ARTICLE KS-2 OBTAINING AND USING CODE STAMPS KS-200 CODE STAMPS BEARING OFFICIAL SYMBOL A Certificate of Authorization to use the Code U3 or UV3 (see Appendix C) symbols shown in Fig. KS- 100 and Fig. KR-401 will be granted by the Society pursuant to the provisions of the following paragraphs. Stamps for applying the Code symbol shall be obtained from the Society. KS-210 APPLICATION FOR AUTHORIZATION (a) Any organization desiring a Certificate of Authori- zation shall apply to the Boiler and Pressure Vessel Committee of the Society, on forms issued by the Society, 1 specifying the stamp desired and the scope of Code activities to be performed. (b) When an organization intends to build Code items in plants in more than one geographical area, either separate applications for each plant or a single applica- tion listing the addresses of all such plants shall be submitted. Each application shall identify the accredited Authorized Inspection Agency providing Code inspec- tion at each plant. A separate Certificate of Authorization will be prepared and a separate fee charged by the Society for each plant. (c) Each applicant shall agree that each Certificate of Authorization and each Code symbol stamp are at all times the property of the Society, that they will be used in accordance with the rules and regulations of this Division of the Code, and that they will be promptly returned to the Society upon demand, or when the applicant discontinues the Code activities covered by this certificate, or when the Certificate of Authorization has expired and no new certificate has been issued. The holder of a Code symbol stamp shall not allow any other organization to use it. 1 The application forms and related information and instructions may be obtained by writing to the Secretary, ASME Boiler and Pressure Vessel Committee, Three Park Avenue, New York, NY 10016-5990. 202 KS-220 ISSUANCE OF AUTHORIZATION (a) Authorization to use the Code symbol stamps may be granted or withheld by the Society in its absolute discretion. If authorization is granted and the proper administrative fee paid, a Certificate of Authori- zation evidencing permission to use any such symbol, expiring on the triennial anniversary date thereafter, will be forwarded to the applicant. Each such certificate will identify the Code symbol to be used and the type of shop operations, field operations, or both for which authorization is granted (see Appendix C). The certifi- cate will be signed by the Chairman of the Boiler and Pressure Vessel Committee and the Director of Accreditation. (b) Six months prior to the date of expiration of any such certificate, the applicant shall apply for a renewal of such authorization and the issuance of a new certificate. (c) The Society reserves the absolute right to cancel or refuse to renew such authorization, returning, pro rata, fees paid for the unexpired term. (d) The Boiler and Pressure Vessel Committee may at any time make such regulations concerning the issuance and use of Code symbol stamps as it deems appropriate, and all such regulations shall become bind- ing upon the holders of valid Certificates of Authori- zation. KS-230 INSPECTION AGREEMENT (a) As a condition of obtaining and maintaining a Certificate of Authorization to use the U3 Code symbol stamp, the Manufacturer shall have in force at all times an inspection contract or agreement with an accredited Authorized Inspection Agency as defined in KG-431 to provide inspection services. This inspection agreement is a written agreement between the Manufacturer and the inspection agency which specifies the terms and conditions under which the inspection services are to be furnished and which states the mutual responsibilities of the Manufacturer and the Authorized Inspectors. certificate holder shall notify the Society whenever hisagreement with an accredited Authorized InspectionAgency is canceled or changed to another accreditedAuthorized Inspection Agency.

(b) Neither Manufacturers nor Assemblers of pressurerelief valves are required to have an inspectionagreement with an accredited Authorized InspectionAgency.

KS-240 QUALITY CONTROL SYSTEM

Any Manufacturer or Assembler holding or applyingfor a Certificate of Authorization to use the U3 orUV3 stamp shall demonstrate a Quality Control Systemto establish that all Code requirements shall be met.The Quality Control System shall be in accordancewith the requirements of Appendix 2.

KS-250 EVALUATION FORAUTHORIZATION ANDREAUTHORIZATION

(a) Before issuance or triennial renewal of a Certifi-cate of Authorization for use of the U3 stamp, theManufacturer’s facilities and organization are subjectto a joint review by a representative of his inspectionagency and an individual certified as an ASME designeewho is selected by the concerned legal jurisdiction. Awritten description or checklist of the Quality ControlSystem which identifies what documents and whatprocedures the Manufacturer will use to produce aCode item shall be available for review.

(b) A written report to the Society shall be madejointly by the jurisdiction and the accredited AuthorizedInspection Agency employed by the Manufacturer todo his Code inspection. This report is then reviewedby the Subcommittee on Boiler and Pressure VesselAccreditation, which will either issue a Certificate ofAuthorization or notify the applicant of deficienciesrevealed by the review. In the latter case, the applicantwill be given an opportunity to explain or correct thesedeficiencies.

(c) Certificates of Authorization will be endorsed toindicate the scope of activity authorized. Authorizationmay include field operations if the review team deter-mines that these operations are adequately describedin the Quality Control Manual and this determinationis accepted by the Society.

(d) Before issuance or renewal of a Certificate ofAuthorization for use of the UV3 stamp, the valveManufacturer’s or Assembler’s facilities and organiza-tion are subject to a review by an ASME designee.A written description or checklist of the Quality ControlSystem, which identifies the documents and proceduresthe Manufacturer or Assembler will use to produceCode pressure relief valves, shall be available forreview. The ASME designee shall make a written reportto the Society, where the Subcommittee on Boiler andPressure Vessel Accreditation will act on it as describedabove.

(e) The purpose of the review by an ASME designeeis to evaluate the applicant’s Quality Control Systemand its implementation. The applicant shall demonstratesufficient administrative and fabrication functions ofthe system to show that he has the knowledge andability to produce the Code items covered by hisQuality Control System. Fabrication functions may bedemonstrated using current work, a mock-up, or acombination of the two.

(f) The Manufacturer may at any time make changesin the Quality Control System concerning the methodsof achieving results, subject to acceptance by the Author-ized Inspector. For Manufacturers and Assemblers ofUV3 stamped pressure relief valves, such acceptanceshall be by the ASME designee.

(g) For those areas where there is no jurisdiction orwhere a jurisdiction does not choose to select anASME designee to review a Manufacturer’s facility,that function shall be performed by an ASME designeeselected by ASME. Where the jurisdiction is the Manu-facturer’s inspection agency, the joint review and jointreport shall be made by the jurisdiction and the ASMEdesignee.

KS-260 CODE CONSTRUCTION BEFORERECEIPT OF CERTIFICATE OFAUTHORIZATION

When used to demonstrate his Quality Control Sys-tem, a Manufacturer may start fabricating Code itemsbefore receipt of a Certificate of Authorization to usea Code symbol stamp under the following conditions:

(a) the fabrication is done with the participation of theAuthorized Inspector and is subject to his acceptance;

(b) the activity is in conformance with the applicant’sQuality Control System;

(c) the item is stamped with the appropriate Codesymbol and certified once the applicant receives hisCertificate of Authorization from the Society.
