Y O U R B E N E F I T S

Our goal is to provide a comprehensive benefits package with your health and financial well-being in mind. The benefits we provide offer flexibility and conve-nience, so you can choose the plan that’s right for you and your family.

E L I G I B I L I T Y

For Federal Employees Health Benefits, you are eligible on the first day of the following pay period from your date of hire.

For all other benefits, you are eligible on the first of the month following the date you were hired, if you are scheduled to work 24 hours or more per week.

E N R O L L M E N T

You can enroll for coverage within 30 days of your eligibility date or during the annual open enrollment period.

If you do not enroll for coverage within 30 days of your eligibility date, you will not receive health coverage during the plan year unless you have a qualified change in family status.

M A K I N G C H A N G E S

The choices you make when you are first eligible are in effect for the remainder of the plan year, which ends on December 31. Once you enroll for coverage, you must wait until the next open enrollment period to change your benefits or add or remove coverage for dependents, unless you have a qualified change in family status as defined by the IRS.

Here are some examples:

» Marriage, divorce, legal separation, or annulment» Birth or adoption of a child» Change in your residence or workplace (if your benefit options change)» Loss of health coverage» Change in your dependent’s eligibility status because of marriage, age, etc.

You have 60 days to make changes to your coverage.

Keep in mind, any change you make to your coverage must be consistent with the change in status.

M E D I C A L C O V E R A G E

ASNA provides you with medical coverage through the Federal Employees Health Benefits (FEHB) Program. Under section 409, “Access to Federal Insurance,” of the Indian Health Care Improvement Act (IHCIA), tribal organizations are able to purchase health coverage through the FEHB Program.

The FEHB Program provides a wide array of plans to cover you and your family’s health care needs. Take the time to review your options and choose the plan that’s best for you.

You have three enrollment options:

» Employee Only—ASNA pays for 100% of your medical premiums» Employee Plus One—ASNA contributes 75% to employee and spouse or

child medical premiums» Employee and Family—ASNA contributes 75% to employee and family

medical premiums

If you are a healthy individual and you don’t take regular prescription drugs, you may want to consider a high deductible health plan with a lower monthly pay-ment. However, if you or a family member takes regular medications and suffers from a chronic illness, you may want to consider a plan with a low annual deduct-ible and higher monthly payments.

Each year, your FEHB coverage will continue. You are not required to re-enroll if you are satisfied with your current coverage. However, the premiums may change from year to year. If you leave tribal employment, you will be offered Temporary Continuation of Coverage (TCC) for 31 days after you leave.

T Y P E S O F M E D I C A L P L A N S

The following are the types of medical plans available to you.

Fee-for-Service (FFS)

With an FFS plan, you have the flexibility to choose from both PPO and non-PPO medical providers. You pay a fee for each service you receive and the plan reimburses you or pays the medical provider directly for covered services. The amount covered by insurance depends on the provider’s participation in the plan.

Consumer Driven Health Plans (CDHP)

The CDHP plan has lower monthly premiums and you assume full or partial re-sponsibility for several thousand dollars in catastrophic coverage and expenses (for costs above a certain level). In-network preventive care is covered at no cost to you. Please note, the catastrophic limit with this plan is higher than other plans.

ASNA offers you and your eligible dependents dental coverage through Meritain to help pay dental expenses for you and your family. The plan helps you pay for most necessary dental services and supplies, including preventative care (such as exams, cleanings, and X-rays), and basic and major restorative services (such as fillings and crowns). Review the chart below for coverage details.

You receive benefits each year for eye exams, eyeglasses, and contact lenses. You’re free to choose any provider you would like—visit an in-network doctor and take advantage of higher benefits coverage.

L I F E A N D D I S A B I L I T Y C O V E R A G E

Basic Life and Accidental Death & Dismemberment (AD&D) Insurance

Life and Accidental Death and Dismemberment (AD&D) insurance can provide economic security for your loved ones. ASNA provides you with this coverage at no cost to you.

