Both processes of Exporting and Uploading clinical exchange information will be explained, beginning with the Export process. EXPORTING CLINICAL EXCHANGE INFO The Export Clinical Exchange Info link will allow you to create and send a CCD, or Continuity of Care Document, file. To begin, go to the Patient Charts Tab, and select a patient. Hover over the Others link, and select Export Clinical Exchange Info. This will take you to the Export Clinical Exchange Information page. The Document type should say CCD, and be grayed out. Select which CCD Sections you want to export by checking the boxes next to the desired sections. You can also select a date range to narrow the CCD. Click Preview.

CHART EXPORT/UPLOAD INSTRUCTIONS OVERVIEW To ensure Office Ally users can meet the Meaningful Use requirements for exchanging clinical and patient information securely, we have made it possible to securely export and upload patient chart information. Both processes of Exporting and Uploading clinical exchange information will be explained, beginning with the Export process.

EXPORTING CLINICAL EXCHANGE INFO The Export Clinical Exchange Info link will allow you to create and send a CCD, or Continuity of Care Document, file. To begin, go to the Patient Charts Tab, and select a patient.

Hover over the Others link, and select Export Clinical Exchange Info.

This will take you to the Export Clinical Exchange Information page. The Document type should say CCD, and be grayed out. Select which CCD Sections you want to export by checking the boxes next to the desired sections. You can also select a date range to narrow the CCD. Click Preview.

The Continuity of Care Document (CCD) will display. From this page, you can click Back to Patient Summary to go back to the patient’s chart, or Select CCD Sections to go back to the previous page to change your selections for the CCD. Click Export to get started with the exchange process.

Clicking Export will display a pop up requesting a password. Create a password for this file and enter it into the Password field. Re-type the password in the Confirm field, and click OK.

You will be prompted to open or save the CCD file. Select the Save As option to save this file to your computer.

Your Save As window will come up. Choose a location to save the file to your computer, and name the file as desired. This will save the file in an encrypted Compressed (zipped) Folder. Click Save.

Your file will now be saved to your computer. Click Open to view the files.

The compressed folder will open. Each file will be password protected, indicated in the figure below. If you try to open any of these files at this point, you will get an error message. These files need to be extracted first.

You can now send the encrypted compressed folder with the CCD file in a secure way. You will also need to supply the recipient with the password you created for this file in order for the provider to open the encrypted folder.

UPLOADING CLINICAL EXCHANGE INFO This sub-link will meet the Meaningful Use criteria of being able exchange clinical information. The Upload Clinical Exchange Info link will allow you to receive CCD and CCR files. When you receive an encrypted/compressed folder you want to upload, save the file on your computer. You will need to have a file extraction program in order to complete the upload.

You will need to install the following program on your computer to extract the zipped files. To install 7-Zip on your computer, go to: http://www.7-zip.org/download.html. Click Download for the 7-Zip appropriate for your computer specifications. Click Run, and follow the install instructions that display on your screen.

Locate the folder you saved, and right click on the folder. Hover over 7-Zip and select Extract files.

The Extract window will require the password that was created by the sender. You will need to contact them if they did not already supply you with that password. Enter the password, and click OK.

You will now have the compressed file with the encrypted information, and the extracted files, which you will be able to open. You will now be able to upload the CCD or CCR file to the patient’s chart.

To begin the upload, go to the Patient Charts Tab, and select a patient.

Hover over the Others link, and select Upload Clinical Exchange Info.

Click Browse…, and select the folder with the extracted files.

When you open the extracted file, you will see multiple choices. Select the CCDXml file.

The file address will populate into the required field. Click Upload.

You will be prompted for a Sha-1 Hash Value

If you have been provided with the Hash Value:

Open the Extracted files folder from your computer, and select the Hash Values file.

The first line will list the CCDXml.xml Sha-1 code. Copy and paste the code into the Sha-1 Hash Value field.

Copy the Hash Value into the field, and click Upload.

You will get a confirmation that the file has not been modified. Click OK to open the file. If you were not provided with the Hash Value: You will need to install the following program on your computer to generate a hash value to upload the document. Go to: http://www.febooti.com/products/filetweak/members/hash-and-crc/. Download the EXE- Setup/Installer file.

After you have installed the Hash program on your computer, locate the file you want to upload. Right click on the file and select Properties, then click on the Hash/CRC tab.

Copy the SHA-1 code, and click Cancel to close the window. Go to the upload page for the patient, and paste the SHA-1 code into the Sha-1 hash value window. Click Upload.

You will get a confirmation that the file has not been modified. Click OK to open the file.

To access the files you’ve uploaded, go to the patient’s chart:

Hover over the Documents link, View All Documents, and select Past Medical Records.

Click the View icon, , in the right hand column for the document you want to see.

Select Open when prompted.

Select the HTML Document type, and the document will open in a new web browser tab.
