Les questions importantes dans la LLC
• Pas de difficultés diagnostiques
– La cytométrie est un outil majeur
• Evolution clinique très variable
– Difficile de distinguer formes graves et stables au diagnostic
– Comprendre cette hétérogénéité pour comprendre les déterminants de la progression
• Pas de traitement curateur (hormis allogreffe)
– Importance de comprendre la physiopathologie pour trouver de nouvelles cibles thérapeutiques

Progression LLC = pool prolifératif
• Jusqu'en 2005 : accumulation clone = défaut apoptose
• 2005 : étude in vivo avec eau lourde (Messmer 2005)
Prolifération = composante majeure de l'accumulation chez les patients progressifs
(renouvellement jusqu'à 1% du clone par jour)
• Au sein du clone LLC existe une sous –population de cellules prolifératives (Callissano 2011)

Le devenir des cellules B normales est déterminé par l’exposition à l’antigène
• Les cellules B immatures doivent traverser 2 checkpoints – Dans la moelle, où elles sont générées et sont sélectinnées négativement – Dans la rate, où les cellules qui ont survécu à la sélection négative vont
renconrer les antigènes du soi conduisant à l’anergie et la mort
• Après ces deux checkpoints, les cellules B maturent and et sont capables de migrer dans les ganglions, via les venules endothéliales
• Les cellules B matures répondent alors à l’antigène par une activiation, une prolifération et une différenciation
• Le binding de l’antgène about it à la foramtion d’une signalosome formé d’un complexe de kinases et centré sur le récepteur B

Les mutations somatiques des immunoglobulines
• Phénomène physiologique au cours de la différenciation des lymphocytes B :
- centres germinatifs des organes lymphoïdes secondaires
- mutations des gènes des immunoglobulines dans les parties variables des
anticorps dans le but d’obtenir une meilleure affinité de l ’anticorps pour l ’antigène

Rôle de l’antigène dans la LLCRépertoire restreint
• Plusieurs arguments pour un rôle fort de l’antigène dans la physiopathologie de la LLC
– Répertoire biaisé : utilisation restreinte à quelques dizaines de VH– CDR3 stéréotypés retrouvés jusque dans 20% des LLC– Chaines légères stéréotypées– Mutations somatiques biaisées
• Restriction très importante des possibilités de diversité– Jusqu’à 1% de BCR identiques
• Exposition à l’antigène dans les LLC mutées– Exposition T dépendante– Dans le centre germinatif
• Exposition à l’antigène dans les LLC non mutées– Exposition T indépendante– Probablement répétées de faible intensité en dehors du centre
germinatif– Antigènes polyréactifs, autoantigènes?
• Expression de surface du BCR est faible, mais moins faible chez les non mutées
• Réponse à la stimulation différente

• Signature génomique de la LLC très forte et différente des autres hémopathies lymphoides
• Signature :
• inactivation de voies d’apoptose
• Seulement une centaine de gènes différemment exprimés entre les deux formes,
• Les gènes les plus discriminants des études génomiques comparant mutées et non mutées sont des gènes d'activation B
• L'expression du gène ZAP-70 en particulier, est très fortement corrélée à l'absence de mutations
