1. Print clearly your full contact information in first box on order form.

If you have any questions put them under “Comments”

2. If you want the items shipped to a different address than your own give that in

second box below.

3. List items desired but list Bibles in top section (where mentioned) on form. Give the

item number, name of item in either Mien or English, quantity wanted, price each, and

total for that line. Then write subtotal for any Bibles at far right.

4. Then list other items you desire. Sub-total those then total of all items.

5. Add shipping & handling charge as shown in this chart:
