Nicotine Use of any tobacco or nicotine product within the past 12 months. Only exception is occasional cigar usage, no more than 1 occasion per month and cotinine is negative.

Nicotine Warning Tobacco and nicotine users have demonstrated significantly higher mortality based on insurance company statistics. The Chesapeake Life Insurance Company will treat misrepresentation of the tobacco or nicotine use question in the same manner as any significant misrepresentation. If discovered during the contestable period The Chesapeake Life Insurance Company will seek to rescind the Policy and deny the entire death benefit. Blood Tests All blood tests are to include a Home Office urine Specimen. Blood Chemistry Profiles are to be drawn by an approved paramedical service.

Agent must see all insureds when application is completed.

Proposed Insured Signature – Only the Proposed Insured may sign on the “Proposed Insured Signature” line. Legal Guardian should sign application if the insured is age 15 or under. No one else may sign the Proposed Insured’s name. If Proposed Insured signs with an “X” or prints their name, a witness must sign “witnessed by” along with their signature and printed name. The witness must be someone other than the writing agent, a minor, the beneficiary, Power of Attorney, policyowner or spouse (spouse cannot sign as witness due to having an insurable interest in the policy). If an applicant is from another country and has U.S. residential status and a Social Security Number, his/her signature should be in their native language and they should not sign with an “X”. Attach a note to the application citing reason for signing with an “X”. A request for an APS should be expected. A Power of Attorney signature will not be accepted for either insured or as a witness.

If an applicant is unable to write his or her own signature, he or she can make an “X” mark on the signature line. The Agent must then write the name beside the mark, showing the first name to the left of the mark and the last name to the right of the mark. The Agent should also write “His” or “Her” above the mark and the word “Mark” below the mark.

A policy can be backdated up to 6 months (3 months in Ohio) to save age if the back premium is collected. Backdating is allowed to save premium, not for eligibility of a policy or rider.

Express App Underwriting Process The process for Medically Underwritten Business is designed to allow the agent to concentrate on presenting the products, preparing a needs analysis, and closing the sale. At the end of the sale, the agent completes the application process by contacting EMSI for the Health Question Interview. 2 Page Application Agent will complete the 2 page application, which contains applicant information, product selection, face amount, etc., on the first page and then obtain all necessary signatures on the second page. Health Question Interview by Service Provider Once the application is completed, the agent should call EMSI at 888-835-7037 for the applicant to complete the Health Questions, which is Part A of the application. A Voice Signature will be accepted on Part A. If the applicant is unable to complete the Health Questions at the Point of Sale, the agent should ask the applicant for the best time EMSI could contact them. The agent should document the day and time on the 2 Page Application, and indicate that the interview has not been completed. Once the application is received in the Home Office, the Health Question interview will be ordered. As soon as the Health Question Interview is completed, it will be electronically sent to the Home Office. Please note that the Health Question Interview will be recorded. Paramed Exam Scheduled Once the Health Questions are completed, EMSI will schedule the applicant for the paramed exam. EMSI will target to have the exam scheduled within 72 hours of the time of the call. The Paramed Exam will consist of photo identification, blood pressure, height, weight, pulse, observation of physical condition, measurements, and will draw fluids (urine or blood as required for the age and face amount). Laboratory Results The paramed will overnight the fluid(s) to the Laboratory as soon as the exam is completed. Lab Results are electronically sent to the Home Office in 24 hours.

The Guaranteed Level Term Plans offer your clients affordable term life insurance coverage to help meet their personal or business needs. With term coverage to age 95, your clients will have the choice of initial guaranteed level premium periods: 10, 15, 20, or 30 years. In addition to low cost protection, the Policy has built in guaranteed renewability to age 95, regardless of health.

Terminal Illness Accelerated Benefit Provision

This benefit is automatically included in the Policy where available.

This benefit allows the Insured to access a portion of the Policy death benefit in the event the Insured is diagnosed with a terminal illness where life expectancy is 12 months (24 months in MA) or less. The election percentage shall be no greater than 50% of the Policy’s death benefit, with a maximum of $250,000. Death Benefit Fixed Income Option This settlement option is designed to provide flexibility in how the death benefit will be paid. The Policy Owner can select a combination of lump sum and monthly income options to provide guaranteed monthly income to the beneficiary instead of the entire death benefit paid at the time of death. Conversion Privilege The Policy can be converted to a C-2 Universal Life policy offered by The Chesapeake Life Insurance Company. The conversion may take place at any time prior to the earlier of the insured’s age 70 or the end of the initial level term period and is subject to the issue age and size limitations of the new Policy.

Return of Premium Rider

Now you have the perfect benefit for your clients who need the temporary Death Benefit protection offered by term life insurance and are also interested in receiving a refund of premiums. The Policy may be surrendered during the initial term period for a refund of premiums paid based on the number of years the Policy is in force (refer to schedule on page 26).

