Medical plan options

TRADITIONAL CHOICE® (INDEMNITY)

In this fee-for-service plan, employees are responsible for getting precertification approvals in the United States and submitting claims. Precertification is not required for emergencies. Deductibles and coinsurance are typical out-of-pocket expenses.

OPEN CHOICE® (PPO)

Our preferred provider organization (PPO) plan lets employees visit any provider they choose — but they typically pay less out of pocket when visiting doctors or hospitals that are part of the Aetna U.S. network.

The Aetna HealthFund Health Reimbursement Arrangement (HRA) is a simple, three-part plan that helps employees take charge of their health and their health care spending. They start out with a fund to help pay for medical expenses — picking and choosing the doctors or services that fit their needs. Preventive care is covered at 100%.

1. The Fund

■ At the start of each plan year, employees receive a fund to help pay for eligible out-of-pocket expenses incurred anywhere in the world!

■ If they incur a medical expense, it gets paid from the fund first.

■ At the end of the year, unused fund dollars roll over to the following year — building for future health care expenses.

Quick fact: Employees receive preventive care coverage — at 100% — without touching their fund dollars!

er No matter where employees travel, we want to get them there, and home, safely and in good health. Our CityHealth ProfilesSM section of our website lets employees look up emergency numbers and other important contact information for hundreds of international locations.

2. The Deductible

■ Once an employee depletes the fund, he or she is responsible for any remaining balance toward the plan deductible.

■ Plan coverage begins once this deductible is satisfied.

■ When expenses are paid from the fund, the deductible is automatically reduced by the same amount — so employees pay less out of pocket to meet the deductible.

■ If the fund grows larger than the deductible, eligible expenses are paid in one of two ways:

> The fund continues to pay 100% of eligible expenses until the fund is gone; then expenses are paid by the medical plan.

> The medical plan provides coverage, but the fund pays coinsurance; when the fund is gone, the employee pays coinsurance.

Quick fact: Employees who grow their HRA balance may build up enough to cover their entire deductible!

3. Medical and Prescription Benefits*

■ After the fund is depleted and the deductible is met, eligible health expenses are covered according to the plan coverage level.

■ If an employee has an AGB prescription plan, eligible drug expenses are also covered once the fund is depleted and the deductible met.
