MIQUEST is a software tool that is used to extract comparable data from different GP clinical information systems in a standardised format

CHART is a data analysis tool from PRIMIS that allows practices to analyse and view the data held on their clinical computer systems in a variety of graphs and tables. It is excel based and is therefore easy for everyone to use and widely accessible

Registering with PRIMIS

In order to download the CHART software you need to register for basic membership of PRIMIS Hub.

Please note if you are already registered with PRIMIS as a Basic or Full Hub member you do not need to register again, simply log in.

If you have forgotten your password please visit the following page to reset it https://www.primis.nottingham.ac.uk/registration/password_generation/default.asp.

If your email address has altered, please email us at [email protected] and we will make the necessary changes to your account. We ask that you do not reregister, as this will create a second account that will not be set up with the products and services that you've previously been granted permission to access.

How to register with the PRIMIS Hub for free basic membership

Visit https://www.primis.nottingham.ac.uk/registration/registration/default.asp and complete the form to create your user name and password.

Your email address will become your username therefore please enter it carefully as it will be used to send you confirmation messages.

Downloading and installing CHART

This guide follows the process for downloading and installing CHART from the PRIMIS Hub. Please note if you already have CHART installed, you can jump to page 36 to check for any updates to the software or libraries.

Log in to the PRIMIS Hub at http://www.primis.nottingham.ac.uk/hub/login/index.php using your username and password:

The following screen will be displayed:

Click on the blue CHART box in the top left corner

Then click on the „CHART installation files‟ download box

The following screen will appear:

Select the version of CHART that is needed. This is based on the version of MS Excel you are using (either Excel 2000/2003 or Excel 2007/2010) and the coding system used by the practice clinical system (either generic Read codes

version 2 (most systems) or CTV3 (TPP SystmOne)). For example if you are an EMIS Web practice using MS Excel 2007, select CHART 2007 generic V2.

Once you are confident about the version you need, click to install. The following message will appear:

Click „Run‟ if you want to install CHART immediately on the computer you are using (alternatively click „Save‟ to store the installer file somewhere that you can access later)

A pop up box may appear asking if you are you sure you want to run this software

Click „Run‟ and follow the install wizard instructions on the screen:

CHART will automatically install at the following location (unless you choose to alter this) C:\CHART\

When completed, a CHART shortcut will appear on your desktop:

Once you have downloaded the CHART software you will then need to open CHART and configure it ready for first use.

o For CHART 2003 users see pages 29-30 and 32-34

o For CHART 2007/10 users see pages 31-34.

Opening CHART for the first time

CHART 2000/2003

Open CHART using one of the following two methods:

double-click on the CHART shortcut that was placed on your computer desktop during the installation process, or

using Windows Explorer, browse to and open the folder C:/CHART on your PC. Inside you will see a file called CHART.xls. Double click on this to open CHART

Excel will open and you will be presented with a security warning box as below:

Click on „Enable Macros‟

Next you will receive a confidentiality warning message. Read this message and click „I AGREE‟ to accept the terms of use

You now need to configure CHART ready for first use. See pages 32-34.

Troubleshooting

If CHART will not open, or if at any time the error „Run Time 1004 – access to visual basic denied‟ appears, then open the Excel menus as follows: Tools > Macros > Security

On the security setting select „medium‟. Then from the trusted publishers tab ensure both options are checked. Close and re-open CHART.

CHART 2007/2010

There are a few additional configuration settings required for users of Excel 2007 and Excel 2010 for CHART to work correctly. Macro Security Settings/Access to Visual Basic

You should take the following steps if

you are using CHART for the first time

CHART will not open correctly,

you see the error message „Run Time 1004 – access to visual basic denied‟.

Firstly, ensure that CHART is closed, but that Microsoft Excel is open

In Excel 2007 In Excel 2010

Click on the Microsoft Office Button in the top left of the screen

Click the File menu

Click on Excel Options then select Trust Center > Macro Settings

Click on Options > Trust Center > Trust Center Settings > Macro Settings

Select “Disable all macros with notification”

Check that the “Trust Access to the VBA project object model” tick box is ticked (as below)

1. Re-open CHART

The rest of the features are the same for both CHART 2003 and 2007.

