SECTION 2 – PROVIDER LIST Section 2 lists the agencies currently authorized to provide services to ATR Ohana clients in alphabetical order. Following the agency information is a list of the ATR Ohana service categories currently authorized for that agency. In cases where a business may have another title, particularly when the alternate name of the agency is significantly different than their legal business name, both listings were included in Section 2. The intent is to increase the likelihood that clients will more easily find the agency they are seeking. ATR Ohana enrollment is available ONLY through ATR Ohana Assessor agencies. All ATR Ohana services must be accessed AFTER enrollment through the ATR Ohana Assessor agency. Open enrollment is available when the agency information is in BLACK. Maximum enrollment has been reached when the agency information is in RED. Additional eligibility criteria, enrollment guidelines, and information about ATR Ohana Project is listed on the ATR Ohana website. Funding limits for ATR Ohana also limit the number of clients that can be enrolled in, and served by, agencies in the ATR Ohana network. When ATR Ohana funds assigned to an individual agency are fully spent, the agency information will be shown in RED to signify that the agency is “at capacity” for ATR Ohana. Agencies whose listing in the ATR Ohana Provider Directory may be able to serve clients through other funding sources, in addition to ATR Ohana. A RED listing in the ATR Ohana Provider Directory only signifies that the agency is not currently able to accept new ATR Ohana clients for the indicated ATR Ohana service(s). To enroll in ATR Ohana, please contact an ATR Ohana Assessor. The Assessor agency is the “gatekeeper” for ATR Ohana. Assessors assure that individuals interested in ATR Ohana are eligible to be enrolled, and understand the ATR Ohana grant mandates and expectations for their enrollment in ATR Ohana funded services. Individuals who choose to enroll in ATR Ohana services must be eligible for participation under SAMHSA CSAT’s guidelines for the ATR Grant. ATR Ohana Assessors are authorized to provide ONLY Assessor services to ATR Ohana clients for the ATR Ohana Project. All other ATR Ohana funded services are provided by other agencies in the ATR Ohana network of providers. Other non-ATR Ohana services may be available at the agencies listed in this Directory, including Assessor agencies. Individuals enrolled in ATR Ohana are encouraged to ask their Assessor and other ATR Ohana provider staff about the other services provided by the agency that are funded by other means outside of the ATR Ohana Project. ATR Ohana is considered a “payer of last resort.” Individuals who may be eligible for services

CLEAN AND SOBER HOUSING - DESCRIPTION: ATR Ohana Clean and Sober Housing agencies provide the following ATR Ohana services:

- Clean and sober shelter to homeless individuals needing a structured living arrangement to support recovery efforts

- Multi-person, non-related individuals living situations - Guided skill building for managing tasks of daily living - Support and practical instructions for moving toward independence from

subsidized housing in a non-using environments with rules and expectations - Living situations that promote health, wellbeing, safety, and sobriety of all

GAP SERVICE - DESCRIPTION: ATR Ohana Gap Service agencies provide the following ATR Ohana services:

- practical assistance to the client in the event of special circumstances - Government Performance and Results Act Follow-Up Screening (GPRA)

ATR Ohana Gap Services require written, direct approval of the ATR Ohana Quality Assurance Monitor who will assist the Gap Service provider staff in determining:

- Whether the stated need is sufficiently justified and related to the client’s recovery - The amount of the need, which shall be confirmed by presence of an original

receipt for the exact amount of the Gap Service requested - Whether the agency is able to front the cost of the Gap Service, and await

reimbursement from ATR Ohana

OUTPATIENT CLINICAL TREATMENT - DESCRIPTION: ATR Ohana Outpatient Clinical Treatment agencies provide the following ATR Ohana services:

- Adolescents: Level I – Outpatient Clinical Treatment, which shall be delivered at a frequency of service less than six (6) hours per week.

Level II.1 – Intensive Outpatient (IOP) Clinical Treatment, which shall be delivered at a frequency of service between six (6) hours and twenty (20) hours per week.

- Adults: Level I – Outpatient Clinical Treatment, which shall be delivered at a frequency of services less than nine (9) hours per week. Level II.1 – IOP Clinical Treatment, which shall be delivered at a frequency of services between nine (9) hours and twenty (20) hours per week.

The following service definitions shall be the basis for determining the type of service delivery:

- Individual service is provided in a one-on-one face-to-face setting. - Group service is provided face-to-face with one or more other clients receiving

the same clinical substance abuse treatment service, at a ratio not more than eight (8) clients to one (1) clinician.

- Family/Marriage service is face-to-face interaction with the client and at least one (1) or more of the client family members (e.g., spouse, sibling, parent, child, extended family member, surrogate family member, or other individuals. determined by the client to be a significant person in the client’s recovery support network), at a ratio not more than eight (8) client and family to one (1) clinician.

The Outpatient Clinical Treatment services assist individuals with: - Developing and maintaining positive relationships with individuals and agencies

important for their recovery - Wellness and recovery at the appropriate level of care in the least restrictive

environment - Government Performance and Results Act Follow-Up Screening (GPRA)

PASTORAL COUNSELING - DESCRIPTION: ATR Ohana Pastoral Counseling agencies provide the following ATR Ohana services:

- One-on-one or group meetings with an ordained minister, priest, rabbi, imam, monk, or other advanced level qualified, authorized and endorsed faith expert to study the application of a religious text to recovery

- Support during a crisis - Assistance developing a faith-based recovery plan - Instruction in religious rituals that provide meaning to the individual with

assistance in determining how faith rituals can replace substance abuse behaviors - Discussion about the philosophy of religion or integration of spiritual tenets into

daily living and sobriety - Opportunities to incorporate faith and specific religious beliefs and convictions in

the treatment and recovery process

The Pastoral Counseling services assist individuals with: - Developing the individual’s own concept of spirituality and its role in sustaining

recovery - Government Performance and Results Act Follow-Up Screening (GPRA)

RECOVERY MENTORING - DESCRIPTION: ATR Ohana Recovery Mentors provide the following ATR Ohana services:

– One-on-one, face-to-face mentoring and encouragement – Assistance finding safe, appropriate childcare for dependent children – Assistance locating and securing long-term appropriate, clean and safe living

arrangements where alcohol and other drug use in not likely to be accepted or encouraged

– Guided skill building for activities of daily living – Lay person or peer coaching in the client’s chosen religious or cultural

community – Government Performance and Results Act Follow-Up Screening (GPRA)

SOBER SUPPORT ACTIVITIES - DESCRIPTION: ATR Ohana Sober Support Activity agencies provide the following ATR Ohana services:

- Assistance to individuals interested in building a clean and sober lifestyle - Opportunities for healthy alternatives to using controlled substances:

o cultural activities o church or faith social activities o athletic activities

- Connections to new friends who are not likely to use alcohol or drugs - Support while individuals develop new, positive relationships - Linkages with treatment and other recovery support services, as needed - Help to individuals looking to identify strengths and needs in recovery - Goal setting around interests, wellness, and other challenges faced in recovery - Government Performance and Results Act Follow-Up Screening (GPRA)

Sober Support Activity providers help the client take practical steps to explore and engage with health activities and sober social experiences.
