Participation of a minimum of 04 (FOUR) athletes from 03 (THREE) countries is required to confirm

the event. If this criteria is not met some events can be cancelled or combined.

Each country is required to bring its own escort athlete to the competitions.

Please send this form to the Organizing Committee by fax or e-mail no later than 15 December 2010

Insurance:

All athletes are strongly advised to take out personal accident insurance for the duration of the

Games. Proof of such will be requested at time of entry.

Photographic & Film Records:

Action from IBSA World Games 2011 will be captured on film and photographic images. If

you do NOT wish your image to be used by IBSA for future publication, please tick this box
