We want to encourage you to follow the links and obtain information that will help and encourage you as a GAPAN member. This is your GAPAN newsletter, website, and Facebook, so please share what is happening in your district with the other GAPAN members. We would appreciate any articles, pictures, or hospital recognitions or awards of our members. If any are involved in hospital research and evidence based practice or any clinical situations or process improvement projects please share this wisdom and expertise among our GAPAN membership. By working together as a team we can reach our goals and our GAPAN mission to promote quality care and enhance excellent Perianesthesia nursing practice through education and leadership. FROM GAPAN’S NEW STATE PRESIDENT

DANA NATIONS, RN, BSN Hello Everyone! My name is Dana Nation. I am honored to be elected as your GAPAN President for the 2015-16 term. Perianesthesia nursing is an exciting specialty that I’ve been a part of for about 15 years. I look forward to sharing my passion for our field during my tenure. First, I hope you will help me continue to educate our colleagues about ASPAN/ GAPAN and promote the organization as a leader among professional nursing organizations. Many perianeshesia nurses do not realize that ASPAN Standards often direct hospital policies for which they are held accountable. I would like to promote increased awareness in this area. I would like to see increased involvement among our members at the local, state and national levels. I am personally extending an open invitation to all GAPAN members to join us for our state board meetings. This is a great way to learn about how the organization functions and make decisions about how much and in what capacity you would like to be involved. We will schedule the meetings for the year as soon as possible and publish them on the GAPAN website. “Willingness to Serve” forms are available on the ASPAN website. I would also like to encourage each of you to explore how you might participate in our organization at the national level. The Leadership Development Institute (LDI) presented by ASPAN is also a great way to learn more about the organization. It will be held on the 3rd weekend of September 2016 and I encourage you to make plans to attend if you have interest in taking a leadership role in ASPAN/ GAPAN at any level. I promise that you will learn a great deal and have the opportunity to network with people from other components. You will also identify resources that you can call or email for any questions that you encounter. GAPAN state executive board members and district presidents will be invited to participate with some financial assistance from the GAPAN treasury. We are fortunate to have many experienced nurses as a part of our organization. Unfortunately, we have a deficit of younger nurses who

are just beginning their careers. We need to continue to reach out to nursing schools and invite nursing students to attend GAPAN events. Perhaps we can facilitate interest in the art of perianesthesia nursing among those students and new graduates. I believe we are a part of a great professional nursing organization. I look forward to working with our other board members this year to continue to build our membership by reaching across multiple generations. 3ND SHINING STAR AWARD FOR GAPAN

NOW WE HAVE THREE SHINING STAR AWARDS FOR GAPAN ABPANC’s Shining Star Award publicly recognizes ASPAN Components for supporting and encouraging CPAN® and CAPA® certification at the local level. All Components, meeting the criteria are awarded the Shining Star Award at the annual CPAN/CAPA Celebration Event, held in conjunction with the ASPAN National Conference. The award, a beautiful silver star, is engraved with the Component’s name and year of award. This star statue can be displayed proudly at Component meetings!

ACCOMPLISHED GOALS LETTER FROM THE 2014 GAPAN PRESIDENT- KEISHA FRANKS

Hello to my fellow perianesthesia colleagues, As my second term comes to an end, I would like to take a moment to reflect on my Presidency. I would like to start by looking back on the Strategic Goals set or both years. Sept 2013-2014 (1) Improve Communication (2) Strengthen Component Viability (3) Mentor New Members. Sept 2014-2015 (1) Member retention and development (2)Component Revitalization (3) Social Media Development and Interaction. I feel GAPAN has done well with communication. The Awakener, our official newsletter and website was awarded the 2014 ASPAN Component Newsletter Award for 15 plus pages. Most of our local districts have either a Facebook or Twitter pages which are used to disseminate information to members and nonmembers. Districts have access to the ASPAN conference line for go-to meetings. Job well done GAPAN!!! My major concern and issues that I reluctantly pass to my successor are membership retention and component viability. These are issues that GAPAN and many other components have difficulty with. I will continue to work with the President and the Board to help strengthen and revitalize Georgia components. I have met with Doug Hanish (Marketing and Communications Manager, ASPAN) and fellow State Presidents. We all realize that there are several obstacles that our professional organizations are competing with. These issues cause the Perianesthesia nurse to choose where to devote their time and money. I will continue to encourage the perianesthesia nurse to embrace GAPAN’s mission: To establish an organization for the mentoring and advancement of Perianesthesia nursing with an emphasis on continuing education, community service and research. In order to uphold this mission we must continue to focus on our specialty. We must stay actively involved at all levels (district, state and national). We must maintain competencies that are specific

It has been a rewarding two years and I have learned a lot about myself as your President. Thank you all for the support to me and GAPAN. 2015 GAPAN/GSA Perianesthesia Nurse Scholarship

