They sent it to Improving Health & Lives in January2015

What’s it all about?
The Joint Self- Assessment is a way of checking how good
services are working for people with learning disabilities
and their family members. It checks things such as:
• Housing
• Going to the doctors or the hospital
• If the local swimming baths or libraries are accessible
• How personal budgets are working for people

What it includes
The big things (sometimes called measures) that have beenchecked:
Staying Healthy- This includes lots of things such as getting agood service from the doctors, chemist, dentists or when goingto hospital.
Being Safe- This includes lots of things such as in their ownhome, in hospital or out and about where they live
Living Well- This is about lots of things such as having a sayabout how services should would or being welcomed by theircommunity

2015/16 commitments
• Throughout 2015/16 improve the independence,wellbeing and health of people with learningdisabilities by continuing to roll out care and treatmentreviews to manage discharges and prevent
inappropriate admissions, ensuring annual healthchecks to support physical health, and extending theoffer of PHBs
• By March 2016 all young people with learningdisability leaving residential school, leave with an
Education, Health And Care Plan to support theirtransition to adult services

2015/15 commitments
• During 2015/16 work with partners to develop a
national framework to close inappropriate facilities
and commission more appropriate local and
community-based alternatives
• Building on preliminary work in 2014/15; establish a
national disability mortality review function in 2015/16,
to inform how we shape future services
• Throughout 2015/16 use reliable real-time data to
track progress and inform learning disability work
