PaymentFull payment for most programs is due at the time of registration. Payment may be made by cash, check or credit card (Visa/MC/AMEX/DISC). Make checks payable to the Carol Stream Park District. There is a $20 fee for returned checks.

Program CapacityAll programs have minimum and maximum enrollments. If the minimum enrollment is not met one week prior to the program date, the Park District may have to cancel, postpone or consolidate the program with another program. If the program is cancelled, full credits will be issued.

Waiting ListsOnce the maximum enrollment is met for a class, a waiting list will be formed. We will create additional classes when feasible and you will be contacted. Refunds will be processed after the first class.

InsuranceThe Park District does not carry medical or accident insurance for program participants or park patrons. Please review your personal health insurance plan to be certain that you and your family have proper coverage.

Financial AssistanceWe offer financial assistance/scholarships for qualified Carol Stream Park District residents. Confidential applications are available at the Simkus Recreation Center.

Satisfaction Guaranteed!Our goal is your satisfaction! If you are not completely satisfied with any Carol Stream Park District program, class or event, please tell us! We will work to earn your satisfaction. If you are still not completely satisfied, we will transfer your registration to another program or session, or issue you a refund - that’s our guarantee!

Green Up Your WorkoutRunning shoes are designed to last for 300-400 miles of use. So what do you do with worn out running shoes? Nike’s Reuse-A-Shoe program was created to solve the problem. Old shoes of any brand are collected and recycled to be used to build new tracks, basketball and tennis courts. Shoes can be dropped off at Nike stores or mailed to the recycling center. The Carol Stream Park District Board of Commissioners consists of seven individuals who are elected by residents to represent the community’s best interests. The charge of the Board is to set policies that will guide the Park District. Board members serve a four-year term and receive no compensation for the time they give. They come from a variety of backgrounds and share one important trait - a commitment to serving the park and recreational needs of our community.

Board MeetingsRegularly scheduled meetings of the Board are held on the second and/or the fourth Monday of each month according to a schedule adopted by the Board yearly. Please check our website for our current meeting schedule. Regular Board Meetings open at 7:00 pm.

Zombie Survival TrainingMonday, Oct 8, 6:30pm As dusk falls on Hampe Park, join us to learn some essential skills to prepare you for the zombie apocalypse! Learn how to zombie proof your house and make a zombie survival kit. Be trained on how to fight and/or outrun a zombie. After the tutorial, test your knowledge in our zombie trivia challenge. Wear your best zombie costume and we’ll provide the make-up to transform you into a zombie. Prizes awarded for best zombie costume, walk and moan.

Letters to SantaExperience the excitement of opening a personalized letter from Santa Claus! Your child can drop off a letter to Santa that we will deliver to the North Pole. Look for the special mailbox inside Simkus Recreation Center from November 26 - December 9. Please make sure that your child’s full name and address are included in the letter so that Santa knows where to send his correspondence. You can also email him at [email protected]. Don’t miss out on this special holiday tradition that is sure to put a smile on your child’s face!

SPECIAL EVENTSPolar ExpressSunday, Dec 9, 11am & 3pm All aboard! Come along on our annual train ride for a reading of the children’s classic story ‘Polar Express’. Bring a copy of the book and read along with us if you wish. We will ride from the Roselle train station to the ‘North Pole’ (Elgin/Big Timber). Santa will then join us on the train for the ride back to Roselle and sing holiday songs with us. Afterwards, Santa will have plenty of time to visit with each child and hear their holiday wishes. Refreshments and gift included in fee. A parent or guardian must accompany children on the train. Adults will also need to be pre-registered for this event. Please arrive 20 minutes prior to departure and parking is available. The deadline for registration is November 2.

Santa VisitsTuesday, Dec 18 & Wednesday, Dec 19 Why wait in line at the shopping mall to sit on Santa’s lap when Santa can come see you? Surprise a child with a visit from Santa right to your house. Santa will arrive in his ‘custom’ RV and listen to your child’s holiday wish list inside the RV! Due to his heavy schedule, Santa can only stay for five minutes in front of each house and will only be able to visit with immediate family members. You will need to be available 5:30 to 8:00PM so Santa finds you at home. Santa visits are open only to residents of the Carol Stream Park District. Santa Visits are sponsored by Hometown RV, located right here in Carol Stream. They’ve generously agreed to donate the use of Santa’s RV. Visit them on the web at www.hometownrv.com. Ages 6mo.-3yrs with ParentDevelop comfort in the water, water adjustment, entering and exiting the water, front and back kick, safety skills, swimming readiness, floating with assistance, putting face in water, and blowing bubbles. Swim instructors facilitate the lesson, parents instruct child.