Configuring the PRIMIS CHART software

Select the „PRIMIS CHART‟ option from the Excel menu bar and then select the „Configure‟ option. From here you can set up all your user preferences.

You will see six tabs: 1. Set Up 2. Display 3. File Paths 4. Set Doctors 5. Practice Details 6. Licence In order to download query libraries and future CHART software updates, you must ensure that both the Practice ID Code (set up) and the GP system (practice details) fields are completed accurately. 1. Set Up

Practice ID Code This should be the practice National ID Code usually five digits prefixed by a

letter, e.g. H12345. If this is not correct the queries will not run on the clinical system

Read Code Set This should reflect the Read code version used within the practice system

─ 9999R2 for 5 Byte (Read Version 2) systems

─ 9999R3 for CTV3 systems

Reference Date This will always default to the current date, and all timescales will go back

from this date. For instance a query looking at all drugs issued in the last 12 months, based on a reference date of 31/03/2014 will look for all issues between 01/04/2013 and 31/03/2014. You can set this date to past and future references if you wish. You can also quickly amend the reference date by double clicking on the reference date on the CHART home page

Query Agreement This should always state ‘LOCAL’

Enquiry Identity This should always state ‘LOCAL’

Delete Redundant Columns Tick this to hide working columns that are used to calculate other fields in the

data sheet. This makes it easier to read the data

Data Sheet Filter Tick this to filter the data in the various fields on the data sheet. A filter button

will appear at the top of each column in the data sheet view, even after having drilled down from the graph

2. Display By clicking on a colour you will get a colour menu where you can select your preferred choice of colour scheme. You can also use the drop down box to choose which item will display when viewing responses. Remember to click on the 'Apply Selection' to save your preferences. 3. File Paths This allows you to choose where your queries will be copied to, and responses loaded in from:

Query Delivery Directory This defaults to the C:/QQQ :, but you can browse and select any network

drive that has been mapped to your PC. You must select a specific folder for the queries to be copied to. The home page of CHART will display the file path you have chosen

Response File Directory This defaults to the C:/RRR but you can browse and select any network drive

that has been mapped to your PC. You must select a specific folder for the responses to be picked up from. The home page of CHART will display the file path you have chosen

Archive Directory This defaults to C:/CHART/Archive but you can change this by browsing and

selecting any network directory. The CHART software will automatically create an Archive folder for the responses to be placed into. Archived responses will be prefixed with the date they were run, and placed into a sub folder for that query library, for easy reference for example: C:\CHART\Archive\IMTDES\Allergies Report (Pseudonymised)\20140331CCALL

4. Set Doctors (Shared Server) This allows you to run queries on specific GPs either within a single server practice or a shared server practices.

Click on ‘New’ and then enter the name, initials and national ID number for the GP

Then click on ‘Accept’ to save these details

You should repeat this for each GP

Then tick the ‘Shared Server’ box. This will now only create responses for the GP‟s you have set up. Any other GP‟s will be ignored by the queries

You can tick the „Swap initials for National ID No.‟ to change the GP‟s ID number in the data sheet to the initials you set up so that it is easier to identify the GP

5. Practice Details This allows you to enter all the details for the practice and these will then appear automatically on any of the summary sheets. This saves the practice re-entering them each time if they are sending off the summary sheet as a report. It is not mandatory to fill in all the fields, however setting the system type in the drop down box is essential in order to download the correct query libraries for your clinical information system. Please do not use the two fields „'PRIMIS Unique ID' or 'Scheme ID'. The 'WebDav' box should be ticked, as this will allow you to upload aggregated results from certain query libraries to PRIMIS should you choose to. 6. Licence This is for administrative purposes only. This can help us diagnose or troubleshoot any problems that you are experiencing with CHART. After making any changes, click on OK to return to the CHART homepage. You are now ready to start using CHART

Requesting access to NHS Improving Quality GRASP tools

In order to download and use any of the GRASP tools (AF, COPD or Heart Failure) all PRIMIS Hub members must request access from NHS Improving Quality. 1. Ensure you are registered for basic membership of PRIMIS Hub and log in 2. Request access

Once logged in, visit the following web page to complete a short form requesting access http://www.primis.nottingham.ac.uk/hub/grasp/

If you are employed by a GP practice, please click on the link at the top and complete a different form providing your practice national ID code.

