LETTER FROM LEADERSHIP

For Baylor Medical Center at Waxahachie’s oncology program, 2013 was a year of maturity and planning for the future.

Using the strong foundation that was built in 2012, Baylor Waxahachie saw increasing demand for its cancer programs

and services. The oncology team continued its commitment to maintaining the quality that earned the hospital recognition

from a number of national organizations, including the American College of Surgeons’ Commission on Cancer.

Much of the oncology team’s focus in 2013 was on planning for the new 16,200-square-foot Charles A. Sammons

Outpatient Cancer Center that will open in the fall of 2014, adjacent to the new Baylor Medical Center at Waxahachie at

I-35 and Highway 287. In addition to designing a facility that will serve the current and future needs of our cancer patients,

we set our sights on recruiting additional physician talent to join the already outstanding team of oncology specialists on

our medical staff. We also continued our efforts to provide focused education for our nurses and other health care

professionals who work with our cancer patients on a daily basis.

A nationwide search was launched for a visionary clinician to lead our cancer program into the future. In early 2014 we

were fortunate to recruit Lynn Whitehair as director of the cancer center. Lynn brings more than 30 years of health care

experience, including the last nine years as regional cancer service line administrator for two of Memorial Hermann’s

hospitals in the Houston area, to our team.

As we anticipate the opening of the new cancer center, with its advanced diagnostic and treatment technology in the

hands of leading cancer specialists, we thank the citizens of Waxahachie and surrounding communities for trusting us

with their well-being. Our heritage of standing side by side with our cancer patients as they fight this formidable disease

remains a constant as our oncology programs and services change for the better.

BAYLOR CHARLES A. SAMMONS CANCER CENTER AT WAXAHACHIE

When someone receives a diagnosis of cancer, a world of difficult

choices lies ahead. One of the first, and most important, decisions

for the new patient is where to get quality care. For people living

in North Texas, that decision became easier with the launch of

the Baylor Charles A. Sammons Cancer Center network. Baylor

Waxahachie is a proud member of the network and worked

diligently in 2013 to achieve the distinction of using the Baylor

Sammons Cancer Center name for its oncology programs by

meeting or exceeding the stringent criteria established by Baylor

Health Care System (BHCS). Baylor Waxahachie is honored to be

part of this collaborative effort to bring quality cancer care to the

citizens of Waxahachie, North Texas and beyond.

In 2013, Baylor Waxahachie maintained accreditation from a

number of organizations for many components of its oncology

program:

• American College of Surgeons’ Commission on Cancer

Accredited Community Hospital Cancer Program

• National Accreditation Program for

Breast Centers

• American College of Radiology

accreditation for breast MRI,

ultrasound, mammography and

stereotactic breast biopsy

• American College of Radiology’s

Breast Center of Excellence Award

All of these accreditations and honors

in addition to participation in research,

education and quality improvement

initiatives signify Baylor Waxahachie’s

commitment to oncology programming

and care and meeting the high standards

of participation in the Baylor Charles A.

Sammons Cancer Center network.

Integration with the network has

allowed Baylor Waxahachie to further

explore opportunities to work closely

with physicians interested in sponsoring

their own clinical studies. Patients are

the major beneficiaries of the network

because they have the assurance that

any hospital carrying the Baylor Charles

A. Sammons Cancer Center brand will

offer consistent quality cancer care. By

receiving care at a network participating

hospital, patients can be treated close

to home for most of their cancer care

needs. As oncology services become

more sophisticated, Baylor Waxahachie

is committed to being a step ahead,

offering more advanced, innovative

procedures with one main focus:

improving patient outcomes.

PROGRAMS OF FOCUS

NICHE Designation

Baylor Scott & White – Waxahachie took

a significant step in improving care for

older adults by becoming the only hospital

in Ellis County to have received

designation as a Nurses Improving Care

for Healthsystem Elders (NICHE) facility.

NICHE is the only national designation

indicating a hospital’s commitment to

excellence in caring for patients 65

and older.

BAYLOR WAXAHACHIE NURSING

Pathway to Excellence

Baylor Waxahachie earned the

prestigious Pathway to Excellence®

designation from the American Nurses

Credentialing Center. The designation

identifies the hospital as having a work

environment where nurses can flourish

and are empowered to provide

outstanding care to patients. It also

substantiates the professional

satisfaction of nurses at Baylor

Waxahachie and identifies it as one of

the best places to work.

Oncology Nursing Forum

In 2013, Baylor Waxahachie

participated in the Baylor Health Care

System oncology nursing forum. The

forum was created to provide oncology

clinicians from throughout the system

with a way to discuss shared concerns/

issues that affect the safety and

outcomes of their oncology patient

population. Lisa Chartrand represented

Baylor Waxahachie at forum meetings.

Baylor Waxahachie continued to reach beyond its walls in 2013,

bringing cancer information and screenings to residents of its

service area. The annual For Women For Life™ symposium

featured physicians on the Baylor Waxahachie medical staff and

oncology team members presenting the latest information on

cancer issues of particular interest to women. It’s A Guy Thing

attracted many area men to hear the latest information on

male-related cancer issues.

The hospital brought cancer information and screenings to health

fairs sponsored by the Waxahachie Chamber of Commerce and

other organizations. Local radio station KBEC aired a health

information show sponsored by Baylor Waxahachie every

second and fourth Friday of the month in 2013. The show covered

a variety of health topics including an interview with Dr. Charles

Lee about lung cancer and an interview with Dr. Katrina Emmett

about cancer prevention and detection.

Another community outreach initiative launched by the Baylor

Waxahachie oncology program in 2013 was mobile screening

mammography. The mobile van visited businesses and school

districts across Ellis County, providing screening mammograms

to employees.