Tiny Tots - Seahorses. Ages 3-5yrs. Learn water adjustment, water entry and exit, supported front and back floats, bubble blowing, putting the face in the water, alternation of arms, front and back kicks, and safety skills.

Advanced Tiny Tots - Guppies. Ages 3-5yrs. Learn to enter the pool with confidence, supported front and back floats, blowing bubbles for five seconds, fully submerging face and head in water, alternation of arms, front and back kicks, and safety skills.

Level 1 - Rays. Ages 5yrs+ Learn to enter the pool with confidence, unsupported back and front float for five seconds, full submersion of face in water and blowing bubbles. Introduction to arm reaches; front crawl arms, front and back kicks, front stroke, back stroke, and safety skills.

Level 2 - Minnows. Ages 5yrs+ Build confidence in the water, unsupported back and front float, underwater bobs, arm reaches, front and back kicks with kickboard, front and back glides, front and back stroke ten feet with assistance, retrieval of underwater objects, and safety skills.

Ages 5yrs+ Learn how to enter deep water with confidence, establish independence from instructor, front and back float, rotary breathing, front and back stroke fifteen feet without assistance, elementary backstroke fifteen feet without assistance, introduction to breaststroke, butterfly kicking, and safety skills.

Pathway to Preschool ProgramChildren must be 2½ years old when registering. Get ready for preschool with classes for 2½-3 year olds that will enhance their social, physical and creative development. Children will learn through play and emphasis will be placed on becoming comfortable in a classroom environment.

Dance

Recital dance classes run September-May and perform in the annual spring dance performance.

Registration is by age. Dancers are to be registered by age as of September 1, 2012.

A $47 recital costume fee is due at time of registration. Costume fee is per registered dancer. Recital costumes are ordered December 1; costume fees are non-refundable beginning December 2.

Recital Dance Creative Movement Children will be introduced to the world of dance through activities that encourage expression, music appreciation, and creative fun. • Registration is by grade level. Players are to be registered for the grade the player will be in for the 2012-13 school year.

• Preschoolers must be entering kindergarten for the 2013/2014 school year.

• All high school players register using the same code for high school softball.

Wait List • Players placed on a wait list are not guaranteed placement

on a team.

Before team formation:

• Player will be moved off of the wait list when there are enough to form an additional team.

After team formation:

• Placement will only be done when a team or teams are below the established maximum number of team participants.

• Players will not be added to any team one week prior to the first scheduled game.

Requests• One mutual friendship request per player will be honored

for players in Instructional, Grade 1-2, 8U and U8 sports leagues. THESE DO NOT APPLY FOR OLDER LEAGUE REQUESTS.

• One practice day conflict form per player will be accepted for players at all levels. Note: This does not apply to games.

• Due to the nature of the program, we cannot accept any other coach, player, or carpool requests

• A park district request forms must be used for these requests and the guidelines must be followed. These forms are available at Simkus Recreation Center and on our web site at www.csparks.org

The Carol Stream Park District continually strives to reduce such risks and insists that all participants follow safety rules and instructions that are designed to protect the participants’ safety. However, participants and parents/guardians of minors registering for the above listed programs/activities must recognize that there is an inherent risk of injury when choosing to participate in recreational activities/programs. PaymentFull payment for most programs is due at the time of registration. Payment may be made by cash, check or credit card (Visa/MC/AMEX/DISC). Make checks payable to the Carol Stream Park District. There is a $20 fee for returned checks.

Program CapacityAll programs have minimum and maximum enrollments. If the minimum enrollment is not met one week prior to the program date, the Park District may have to cancel, postpone or consolidate the program with another program. If the program is cancelled, full credits will be issued.

Waiting ListsOnce the maximum enrollment is met for a class, a waiting list will be formed. We will create additional classes when feasible and you will be contacted. Refunds will be processed after the first class.

InsuranceThe Park District does not carry medical or accident insurance for program participants or park patrons. Please review your personal health insurance plan to be certain that you and your family have proper coverage.

Financial AssistanceWe offer financial assistance/scholarships for qualified Carol Stream Park District residents. Confidential applications are available at the Simkus Recreation Center.
