Time hasr- ielded considerable, if not immense . technological advances encompassing basic and ap-plied disciplines and . hence. a large spectrumof therapeutic modalities . The availability- ofthese modalities, ill turn, has increased thedemands upon the pllvsiciazi for precision illdiagnosis and treatment, a principle whicthguided the formulation for this book . We be-liere that a sound acquisition . Of f •undametttalsis a prerequisite to a rational therapeutic ap-proach, asci have therefore devoted a portionof the book to mcchanisms and the balance tothereperrlics .From its simpler origins, astlnla has now
