharing Hope Fund will help subsi-dize a portion of much-needed medical services for low-income uninsured community members with cancer concerns who are unable to get help from existing or available resources, such as a diagnostic mammogram, an ultrasound or hepatitis B & C tests. By donating to the Sharing Hope Fund, you can directly help save the lives of community members who have no access even to safety-net services. Even a small amount helps.