This extra benefit Rider provides for the return of the annual premiums paid, for the Policy and any additional riders, for the initial term period. This Rider will terminate upon Policy renewal however, the benefit (return of premiums paid during the initial term period) will continue throughout the life of the Policy.

If, however, the client chooses to terminate the Policy prior to the end of the initial term period, only a portion of the premiums will be returned. (refer to schedule on page 26).

All refunds will be decreased by any prior amount paid by Us under the Policy or any of its Riders.

Waiver of Premium Issue ages: 18-55 Not available on Substandard Policies This rider waives the premium due if the Insured becomes totally disabled. Until the Insured has been disabled for a continuous period of 6 months premiums must be paid. However, we will refund the six months of premiums paid once the claim has been approved. Unless the Insured is disabled, coverage will cease on the Policy anniversary on or after the Primary Insured’s 60th birthday.

Children’s Term Rider Available at issue for children age 15 days through age 17 years. Premium for this Rider is $6.00 per unit annually. The minimum benefit is 1 unit ($1,000) and the maximum benefit is 25 units ($25,000).

This Rider provides term insurance coverage for all children of the Insured and future children born or legally adopted. Coverage lasts to the child’s 25th birthday.

Upon termination, this Rider may be converted without evidence of insurability to any permanent life plan (other than term insurance) that is being issued by us and subject to the provisions in the Rider. The new Policy may be in an amount up to five times the Rider benefit to a maximum of $50,000. At conversion, any increase in the death benefit would require underwriting. All children proposed for coverage should be seen at time of application or an explanation should be submitted as to why they were not.

Accidental Death Benefit with Common Carrier to Age 75 Issue ages: 18-65 Not available on Substandard Policies Maximum Issue Limit: $300,000 Premium: $0.91 per $1000 annually Coverage will cease on the Policy anniversary on or after the Insured’s 75th birthday.

Disability Income Rider Issue ages 18-50 Not available on Substandard Policies

After a 90 day elimination period, this Rider provides a monthly benefit for up to 2 years while the Insured is totally disabled.

Premiums for this Rider are guaranteed for 1 year. The maximum monthly benefit is the lesser of $1,800, or 1.5% of the face amount. Premiums are based on number of $100 units of monthly benefit. This rider expires at age 60 however, if the disability occurs prior to age 60 benefits can be paid until age 62.

This Rider will only be active during the Initial Term Period. Premiums and benefits will terminate upon Policy renewal.

Refer to your Disability Income Rider Occupation Listing for details on the qualifying occupations for this Rider.

Not available for Self Employed.

Critical Condition Rider Issue ages: 18-65 Not available on Substandard Policies

Upon receipt of proof of the first occurrence of a qualifying event, we will pay an accelerated benefit in a lump sum benefit amount. The Policy Owner can choose either a 25% or 50% benefit amount. This Rider will terminate after we have paid either 25% or 50%, depending on the benefit amount selected, of the original base Policy benefit in critical condition benefits. The remaining base Policy benefit will be adjusted according to the amount paid. No Rider (or combination of Riders) can accelerate more than 50% of the original base Policy benefit with a maximum of $250,000 payable.

This Rider will be made a part of the Policy contract for the Initial Term Period only. Premiums and benefits will terminate upon Policy renewal. The Death Benefit in the Policy will be reduced by any amount paid under this Rider.

Death Benefit Fixed Income Option This settlement option is designed to provide flexibility in how the death benefit will be paid. The Policy Owner can select a combination of lump sum and monthly income options to provide guaranteed monthly income to the beneficiary instead of the entire death benefit paid at the time of death.

Partial Surrender A Partial Surrender may be made on any monthly anniversary date after the first Policy year. The minimum withdrawal is $500.00 and a $25.00 processing fee will be charged. A pro rata surrender charge will also be deducted if applicable. The amount of the Partial Surrender cannot exceed the amount you would receive if you surrender the Policy.

Policy Loans Policy loans, in an amount that does not exceed the accumulation value less any surrender charge, are available any time.

The loan interest rate is 7.4%. On each Policy anniversary, the loan interest must be paid in advance. At the time the loan is made, interest for the year will be deducted from the loan amount received. Interest not paid when due will be added to the loan and will bear interest.

After the Policy has been in force for 10 years, the current interest rate credited to the amount borrowed will equal the loan interest rate, providing the new loan made does not exceed 10% of the account value.

C-100 The C-100 is a flexible premium, adjustable Universal Life contract that will mature at age 100. It offers the flexibility of Universal Life insurance and the guarantees of Whole Life.

The C-100 features level premiums to age 100 in a permanent life insurance plan. Like most Universal Life policies, it also offers flexible premiums, flexible death benefits, and in most cases, an income tax-free death benefit to beneficiaries. Cash values can accrue if your client chooses to pay above the required minimum premium.

When your clients ask for flexibility and security with affordable premiums... you have the answer with the C-100 UL.

No Lapse Guarantees

Minimum Monthly Premium - 15 Year No Lapse Guarantee The premiums and death benefits will be guaranteed for 15 years. This policy will not enter into the grace period if on any monthly anniversary date the sum of the premiums paid, less any indebtedness and partial withdrawals, equals or exceeds the sum of all minimum monthly premiums beginning with the Policy date.