Once you have received a message confirming acceptance of your request you can go on to step 3.

3. Accept the GRASP terms and conditions of use

Scroll down the screen to view and accept NHS Improving Quality‟s GRASP terms and conditions of use.

Once all three steps have been completed successfully, the GRASP tools will be available for download from within the PRIMIS CHART software (see next page).

PRIMIS can help! By ordering the CHART Remote Installation and Support Service, a PRIMIS expert can set up CHART, run the GRASP audits on your system and upload aggregate data to CHART Online on your behalf (with permission).

Downloading and Installing CHART query libraries

Open CHART

Click the „Check for updates before running audit „ link

The following box will appear. Enter your PRIMIS Hub login and password

Click OK

A new internet browser window will now open on your computer.

If you encounter any problems accessing the „check for updates‟ facility:

Ensure that both your practice ID code and clinical system have been entered correctly in the configuration settings within CHART

This screen confirms which version of the CHART software you have installed, whether this is the latest version and which query libraries are available to download.

green ticks indicate that the most up to date version is present

red crosses indicate that the version needs to be updated

orange exclamations show that you have not yet installed the library

The list of libraries that you see is determined by your level of PRIMIS Hub membership (full or basic) and whether you have been granted access to any GRASP tools – see page 35 for information on requesting access to GRASP.

Click the „Download‟ button in the „Action‟ column for the query library you want to install. The download should start automatically.

If you have a pop-up blocker installed, you may need to right click on the yellow banner that appears and select „Download File‟:

You will be asked if you want to run or save the installer file, click „Run‟:

A pop up box may appear asking if you are you sure you want to run this software. Click „Run‟ and follow the install wizard instructions on the screen:

The following Microsoft Windows setup wizard will appear:

Click on „Next‟ to continue. The following screen will be shown:

The query library will automatically be installed on the C:\ drive in a folder named CHART

Click on „Next‟ and then „Install‟

When prompted click on „Finish‟

After a new installation or update, CHART must be closed and re-opened. Additionally the update screen will only refresh after CHART has been closed.

Installing more libraries

You can repeat the above steps to install more CHART libraries. Once you have finished installing, close your internet browser window and navigate back to CHART.

Upon navigating back to CHART, the following message will be displayed:

Click on OK. Close CHART by clicking on ‘Close Applications and Databooks‟. Re-open CHART to start using the libraries you have downloaded.

Preparing queries in CHART

1. On the CHART home page select a query library from '1.Select Query library' drop down box on the left hand side of the screen

2. Some query libraries only have one topic. Some have multiple topics. Click on the second drop down box to see how many query set topics there are for your chosen query library. For example, GRASP-AF contains both an AF case finder set and an AF care set of queries.

If you wish to run all sets within a library you must tick the box ‘2.Tick to copy out full library of queries’.

3. Click on ‘3.Copy queries to selected drive’. If you chose to copy out the full library of queries, CHART will confirm that is what you want. Click „Yes‟.

4. You will now be prompted to choose between running patient identifiable queries or pseudonymised queries. Both queries bring out the same medical information for patients. However, in the datasheet:

o Patient identifiable queries show you the patient‟s surname, forename, date of birth, patient number at the practice and GP number. This is most useful for audit purposes and for close interrogation by practice staff.

o Pseudonymised queries only show a pseudonymised patient number and their age. It is much harder to identify patients. Pseudonymised queries are best when it is not necessary to identify patients (eg only summary figures are required as is the case when uploading data to CHART Online for comparative analysis).

5. Once you have decided whether to run identifiable or pseudonymised queries, CHART will create them at the location you specified as your ‘Query Delivery Directory’ (see CHART Configuration settings).

