Regardless of whether or not your son is thinking of participating in a sport at
Christian Brothers Academy, please complete the following.
By signing below, I confirm that my son has NOT participated on a high school
(Grades 9-12) athletic team while he was in the 6th, 7th, or 8th grades.
*Interpretation: By signing this, you are stating that your son has not competed on an actual
high school team. (ex. Christian Brother Academy or Colts Neck High School) before he was in
high school. In some areas of the country, they allow middle school students to compete on
the high school team. This will affect your son’s eligibility in New Jersey.
**Note: this does NOT include travel teams, club teams, academy programs, camps or clinics.
This ONLY applies if your son represented the ACTUAL HIGH SCHOOL TEAM.

TUITION AGREEMENT

enrolled at Christian Brothers Academy, I (we) expressly represent that we have the financial ability to enter into this tuition
agreement with the Board of Trustees of CBA.
2. I (we) agree to pay the yearly tuition to CBA for regular educational services through FACTS Management Co. Regular
educational services are defined as those that do not require Child Study Team (CST) evaluation, classification and
programming, special classroom testing, as well as instruction by a special education teacher. CBA is unable to offer
Special Education services or programs because of the open ended and possible extensive involvement of staff and
resources this could require. By signing this agreement, I (we) am (are) stating that our son is not a classified student
in any school district. Furthermore, I (we) agree that should CST referral be deemed as necessary, I (we) may have to
withdraw our son no later than the end of the marking period in which the determination is made.
3. Tuition payments must be made using FACTS Management Co. and the Tuition Payment Plan that is presently being
offered. It is agreed that I (we) will adhere to the selected tuition payment schedule. It is also understood that services
(report cards, student records, senior transcripts, overseas trips, etc.) may be withheld until the tuition account is current.
Tuition payments are automatically drawn against your financial institution/credit card on the date(s) designated by you
per the payment plan you choose.
4. It is agreed that registration for the next school year will be contingent upon all tuition for the preceding school year being
paid in full.
5. In split parental/guardian arrangements (e.g., divorce), both parties are required to provide a signed Tuition
Agreement and set up two distinctive tuition accounts with FACTS Management Co. If a split
arrangement is to be enacted, please indicate by checking here __________ .
6. It is agreed that I (we) may withdraw our son at any time prior to the commencement of the following school year.
However, it is also agreed withdrawal would pose a significant setback to CBA given the school's stringent efforts
to accurately budget and prepare for the education of its students on an annual basis. Therefore, it is agreed and
understood that in the event that I (we) withdraw our son prior to the commencement of the following school year, the
tuit ion deposit in the amount of $1,000.00 will be 100% refundable up to 2/15 of that year (or the next Business Day if
2/15 falls on a holiday or weekend), and thereafter, no refund will be given. I (we) further agree that any amount of the
tuition deposit forfeited in accordance with the terms set forth above shall be liquidated and agreed damages.
7. It is agreed that I (we) may withdraw our son at any time during the school year in which he is attending CBA. At the time
of withdrawal, the tuition bill will be paid in accordance with the "Tuition Refund Policy" as outlined in the current CBA
Handbook/Calendar before the appropriate student records will be forwarded to the school chosen.
8. It is agreed that the above conditions will be met for the entire time my (our) son remains a student at Christian Brothers
Academy.

FACTS Management Agreement #
FRESHMEN MATHEMATICS COURSE FOR 2018-19
1. Approximately 70% of incoming freshmen will take Algebra I for their first year of high
school mathematics.
2. Students who are taking a full year of high school Algebra in 8th grade may register to sit for
the Algebra I Exemption Exam. Material covered by the aforementioned exam can be found
in any standard Algebra I textbook. As further guidance, below is a general listing of topics
tested by the Algebra I Exemption Exam.
a. Students who demonstrate their mastery of Algebra I by successfully completing the
Algebra I Exemption Exam will be placed in Geometry for their freshman year.
b. Students who place into Geometry Honors as a result of their performance on the
November entrance exam and passing the Algebra I Exemption Exam will have the
opportunity to take the six week Summer Honors Geometry course offered at CBA.
Successful candidates will be contacted with information regarding the aforementioned
course.
c. Students who successfully complete the Algebra I Exemption Exam and have taken a full
year of Geometry in 8th grade may register to sit for the Geometry Exemption Exam at a
later date.
i. Students who demonstrate their mastery of Geometry by successfully completing the
Geometry Exemption Exam will be placed in Algebra II/Trigonometry for their
freshmen year.
ii. Students who successfully complete the Algebra I as well as the Geometry Exemption
Examinations and have taken a full year of a math course higher than Geometry will
have the opportunity to sit for other exemption exams.

MEDIA CONSENT AND RELEASE FORM Class of 2022
Various photographs or videos are taken of our students for use in Pegasus: The Yearbook of
Christian Brothers Academy and The Academy Torch throughout the school year. We may
utilize some of these images for promotional materials for the school. Additionally, students
may be interviewed and quoted for various articles or publications. We ask that you indicate
below whether or not you give CBA the authorization to allow us to include your son’s
photograph and/or quote in CBA brochures, pamphlets, on the website and in other CBA
related materials which we may publish or distribute from time to time. Please sign the form
when completed.

Under the provisions of N.J.S.A. 18A: 58-37.1 et seq., I hereby request the Middletown (Public School
District) to loan textbooks to Christian Brothers Academy (Nonpublic School) in which my child is
enrolled. I certify that my above named child and I are residents of the State of New Jersey. I understand
that the Board of Education of the public school district in which the nonpublic school is located with
state funding is responsible for providing the loan of textbooks to nonpublic school students pursuant to
law and regulations.

Setting up a FACTS Payment Plan for CBA
In order to enroll your son in Christian Brothers Academy, a FACTS account must
be established and the $1,000 tuition Deposit must be paid prior to Enrollment day.
NOTE for those with a current CBA FACTS account: If you already have an
account for an upper-class CBA student, please log on to your current account.
Click “Set up Payment Plan for the New Student Tuition Deposit 2018-19 Term”
and make your payment. You MUST use the same banking, payment plan, and
payment date that is currently on file with FACTS. Once you have added the new
student to your existing CBA account, you may skip to the item below marked
“Review” and follow from there.
To create a New Account, follow the steps below.
1. Go to factsmgt.com and click on the “Log In” tab in the top right.
2. On the next page, under “New Account” click the box “Create user name and
password”. From this point, you will be creating a new account, simply follow all
prompts.
o Create a user name, password, and select security questions.
o Choose the appropriate school year and click “Continue”.
o “Contact Information”: Review the information and choose a correspondence
method.
o “Students”: Each student included in the payment plan must be added—
important for parents with twins.
o “Plan Options”: Select a payment plan and click “Next. Please note the
information about fees with the various plans.
o “Payment Details”: Enter your financial account information.
o “Payment Schedule”: Select your payment date and review your schedule.
o “Review”: You can select the links to the right to go back and edit the entries.
Check the box to “Agree to the Terms & Conditions” and click “Submit”.
o “Thank You”: Your confirmation page will include your agreement
number, this is a nine digit number beginning with 10-------. This number
must be written on the bottom left hand side of the CBA Tuition
Agreement. This form will be submitted on Enrollment Day.
o Once your account is created, you will have access to the Grant & Aid portal,
via FACTS Management.

FRESHMAN WORLD LANGUAGE COURSES FOR 2018-19
1. All CBA students are required to begin their World Language study in the ninth grade and complete three
consecutive years of that same language at CBA.

GYM UNIFORM ORDER