Extended No Lapse Guarantee to age 100 After the Minimum Monthly Premium (15 Year No Lapse Guarantee), the policy is guaranteed to remain in force to age 100 if the Extended No Lapse Guarantee Premium payment requirements are met.

Catch-Up Provision - Starting on the 15th Policy Anniversary and every fifth anniversary following, the Policy Owner must pay the difference between premiums paid less any indebtedness and partial withdrawals, and the sum of Extended No Lapse Guarantee Premiums due. If the Catch-Up premiums are not paid on these anniversaries, the Extended No Lapse Guarantee will no longer be in effect. Surrender Charge The surrender charge decreases to 0 at the beginning of Policy year 20. Per Unit Expense Charge The guaranteed maximum monthly expense charge will vary by issue age and rate class. Premium Load A guaranteed maximum of 5.0% level percent of premium load will be charged in all years.

Terminal Illness and Extended Maturity Date benefits are

Terminal Illness Accelerated Benefit Provision This benefit allows the insured to access a portion of the Policy death benefit in the event the insured is diagnosed with a terminal illness where life expectancy is 12 months or less (24 months in Massachusetts). The election percentage shall be no greater than 50% of the Policy’s death benefit, with a maximum of $250,000.

Extended Maturity Date At any time before the maturity date, the Policy Owner may elect to extend the maturity age of 100. This Rider provides that after the Policy’s original maturity date:

• The death benefit will be the greater of the death benefit on the maturity date or the cash surrender value multiplied by 1.01. This amount will reflect a deduction for any outstanding indebtedness.

• No monthly deductions for COI or administrative fees will be made.

• Interest will continue to be credited to the Policy’s accumulation value.

• Policy loans will continue to accrue interest, loan payments will be accepted.

• Policy premium will no longer be accepted.

The Policy will remain in force until the death of the Insured, or the date that we receive written request to surrender the Policy.

This provision may not be exercised if it would disqualify the Policy as life insurance under the Internal Revenue Code.

Accidental Death Benefit with Common Carrier to Age 75 Issue ages: 0 to 59 Not available on Substandard Policies Maximum Issue Limit: $300,000 Coverage will cease on the Policy anniversary on or after the Insured’s 75th birthday.

Accidental Death Benefit If the Insured on this Policy dies within 90 days after an injury, while coverage under the Policy and this Rider is in force, the beneficiary will receive a total benefit amount equal to 200% of the Death Benefit in the Policy.

Common Carrier Accidental Death Benefit If the Insured on this Policy dies within 90 days after an injury which was sustained while riding as a fare paying passenger on a common carrier, while coverage under the Policy and this Rider is in force, the beneficiary will receive a total amount equal to 300% of the Death Benefit in the Policy.

The amount of ADB coverage is limited to a base face amount of $300,000. Children’s Term Rider Available at issue for children age 15 days through age 17 years.

This Rider provides term insurance coverage for all children of the Insured and future children born or legally adopted. Coverage lasts to the child’s 25th birthday. All children proposed for coverage should be seen at time of application or an explanation should be submitted as to why they were not.

Upon termination, this Rider may be converted without evidence of insurability to any permanent life plan (other than term insurance) that is being issued by Us and subject to the provisions in the Rider. The new Policy may be in an amount up to five times the Rider benefit to a maximum of $50,000. At conversion, any increase in the death benefit would require underwriting.

Annual Rates Premium for this Rider is $5.00 per unit annually. The minimum benefit is 1 unit ($1,000) and the maximum benefit is 25 units ($25,000).

Critical Condition Rider C-2 only Issue ages: 0-65 Not available on Substandard Policies Upon receipt of proof of the first occurrence of a qualifying event, We will pay an accelerated benefit in a lump sum benefit amount. The Policy Owner can choose either a 25% or 50% benefit amount. This Rider will terminate after we have paid either 25% or 50%, depending on the benefit amount selected, of the original base Policy benefit in critical condition benefits. The remaining base Policy benefit will be adjusted according to the amount paid. No Rider (or combination of Riders) can accelerate more than 50% of the original base Policy benefit with a maximum of $250,000 payable.

The Death Benefit in the Policy will be reduced by any amount paid under this Rider.

Bank Draft Date A premium will be drafted on the same day of each month. A specific draft date may be requested by the applicant. We cannot honor requests to draft monthly premiums on the 29th, 30th, or 31st of any month.

Important Dates

* 3-day dating applies to Universal Life only.

Application Date Applications must be dated the day the application is completed; not the day it is sent to the Home Office nor the date the insurance is to become effective.

Staledated Application An application dated 30 or more days prior to receipt will require a new application to be written with a current date.

Policy Effective Date Upon Underwriter approval, a Policy will become effective 3 days after the bank draft date on a Universal Life policy; same day of bank draft date for a Term policy.