6. If you want to run both named AND pseudonymised queries to run at the same time, simply repeat the process from point 1, selecting the other type of query at point 5. Remember, if you run the patient identifiable queries, the responses should be kept secure and not distributed outside of the practice. You cannot use the identifiable query set to upload data to CHART Online due to an inbuilt security feature.

7. The queries then need to be run on the MIQUEST Interpreter on your clinical system. Refer to the specific GP clinical information system MIQUEST instructions for your system.

Loading and displaying responses

The clinical system will have exported a set of csv (comma separated value) response files. There will be one csv file for each hql file that you originally loaded into the clinical system. The csv response files will have the same names as the hql files. To load responses into CHART:

Make sure you are on the home page of CHART

On the right hand side of the screen, click on hyperlink three ‘Click here to view new results’

Click on the 'Browse' button and browse to the location of your csv files. You will then see the reports that you can load up into CHART. You can load in as many files as you wish at this point by holding down control and clicking on the responses to import. Click „select’ to open the files

CHART audit tool instructional guides

On the CHART home page you will find a link entitled ‘Click for description of query’. Clicking this will give you access to any instructional guides written for the query library currently highlighted in „1. Select Query Library‟. Many PRIMIS audits are now accompanied by a detailed step by step guide to help practices act upon their audit results. They include recommended tasks for practices and useful information on the rationale behind many of the audit features.

Viewing results in CHART

There are three main methods of viewing results within CHART. These are the summary sheet (classic view and sometimes also a dashboard view), pre-set graphs and the full patient datasheet. Navigation The query responses will always load with the summary sheet showing. It is important to understand how to navigate between the different sections. The following menu appears when viewing responses: 1 2 3 4 5 6 7 8

Running from left to right the functions are:

1. Home icon – takes you to the CHART home page

2. Disk icon - archives the current set of responses

3. Up and down arrows– allows you to move through the graphs

4. Drop down box - allows you to select of any of the graphs by name

5. Icon one – takes you to the datasheet or graphs (depending upon which view you are currently in)

6. Icon two – takes you to the graphs or the summary sheet (depending upon which view you are currently in)

7. Printer icon – allows you to print the current view

8. Drop down box - allows you to navigate between the different responses you have imported into CHART

The summary sheet

Provides an overview of the information contained in the responses. Some libraries have a dashboard as well as a classic summary sheet. The dashboard shows multiple graphs of different areas of interest (see next page for examples of both views of the GRASP-AF summary sheet).

Pre-set graphs

These provide set displays of the data in graphical format. You can drill down on a column in the chart to view the patients who directly relate to that area. Simply click so the bar is selected, then double click. A new worksheet will open up. This worksheet will show you the patients who made up the selected part of the bar chart.

The datasheet

The CHART datasheets lists all patients that are included in the audit. Hovering your cursor over a column header gives a brief description of that field. If the filter box has been checked (in the configuration menu) columns can be filtered by clicking on the grey arrow box below the column header. Note: the filter option that can be used in Excel is discussed later and is a useful tool for filtering results.

Some datasheet rows are highlighted with colours placing patients into distinct categories. For GRASP-AF these colours can be decoded as follows: Red: Those classified as high risk of stroke using CHADS2 who are not on

anticoagulant and are not contraindicated (CHADS2 score >1)

Orange: Those classified as medium risk of stroke using CHADS2 who are not on anticoagulant and are not contraindicated (CHADS2 score = 1)

Black : Those classified as low risk of stroke with a CHADS2 score of 0

Further assess patients’ risk using the neighbouring CHA2DS2-VASc column

Magenta: Patients with a H/O haemorrhagic stroke who are on anticoagulant and / or antiplatelet

Note: if a patient falls into more than one of these categories they are only included in the most significant category.

How to filter in CHART

You can use filtering functionality to search the datasheet using your own criteria. For example if you select the grey box with the arrow on the Patient Age column, you will then be able to filter on patients who meet a specific age criteria, e.g. over 65 years old. You can continue to filter further on any of the columns using the built in Excel filters. Applying a filter to a column will change the colour of the arrow to blue. Please remember to remove all filters when you have finished. The quickest way to remove multiple filters is to Select Filter on 'All' to remove the filter or select 'show all' from the Excel Data menu.