Sandra Klein, MSN, RN, CAPA, CPAN Senior Staff Nurse Educator for Perianesthesia

Department Georgia Regents Medical Center

Sandra Klein is a native of Augusta, GA. She has been employed at Georgia Regents Medical Center since November 2012 as Nurse Educator for Perioperative Services. She is responsible for 5 distinct areas in perioperative services including PACU, PAT, 8 West , PPC, ENDO, and patient holding. She has over 25 years of perianesthesia experience and has completed successfully her dual certification for perianesthesia nursing. On September 20, 2015 she was awarded the Georgia Association of Perianesthesia Nurses/Georgia Society of Anesthesiologists scholarship based on dedication in pursuit of continuing education and clinical excellence in perianesthesia nursing. She was the first in the August district of the Georgia Association of Perianesthesia Nursing (GAPAN) to receive dual certification as Certified Post Anesthesia Nurse (CPAN) and Certified Ambulatory Perianesthesia Nurse (CAPA). Sandra was nominated as CSRA Nurse Educator of the year in 2014 at GNA Showcase and won the Spirit of Nursing award from GAPAN in 2014. She is very active in GAPAN and is serving a third term as Vice president for the district and is newly elected to the State level as Membership Chair. She volunteers her time as ABPANC certification coach for nurses pursuing CAPA/CPAN certification. Sandra is a participant in the clinical ladder program and is currently a CNIV and a member of the American Association of Critical Care nurses (AACN). Additionally she is involved with many committees at Georgia Regents University (GRU). She is the facilitator for the Perioperative Shared Governance monthly meeting, and department joint commission readiness monthly committee. She is the co-chair for the hospital moderate sedation committee and attends several quality meetings weekly and monthly. Mrs. Klein has a BSN from the Medical College of Georgia and Master’s degree in nursing with a specialty in health education from the University of Phoenix. She is certified as a basic life support instructor (BLS) and advanced cardiac life support (ACLS) instructor for the American heart association. To date her most passionate professional accomplishments have been to mentor ten nursing staff to complete nursing certification and assist twelve nurses to successfully completing the clinical ladder program. She enjoys collaborating with the medical team to improve and implement changes in quality and safety concerns.

A Review of Charting: A Legal Document Janice A. Wilson RN, BSN, CAPA

The patient’s chart is a legal document. Your documentation is of extreme importance and must provide a complete and accurate accounting of the patient’s condition and the care you provided.2 Your documentation record should be complete, accurate, and legible in order to defend yourself against allegations of improper care. Your documentation can discourage a plaintiff from pursuing a legal claim or can provide the fuel for a lawsuit.3,4 The law defines negligence as:

Negligence is the failure to provide a patient with the standard of care that a reasonably prudent nurse would exercise under the same or similar circumstances. 5,6

Patient’s attorney must prove four elements to prove that the nurse was negligent:

1. Thenursehadadutytoprovidecaretothepatientandtofollowanacceptablestandardofcare.

2. Thenursefailedtoadheretothestandardofcare.3. Thenurse’sfailuretoadheretothestandardofcarecausedthepatient’s

injuries.4. Thepatientsuffereddamagesasaresultofthenurse’snegligentactions.5,6

By practicing in a nursing specialty area, American Society of PeriAnesthesia Nursing (ASPAN), we must be familiar with and demonstrate compliance with the Standards developed by our specialty organization, the Perianesthesia Nursing Standards, Practice, Recommendations and Interpretive Statements as published by the American Society of PeriAnesthesia Nurses, which serves nurses practicing in all phases of peranesthesia, postanesthesia care, ambulatory surgery and pain management. It is also important to be familiar with the standards developed by The Joint Commission, the National Council of State Boards of Nursing, and the American Nurses Association, 6,7,8 as well as following documentation policies established by your employing facility. The saying “If it wasn’t documented, it wasn’t done” is still valuable today. Your documentation should show that you continuously assessed your patient’s condition and monitored their progress. Documentation of hourly rounding improves the quality of patient care by showing frequent assessment and monitoring. All identified patient problems, nursing actions, and patient responses should be documented.11 Nursing documentation that contains misspelled words or grammatical errors can lead lawyers and jurors to conclude that the nurse is uneducated and/or careless.3 Your charting should be

Examples of Illegal Tampering With Medical Records6

treatmentsconstitutefalsificationofrecords.6 When Charting by exception (CBE) take extra precautions 6,9 CBE documentation does not give a clear, accurate description of the patient’s condition; therefore the nurse needs to write the patient’s condition out in a narrative note to allow the reconstruction of an accurate picture of the patient condition in the future. In addition anything out of the ordinary needs to be written in a narrative note. Describing a patient’s behavior as uncooperative, difficult, manipulative, or referring to the patient in a sarcastic manner alerts the patient’s lawyers that a nurse did not respect or value the patient,10 therefore make descriptions only by giving factual and impartial descriptions without additional commentary. Recognize and respond to complications. 11 Assess and monitor patients regularly and report a significant change in a patient’s condition to physician. Nurses should report situations in which a patient’s condition undergoes

Documenting in Emergencies 6,7 Make sure your documentation addresses these issues: 1. Thepatient’sconditionbeforetheemergency2. Thepatient’sconditionwhentheemergencybegan3. Whentheemergencyoccurred4. Whenthephysicianwasnotified5. Whatinterventionswereusedandwhentheywerestarted6. Howthepatientrespondedtotheinterventions

If, in your professional judgment, you consider the physician orders placing the patient in jeopardy, you must intervene on the behalf of the patient and clarify the treatment plan or verify the medication with the physician. The nurse must be persistent in an attempt to notify and convince the physician of the seriousness of the situation. If a change in a patient’s status warrants notifying the physician and a potential change in the treatment or medication, the nurse must be able to communicate essential information in a clear and logical manner that expedites understanding and intervention.1.3.6 Organizing Your Reporting Data4

Document each time you phone a physician.4 Chart the details of your message and the physician’s response.4,6 Specify the details you reported, the time you called, the new orders or no orders received and additional actions you take.4 Always note in the chart the specific change in the patient’s condition or diagnostic test result that prompted your call to the physician. Documentation that reflects the nursing process, including

1. competentassessment,2. frequentobservation,3. timelyandaccuratereporting,andthe4. useofthechainofcommand,ifnecessary,oftenwillprotectthenursefrom

accusationsofnegligence—evenwhenthereisapooroutcome.10 Legally credible documentation provides an accurate written record of the care your patient received and evidence that you met an acceptable standard of care. Let your charting tell others that you did all you were expected to do in providing your patient with quality nursing care.4,6
