Design Paradigms
Observation by different person
Wrong evaluation method
Design Reliability
Reliability comes from six major sources:
1. Proper functional design.
2. Proper safe life design.
3. Proper fabrication.
4. Proper QC.
5. Proper maintenance.
6. Proper usage.
Failure in Design
Example 1-column transportation to Apollo at Selinous (Function design Error)
Transportation of a round column
Failure in Design
• Transportation of a rectangular column
Failure in Design
• Transportation of a rectangular column- a new design
Failure in Design • Example 2 – Kansas city Hyatt Regency
(improper fabrication)
Failure in Design • Example 3 – improper safe life design
Failure in Design • Example 4 – improper safe life design
Failure in Design • Example 5 – Wrong Analysis
Failure in Design • Example 6 – Fail of Dee Bridge (not
enough safety factor)

Failure in Design • Example 7 – Columbia disaster (human
error)
Failure in Design • Example 7 – Columbia disaster
Failure in Design Lesson should be learned1. Failure mode should be recognized.2. Engineering Judgment is a important issue to designer.3. Human error is incontrovertibly a major cause of
design.4. Experience is important to mechanical engineer.5. success can be replicated virtually without risk, but it
can be extrapolated only by a proper application of the engineering method embodied in a proper perspective on failure.
6. Respect of law of nature. 7. Engineering history is important.
