All members employed by UCLA hospital will understand:

a) All patient’s record must be seen only by patient’s assigned employee.

b) Staff that opens a patient file will attach an idea number / secure password.

c) All staff will enter their social security into the system for verification

All staff must attend a 4 hour HIPAA orientation on patient privacy before being hired on staff.

All staff must be required to take a quiz on patient privacy.

All staff must protect patient privacy and must not discuss any medical records.

Security classes at UCLA

All employees will be finger printed

All employees will have their picture taken

All employees that are (see note below).

No supervisor shall share their special pass code with any employee.

a) All entertainment industry patient’s medical record will be utilized by employees that are servicing the them.

b) Finger print initiation (see below)

All rules must be followed at all times

Failure to follow rules

No talking to the media without hospital representatives (ex: lawyer)
