INSTITUTE OF MEDICINE Cancer Care for the Whole Patient: Meeting Psychosocial Health Needs The Institute of Medicine (IOM) was asked by the National Institutes of Health to form a committee to assess the current delivery of the wide range of psychosocial services that cancer patients and their families need and to recommend ways for improving how these services are provided. Resources for Information on Cancer and Free Psychosocial Health Services One of the IOM committee's overarching conclusions centers on the wide array of resources that already exist for cancer patients. The committee compiled two lengthy lists of some of these: Table 3-2 (see p. 2) from the report provides a list of selected sources of information on cancer and cancer-related services; Table 3-3 (see p. 10) also lists additional free psychosocial health services that are available to cancer patients and their families. Both of these tables highlight the potential for better delivery of psychosocial information and services if doc- tors give patients this kind of information and the patients follow up. The com- mittee acknowledged that some psychosocial problems, such as poverty, cannot be solved, but every cancer patient can be helped in some way. Approaches to Delivering Psychosocial Services An additional conclusion of the IOM committee identified three different approaches for delivering these psychosocial services to cancer patients. Services can be: 1) integrated into the current biomedical delivery system; 2) obtained from local resources; or 3) obtained from remote providers via the telephone or Internet. There are a number of examples of cancer care providers already deliv- ering psychosocial health services in these ways. These illustrate how each approach is possible (see p. 29).

Addressing Patients’ Needs:Three Approaches

The IOM committee on Cancer Care for the Whole Patient identifies three approaches for providingpsychosocial health services to cancer patients. For more information, please see Chapter 5 in the report.

Collocated, Integrated Psychosocial and Biomedical Health CareThe Rebecca and John Moores Cancer Center, University of California, San Diego

The Moores Cancer Center's Science of Caring program provides comprehensive psychosocialhealth care integrated with biomedical treatment for all cancer patients seen in its outpatient clinic.Every patient uses a laptop computer to complete a touch-screen questionnaire, which provides an auto-matic link to psychosocial health services. For problems requiring a more complex intervention, theautomated screening tool generates an e-mail to the appropriate team member. Psychosocial care is coor-dinated with medical care through meetings and direct, face-to-face communication among providers,and follow-up on the services occurs regularly. The West Clinic, Memphis, Tennessee

The West Clinic consists of three free-standing ambulatory oncology practices in metropolitanMemphis, plus three satellite offices nearby. Screening of patients for psychosocial health problems andquality-of-life assessments take place at every visit by means of a computer-based screening instrument,which collects information from individuals about their cancer-related symptoms while they are in thewaiting room. For some psychosocial health problems, patients receive the needed services directly onsite; for others, they are referred to resources in the community.

Provision of Psychosocial Health Services Using Local ResourcesKansas City Cancer Center

The KCCC addresses patients' psychosocial health problems by linking patients with communityproviders. Nurse practitioners screen patients at their initial and subsequent visits using a one-pagescreening tool to detect psychosocial health problems. Based on the results of the screen and periodicpsychosocial assessments, nurses link patients with multiple psychosocial services available in theKansas City area. Psychosocial health issues are addressed during and after treatment. The KCCC alsohas a survivorship program that provides education about the adjustments required after treatment. Tahoe Forest Cancer Center

The TFCC uses the resources of its community hospital to address patients’ psychosocial healthneeds as part of regular oncology care. The solo oncologist practice uses hospital personnel to helpaddress psychosocial needs of patients including social services; nutritional counseling; rehabilitationtherapy; and support group meetings for cancer patients, family and friends at the hospital's local Centerfor Health and Sports Performance. Patients' psychosocial needs are identified during office visits orweekly meetings of the entire team. Coordination efforts with biomedical care and follow up are per-formed by TFCC nurses.

Use of Remote Providers of Psychosocial Health Services When a clinical practice has few staff, limited resources, or is located where there are few or no psy-

chosocial health care resources, such as in rural or remote areas, the only way to provide psychosocialhealth services on a frequent and timely basis may be to link patients with remote providers throughtelephone or Internet access.

This option may also be preferred by some individuals, even when psychosocial health services areavailable in their communities. Those with rare cancers may wish to connect with others who have thesame type of cancer. Others may simply desire the convenience or anonymity of using the telephone orthe Internet in their own homes. In particular, adolescents and young adults who use the Internet routine-ly also may prefer this mode of communicating.
