All registration methods will receive a confirmation letter by mail within two weeks of processing registra-tions. the location, start date and time of the course will be included. Please check all details carefully, and call 905-338-4250 if there are any discrepancies, or if you do not receive this letter prior to the start date of the program.

Excellent programs can get cancelled because people wait until the last moment to register. All programs need to be cost effective and cannot run if there are not enough participants. Please register early to avoid the disappointing call that the program has been cancelled.

All participants must meet the course age requirements. late reg-istrations will be accepted by fax, mail or drop off on a case-by-case basis and subject to approval from the program supervisor.

in the event that the course you selected is full, you may be placed on a waitlist and a waitlist confir-mation will be mailed to you.

Payment:Full payment of program fees must be submitted with your registration form. if paying by cheque,

please make payable to ’town of Oakville’. iris registrations must be paid by credit cards – ViSA, MasterCard, Amex or by using credit on your iris account.

We will try to ensure that the regis trations are processed together for friends or relatives. registra tion form, individ-ual cheques and/ or credit card numbers are required for each family in order to receive separate receipts.

duplicate receipts will only be issued upon request and are subject to a $10.50 administration fee per request. An administration charge of $25 will be levied for all NSF cheques and returned payments.

Refunds/Withdrawals:Pro-rated credits or refunds/ withdrawals can be requested no later than one business day prior to the third class. You are also able to withdraw online from a program using iris, up to seven days prior to the program start date. A credit will be placed on your account. Withdrawal and/or refund requests on or after the third class of the pro-gram will be pro cessed for medical reasons only for the remaining classes of the program and must be accompanied by a doctor’s note. these medical requests will be pro-rated, but will not be subject to an administration fee.

between 8:30 am and 4:30 pm Monday – Friday. All requests for refunds are subject to a $10.50 charge per registration per transaction.

refunds will be issued for all programs cancelled by the library.

For Safety’s Sake:to ensure participant health and safety, please

make program staff aware of any medical conditions or spe-cial needs. A participant profile – Special Needs/Medical form should be completed and sent to the library or the registration section of the recreation and Culture department. do not leave children unattended in or about the library premises. the library is a public place and children may be at risk in a public place. library staff cannot be respon-sible for the safety or supervision of unaccompanied children. We are concerned about your child’s safety. Please ensure your child’s visit to the library is a safe and happy one.

Cancellation Information:the Oakville Public library reserves the right to cancel or alter any classes, time, costs or locations outlined in this brochure without notice, as required, due to a lack of registration, change of policy, or availability of facili-ties or instructors. Full refunds will be issued for Course Cancellations only.
