The 25th edition of

Standards for Blood Banks

and Transfusion Services

details the latest standards

of practice in blood banking

and transfusion medicine.

As in the past, a quality

management system is the

framework of the standards,

with each of the 10 chapter headings representing one of the 10 Quality

System Essentials.

Signifi cant changes:Requirement for implementation of ISBT 128.• Requirement for use of diversion pouches for collection of platelets.• Requirement for West Nile virus RNA testing, per seasonal criteria.• Revised record retention periods.• Revised requirements for allogeneic donor qualifi cation.•

Quality Manual Preparation Workbook for Cellular Therapy Product ServicesBy Lucia Berte, MA, MT(ASCP)SBB, DLM, CQA(ASQ)CMQ/OE

Cellular therapy product services have long

been required to write and maintain “procedure

manuals” containing documents that describe

the service’s technical work. However, how to

conduct essential nontechnical work has been

left out. The CT Quality Manual Workbook

remedies this oversight. This workbook provides

guidance for writing policies for the QSEs and

identifying the processes and procedures needed

to develop a quality manual. It includes templates

for QSE policies that address requirements of the

AABB Standards for Cellular Therapy Product

Services.

Highlights:A• nswers to the most commonly asked questions

about a quality manual.

Templates for policies for each of the cellular • therapy product service QSEs, both in print and

electronically captured on a CD-ROM that can

be used to develop a cellular therapy product

service quality manual.

Serologic Problem-Solving: A Systematic Approach for Improved PracticeEdited by Sally V. Rudmann, PhD

Solving serologic problems and teaching others

how to solve these problems can present

a challenge to even the most experienced

technologists. Those so-called “expert”

immunohematologic problem-solvers may

never have had the opportunity to step back and

evaluate how they approach a problem and the

steps they take to resolve it. The fi rst chapter

reviews the problem-solving process, including

in what manner an individual approaches

any problem. In the remaining chapters, the

problem-solving model is applied to many types

of serologic problems and is emphasized through

case studies.

Highlights:Resolution of ABO and Rh typing • discrepancies.

Evaluation of positive direct • antiglobulin tests.

Antibody identifi cation.• Monoclonal reagents.• Application of patient clinical • information to solve a problem.

Guidelines for the Management of Transfusion-RelatedAcute Lung InjuryDeveloped for the Scientifi c Section Coordinating Committee by Mark A. Popovsky, MD

This Guideline addresses the clinical

and laboratory features of transfusion-

related acute lung injury (TRALI), as

well as its management. TRALI is a life-

threatening complication of transfusion

that is clinically indistinguishable from the

acute respiratory distress syndrome (ARDS), but which has a more favorable

prognosis. Nevertheless, in the developed nations, it is the second or third

most common cause of death from transfusion.

Guidelines for the Laboratory Evaluation of Transfusion ReactionsDeveloped for the Scientific Section Coordinating Committee by Robertson Davenport, MD

While infectious transfusion complications

are often the focus of patients, physicians,

blood transfusion professionals and regulatory

agencies, nonviral and immunologically based

complications are the most prevalent risks

for a transfusion recipient. The diagnosis and

evaluation of any transfusion reaction is a matter of clinical judgment by

a physician trained in the adverse eff ects of transfusion. Negative testing

results should not be interpreted as defi nitely excluding a transfusion reaction

if there is a strong clinical suspicion that the patient’s symptoms are related

to blood transfusion. The laboratory evaluation of suspected transfusion

reactions is critical not only to the health and well being of the patient, but

also to the process of improvement through corrective and preventive actions.

Strategies for Centralized Blood ServicesEdited by Marcus B. Simpson, MD

The value of centralized transfusion

services has been demonstrated by their

increased effi ciency, improved resource

management, cost reduction and

enhanced quality. This volume examines

how such effi ciency is achieved with up-

to-date and in-depth discussions of both

US and non-US networks: Puget Sound

Blood Center in Seattle, the Institute for

Transfusion Medicine in Pittsburgh, Florida Blood Services at Tampa Bay, and

the Hemominas Foundation in Brazil. Because areas with diff erent needs

require diff erent designs, the variation among these systems is instructive.

The critical issues for success in centralized networks are discussed in the

context of these systems — for example, geography, logistics, competitive

viability and fi nancial concerns. Separate chapters focus on informatics,

legal, regulatory and quality considerations, as well as the changing value

propositions for centralized networks today. The book serves as the fi eld’s

primary resource on this distinctive delivery model.

Screening Blood Donors: Science, Reason, and the Donor History QuestionnaireEdited by Anne Eder, MD, PhD, and Celso Bianco, MD

Every day, blood center staff evaluate

donor answers to screening questions

and make decisions about deferrals

— decisions for which guidance

is sometimes lacking. Regulatory

recommendations and the donor history

questionnaire cover the broad issues, but what about medical conditions

that are not addressed? For those that are addressed, what about “special

cases”? What scientifi c evidence can be referenced in these situations? What

background is necessary to understand the rationale behind the screening

questions as blood centers manage new issues that arise? This book is an

indispensable guide to coping with such questions, off ering solid support for

medical decisions that protect the safety of both blood donors and recipients.

Topics:

Blood donors with cardiovascular disease.• Blood donors with impaired hemostasis.• Blood donors with a history of cancer.• Medications and vaccinations.• Infectious disease risks.• Postdonation information.• Donor reentry.•