Loading and saving filters In CHART you can save your own bespoke filters to load them at a later date. This is useful if you use them repeatedly.

To store a filter:

Set the filter you want to save in the datasheet view

Click on the PRIMIS CHART menu and scroll down and select 'Store Filter'

Fill in a description of the filter

Click ‘Save’

Some query libraries will have pre-loaded filters. You can either use these, or clear them and create your own. To load a pre-set or saved filter:

Click on the PRIMIS CHART menu and scroll down and select „Load Filter‟ and it will apply the chosen filter.

You can clear a filter at any time by highlighting and selecting the „Clear Filter‟ option in the box above or remove multiple filters as explained earlier.

Loading and saving archived responses

To archive a response you must be viewing the results. Then from the navigation menu select the icon as shown previously, and it will archive the results to the default folder or the folder you created. They will be saved in the following format: Archive directory/Query library/Query/Date query ran e.g. C:\CHART\Archive\GRASP_AF\Report on patients with AF (Pseudonymised)\ 20131107AFREP2A.CSV Once these are saved in the archive directory they can be retrieved at any time. This is done from the CHART home page using „click to view archived results‟. This will take you to your Archive folder.

Printing the datasheet

Select the columns to be printed by holding down shift and selecting a range of adjacent columns or by holding down Control (Ctrl) and clicking on non-adjacent columns. The columns will appear a darker blue to show they have been selected (see below):

Click on the print icon:

The following dialogue box appears:

Choose the paper orientation required and click on the OK button.

Emailing a summary sheet

From the summary sheet view, click on 'email summary sheet' on the toolbar

A dialogue box will appear where you can enter the recipient‟s email address

You have the option to define a password either to protect the file from being edited or to prevent anyone without the password from opening it. Enter a password if you wish to use this functionality (don‟t forget to share the password with the recipient in the latter option)

Click 'email summary sheet' (ensure Microsoft outlook is open and you have logged in)

A warning box will then appear stating that an email is being sent, click on ‘allow’ to proceed or ‘deny’ to stop the transmission

If you click ‘allow’, your email will be sent with an Excel attachment to your recipient

Uploading data to CHART Online

Variations in data management and activity are more visible when compared across groups of GP practices. Comparative data analysis provides a powerful tool for standardising care across localities. For example, many users of the GRASP-AF tool frequently upload data to CHART Online resulting in a comprehensive, well-populated database that contains valuable information regarding AF care within England. As at October 2014, over 2,650 practices from 156 Clinical Commissioning Group (CCG) areas had uploaded GRASP-AF data to CHART Online. These data show 335,203 patients with a diagnosis of AF equating to an average recorded prevalence of 1.78% of the practice population.

Once the data has been viewed in CHART within the practice it is a simple process to upload it to CHART Online. Note: only aggregated data from pseudonymised queries (i.e. not patient level data) can be uploaded to CHART Online.

It is recommended that you always upload the initial baseline set of data (first run of the query library). Subsequent uploads can be made to CHART Online when ready. This will enable you to make comparisons between your latest results and the original set in the future.

For ongoing monitoring of patients, you are advised to run the GRASP AF library quarterly or at least annually.

Ensure you are in summary sheet view (either classic or dashboard) and then click the „upload data to web‟ icon on the CHART toolbar (it looks like a folder with a globe – this will only be present when viewing pseudonymised results):

A dialogue box appears showing you the data string that will be sent to PRIMIS. Click 'Close the box to continue'

The following message will appear:

Click Yes

Enter your PRIMIS login details when prompted. If on log in, the CHART Online Data Collection Agreement (DCA) for that audit has not been signed, the following message will appear.

Please read this carefully and follow the instructions to confirm that you accept the agreement.

Repeat the steps to upload your data to CHART Online.

Click 'OK' when you receive a message confirming that your data has been sent
