What is CDSMPEvidence based, peer-led
programming designed to help people gain self-confidence in their ability to control their symptoms and health conditions and how their health problems affect their lives.
Small group workshops include six 2½-hour sessions

Statewide Goals for CDSMP Increase availability of CDSMP to
older adults in ColoradoEstablish a statewide CDSMP
