6. A pupil who is promoted during the year will be reported in each grade in which he is enrolled.

12. The sum of the ages of all pupils concerned should appear where "Total Age of Pupils " is called for. For example; Total age in a class of 40 pupils is 380. Average age 380/40 = 9.5.

7. In the column for "Final Rating" should be indicated the final rating received by the pupil during the months he was enrolled in the grade for which the report is made. For example, if a pupil is enrolled in Grade I in July reported on DepEd From 18-E-1 both for Grade I and Grade II. His final rating for Grade II will be based upon his work from October to March, and not on those months in which he was studying in Grade I. The term "final rating" signifies either the average of all the periodic ratings according to the averaging system of grading or the last cumulative system of grading, the periodic rating, after the first, is obtained by dividing by 3 the sum of twice the tentative rating and the periodic rating immediately preceding ; and the last periodic rating becomes the final subject rating.

13. In indicating action taken on this form use only the word "promoted and the word "retained" and abbreviate them to Prom., and Ret., respectively.

Division 8. Great care should be exercised to make sure that the total number of days the pupil has attended the grade in both present and the past school years is correctly recorded.

Municipality 9. Under "Remarks" indicate all pupils promoted during the school year to the grade for which the report is made, by writing the words "Promoted from Grade _____, " giving the number of the grade and the date on which the promotion was made. Likewise indicate all pupils promoted to a higher grade from the grade for which the report is made by writing the words "Promoted to Grade ___" giving the number of the grade and the date on which the promotion was made. Indicate all the Grade I pupils who enter the grade for the first time during the school year by writing the words "Entered School ___," giving the date on which the pupil entered.

School Note: The person making this report shall also fill in the blank spaces under "Report on Promotion" on this side of the form.

This form will be made out in duplicate at the close of the school year by each teacher who is in charge of the enrolment and attendance records of any class in Grades I, II, or III. The original copy should be retained on file in the office of the principal of the school and the duplicate sent to the division office for filing.Teacher

10. When the blanks under "Summary of Pupils Enrolled" are filled in, the following points should be born in mind:

Date 2. In the first column will be written the names of all pupils who have been enrolled during the year in register of the class for which this report is made. List names of boys first.

1) The March monthly-enrolment figures given on the first line should agree with the monthly enrolment figures given on the DepEd Form 2 for March. 3. Under "Years in School," write 1, ½, 2, 2 ½, 3, 3½ etc., to indicate the

exact length of time the pupil has been in school from the first time he entered any school to the date of accomplishing this form.

2) The data for the second line should include all pupils promoted to the next higher grade before the end of the school year.

4. The age of pupil as reported on this form should be his age as of March 1. to find his age as of April 1, add to hia age as of the beginning of the school year, recorded in DepEd Form 1 - School Register.

3) The data for the third line may be found by adding the figures on the first line to the figures on the second line.

5. Under "Home Address" should be written the actual residence of the pupil, not a temporary residence which he may have taken up while attending school.

11. Data for the "Summary of Pupils Promoted" may be obtained from the face of the form.

1. This form, accomplished in triplicate, should serve as a permanent record of all promotions during, or at the end of the school year in Grade IV - VI, inclusive.

Circulars: Nos. 24 and 24, s. 1928, 45, s. 1930; 33, s. 1932; 23, s. 1933; 15 and 48, s. 1934; 18, s. 1936; 46, s. 1937; 12 and 44,

s. 1938. 2. The copies of this form for Grade VI should be fully accomplished a week before end of the school year, and those for Grades IV and V at the close of the school year. The original copy should be retained in the office of the principal, the duplicate should be forwarded to the division office as soon as accomplished and approved, and the triplicate should be kept on file in the office of the district supervisor.

CURRICULUM 3. A separate report for each section of each grade of each curriculum is required. 4. Names of boys should be written first, followed by names of girls listed seprately. Pupils' names should be written in the same order on all copies. The total number of pupils listed should agree with the yearly enrolment reported on DepEd Form 2 for March plus the number promoted from the grade to a higher grade during the school year. These should be listed separately at the bottom of the form.

Division 5. Under "Years in School," write 4, 41/2, 5, 51/2, 6, 61/2, etc. To indicate the exact length of time the pupil has been in school from the first time he entered any school to the date of accomplishing this form.Date of close of School Year

Certificates 6. The age of the pupil as recorded on this form should be his age as of the end of the school year as recorded in DepEd Forrm 1 (School Register)

7. Opposite the name of each pupil who drops out during the year should be entered such brief explanation of the cause as "III," "Deceased," "Dropped January 12", etc.

8. Under the "Total Number of Days in Grade," indicate total number of days the pupil has attended the grade in current and preceding school years.

10. The data for "Average" at the bottom of the sheet will be found by adding the entries in the column and dividing the total thus obtained by the number of pupils for whom final ratings are entered.

11. The term "Final Rating" signifies either the average of the periodic ratings in a subject according to the averaging system of grading.

12. In indicating action taken on this form use only the word "Promoted" and the word "Retained" and abbreviate them to "Prom. " and "Ret." respectively.

13. In Grade IV and in the intermediate grades, the general average of each pupil shall obtained by dividing the sum of the subject final ratings by the number of subjects or combinations of subjects entered under item 16 following. Each subject or combination of subjects will have a weight of one. In these grades a minimum average of 75 percent is required for promotion.

14. When the blanks under "Summary of Pupils Enrolled" are filled in the following points should be remembered: the March monthly enrolment given on the first line should agree with the March monthly enrolment as it appears on DepEd Form 2. The data for the second line should include all pupils promoted to the next higher grade during, not at the close of, the school year. The data for the third line are found by adding numbers given on the first and second lines.

15. The sum of ages of all p[upils concerned should appear where "Total Age of Pupils" is called for.

16. In indicating the subjects under the column "Final Rating In" arrange the subjects alphabetically.