PATIENT SUPPORT +

Lindsey’s journey of breast cancer started with a lump in her

breast she thought was nothing at first. After all, she was only 28

with no breast cancer in her family history. Then the pain began.

She consulted her doctor who referred Lindsey to Baylor.

Mammography, a sonogram and two biopsies identified the

cancer and the course of treatment: chemotherapy, a double

mastectomy, radiation and breast reconstruction.

When Lindsey was wheeled back to her hospital room after her

double mastectomy, she found it decorated with streamers and

balloons — a surprise from her nurse navigator and several mem-

bers of the staff. “The decorations let me know I mattered to them

as a person,” she explains. “It was scary,” she admits, “but my

nurse navigator explained everything and put my mind at ease.”

Today, Lindsey is cancer-free, back to working as a teacher’s aide

and playing with her toddler Olivia. “I’m healthy again,” she says,

“and I owe that to Baylor.”

AMERICAN CANCER SOCIETY

PATIENT SUPPORT +

The American Cancer Society has been an incredible resource to

Baylor Sammons Cancer Centers, including Baylor Waxahachie.

American Cancer Society representatives collaborated with

oncology staff to deliver support, and served on the cancer

committee to help provide resources to fulfill the Commission on

Cancer standards for cancer care.

In 2013, the American Cancer Society engaged with 342 patients

providing 1,010 services at Baylor Waxahachie. All newly

diagnosed patients received a Personal Health Manager kit from

the Society which provided personalized information on the

patient’s specific cancer type, resource information, and tools that

helped patients and caregivers keep appointments, monitor test

results and keep prescriptions organized throughout treatment.

American Cancer Society patient navigators provided free and

confidential support and guidance to all patients and their

caregivers during their cancer journey. Patients received support

from the Society’s wig program as well as transportation services.

Newly diagnosed breast cancer patients received assistance

through the Reach To Recovery® program where Society-trained

breast cancer survivors provided valuable

information and encouragement to

patients in their journey through breast

cancer. In addition, female patients were

introduced to the Look Good Feel

Better® program, dedicated to

improving appearance-related side

effects, and building personal confidence

while undergoing treatment.

As the official sponsor of birthdays, the

American Cancer Society knows how

important each and every birthday can

be. In May of 2013, the Society

celebrated its 100th birthday – one

hundred years of saving lives and twenty

years of collaborating with Baylor

hospitals. The Society saves lives by

helping individuals stay well through

prevention and early detection, helping

them get well by being there during and

after a diagnosis, finding cures through

groundbreaking discoveries, and fighting

back through public policy. In the last two

decades the Society has contributed to

a 20% decline in cancer death rates in

the U.S. Last year the Society and Baylor

hospitals reached over 2,100 patients with

more than 6,000 programs and services;

that’s one in four cancer patients treated

at Baylor hospitals.

Breast nurse navigator Denell Flores saw her role grow

as more patients sought diagnosis and treatment at Baylor

Waxahachie. New to the hospital’s oncology team in 2012,

Flores hit the ground running and in 2013 stayed by the side of

354 women as they underwent a breast biopsy procedure. After

the procedure, she placed a follow-up phone call to all patients

to check on their status and to answer any questions that they

had. Flores also coordinated the following initiatives in 2013:

• Facilitated the breast cancer support group

• Taught breast surgery classes for patients facing the

procedure

• Formed a Relay for Life team to participate in the American

Cancer Society signature fundraising event

• Organized Reach to Recovery® efforts with the American

Cancer Society

• Attended Baylor Health Care System monthly nurse navigator

conferences

• Attended monthly tumor boards and cancer committee

meetings at the hospital.

Flores also received personal honors by being recognized with

service excellence awards and by being nominated to Top 100

Nurses in Dallas Fort Worth.

PATIENT OUTCOME STUDY +BAYLOR WAXAHACHIE INCREASES PASTORAL CARE AND SOCIAL WORK VISITS TO ONCOLOGY PATIENTS

Plan: 90% or greater Colon/Lung/Breast Cancer patients (IP admissions) to receive a documented Care Coordination/SW and Pastoral Care visit during their

admission. At the end of 2012, approximately 87% of Colon/Lung/Breast Cancer patients were receiving a Care Coordination/SW visit and approximately

68% were receiving a Pastoral Care visit.

Do: CC/SW focused on education and reminders for visit and utilizing Eclipsys tools for communication. In August 2013, Pastoral Care worked with the MIDAS

support team to set up a Cancer Diagnosis Readmission Work List Rule to enable a print out for weekend on call staff to be notified of need for patient

visit. Pastoral Care now also documenting all attempts to interact with patients, including failed attempts.

Check: CC/SW achieved annual performance goal of 90%. Pastoral Care performance improved, but then dipped the final three months of the year. Although

Pastoral Care did improve from approximately 68% to 80% they fell short of the goal of 90%. Very short (1 – 2 days), including weekend/holiday stays,

remained challenging for both disciplines, especially for Pastoral Care as weekend/holiday coverage only come to the hospital for urgent pastoral needs.

Act: Both groups will continue their work in assuring that the majority of patients with a diagnosis of cancer receive a visit from both Pastoral Care and Care

Coordination/SW.

CANCER CONFERENCES +

Cancer conferences are an essential forum to provide multidis-

ciplinary, consultative services for oncology patients, as well as

to offer education to physicians and allied health professionals.

Baylor Waxahachie held a monthly general cancer conference

in 2013. The tumor conference reviewed 47 prospective cases

which represented 48.9% of the analytic load.

The top five cancer sites presented included breast, colorectal,

lung, bladder and lymphoma.
