Why use the DMC?

Each evaluator is responsible for documenting information related to the three child outcomes areas:

1. Positive social emotional skills

2. Acquiring & using knowledge & skills

3. Using appropriate behavior to meet needs

Evaluator’s Role

The evaluator is responsible for ensuring that all professionals involved in the child’s multidisciplinary evaluation collaborate to complete and submit the DMC (if chosen).

If the child is found to be not eligible for the EIP, the DMC is not required.

However, you should document the procedures conducted and the child’s skills and abilities in the child outcomes areas assessed.

DMC - Evaluator Edition

Behaviors typically present in each of the three outcome areas from birth to 36 months are listed in the DMC.

Evaluate whether or not the child has attained each milestone by selecting yes, no, or inconsistent.

The rating on this scale can be the result of direct observation, parent report, or extrapolation.

The developmental milestones are listed starting with 3 months and ending at 36 months for each outcome.

The DMC begins with behaviors and skills related to Outcome 1.

When completing the DMC, pay close attention to the age, the behaviors expected of that age, and to which Outcome

the behaviors belong.

Completing the DMC

Check ONE box under the heading titled “Has Child Attained Milestone?” for each behavior or skill listed.

Rating (checkbox) What it means

Yes the child typically displays the behavior or skill across different settings and situations.

No the child does not display the behavior or skill.

Inconsistentthe child displays the behavior or skill in SOME situations or settings but NOT ALL situations or

settings.

Completing the DMC

Direct Observation* If the behavior was directly assessed or observed during the assessment session.

Parent Report*If the rating was based on information provided by the parent or caregiver during the

assessment session.

ExtrapolationIf the rating was based on clinical judgment and not directly observed or reported by

the parents or caregivers.

*Specify the Number Codes of tests used at the bottom of the checklist, if applicable.

NOTE: You may check as many boxes as apply to that rating.

the end of the DMC (see below).
