Transcript of Chapter 5-1: VeinGogh - Ohmic Thermolysis
Newly Designed Needle – ‘Bristle’
• Using heat energy through a specialty designed needle to coagulate spider telangiectasia
• 27-gauge • Needle is insulated except for distal 2mm
Needle
• Heat is conducted efficiently without loss to surrounding tissues due to the insulation
• Very low power settings needed due to insulation • Minimal to no collateral skin damage • See fast resolution of treated vein within a month
Technique
• Needle may or may not need to be inserted completely through the skin, depending on depth of spider telangiectasia
• Mechanism of action is destruction of spider vein by loss of endothelium and fusion of vein wall by thermal energy
VeinGogh Device
• May be used on all skin types • All patients need a preliminary test of a small area before beginning
treatment • As with any other device, test spot should be done to rule out any adverse
reactions
Clinical Experience With VeinGogh - Ron Bush, MD, FACS
• Used VeinGogh in over 500 patients with all skin types • No complications • Skin tracking is almost non-existent compared to the original ballet needle
on the leg • Leg – Prefer for spider veins < 0.6 mm • Red Vein – Vessels most amendable to this device
• Larger veins – 1 mm, can be treated after experience is gained with this
device • Training is needed by a professional • Currently doing studies with VeinGogh and sclerotherapy at the same
setting
• More than 40 biopsies with histological evaluations have been completed using VeinGogh device
• THERE ARE NO CONTRAINDICATIONS & NO PERMANENT SKIN
DAMAGE! 89/27/12 For patient comfort during a VeinGogh session, we have been using "BLT" cream. This is 20% Benzocaine, 10% Lidocaine and 10% Tetracaine. At the consult, we give the patient a small amount of the cream and have them apply it 1 hour before their procedure. We ask them to rub it on the treatment areas for about 2 minutes. We remind them to wash their hands to avoid numb fingers! The numbness starts about 30-45 minutes after application and remains for approximately 3 hours. This is really helpful in the area around the nose. It does not completely numb the discomfort, but lessens it dramatically. It also seems to help with the burning discomfort/sensation post procedure too. We have purchased this cream from MD Custom RX in Wisconsin. Their number for ordering is 262-373-1050.