You automatically receive basic employee Life and AD&D coverage in the amount of 2.5x your salary, up to $350,000. Be sure to choose a beneficiary to receive benefits—and don’t forget to change your beneficiary when your life changes.

Voluntary Life Insurance

You have the opportunity to purchase additional Life insurance coverage for yourself at group rates. Consider costs such as funeral expenses, legal expenses, and general living expenses for your surviving family members when determin-ing an appropriate amount of additional coverage.

E M P L O Y E E : This is available in $10,000 increments, up to 3x your annual earnings to a maximum of $300,000. Evidence of insurability is required over $100,000.

If you enroll for additional coverage for yourself, you may choose to elect cover-age for your spouse and/or your child(ren) in the following amounts:

S P O U S E : This is available in $5,000 increments, up to a maximum of $100,000. Evidence of insurability is required over $30,000.

C H I L D ( R E N ) : This is available in $5,000 or $10,000 per child (between the ages of 6 months and 25 years).

Voluntary Accidental Death and Dismemberment (AD&D) Insurance

You have the opportunity to purchase additional AD&D coverage for yourself at group rates.

E M P L O Y E E : This is available in $10,000 increments, up to 10x your annual earn-ings to a maximum of $300,000 for employees age 69 or younger. Evidence of insurability is required over $100,000.

If you enroll for additional coverage for yourself, you may choose to elect cover-age for your spouse and/or your child(ren) in the following amounts:

S P O U S E : This is available in $10,000 increments, up to a maximum of $100,000 for spouses age 69 or younger. Evidence of insurability is required over $30,000.

S H O R T - T E R M D I S A B I L I T Y ( S T D )

You are automatically provided with Short-Term Disability (STD) coverage at no cost to you.

F U L L - T I M E E L I G I B L E E M P L O Y E E S : Your STD plan provides 60% of your weekly salary to a maximum of $1,500 per week for the first 15 weeks of a disability (after a 14-day waiting period).

E X E C U T I V E S , D O C T O R S , A N D A D M I N I S T R A T O R S : Your STD plan provides 60% of your weekly salary to a maximum of $2,500 per week for the first 15 weeks of a disability (after a 14-day waiting period).

L O N G - T E R M D I S A B I L I T Y ( LT D )

ASNA believes that Long-Term Disability (LTD) coverage is important because anyone at any age may become injured or ill for an extended period of time. You are automatically covered under the LTD plan at no cost to you.

F U L L - T I M E E L I G I B L E E M P L O Y E E S : Your LTD coverage will replace 60% of your base salary to a monthly maximum of $7,500 if you are disabled for more than 120 days and are unable to work.

E X E C U T I V E S , D O C T O R S , A N D A D M I N I S T R A T O R S : Your LTD coverage will re-place 60% of your base salary to a monthly maximum of $12,000 if you are dis-abled for more than 120 days and are unable to work.

LTD benefits are offset with other sources of income, such as Social Security and workers’ compensation.

R E T I R E M E N T

As an ASNA employee, you have access to retirement planning benefits through John Hancock USA. Your 401(k) account is set up as soon as you begin employ-ment.

Your 401(k)

ASNA automatically contributes the equivalent of 8% of your bi-weekly earnings into your 401(k) account, up to the federal maximum.

» 3% immediately vested; 5% per 5,000 hours vesting scheduling; 1,000 regular work hours for 20% per calendar year (January 1 – December 31)

There is a ROTH option available.

E D U C AT I O N S A V I N G S P L A N

Earnings in a 529 account are free from federal tax and, in most cases, state tax-es—provided they are used to pay for qualified education expenses like tuition.

As an ASNA employee, you’re eligible to receive Accident coverage through MetLife at no cost to you. If you experience an accident, you may have unexpect-ed expenses. The Accident insurance plan can be there for you through the many stages of care, from the initial emergency treatment or hospitalization to fol-low-up treatments or physical therapy. Group Accident insurance from MetLife helps with out-of-pocket costs that arise when you have a covered accident such as a fracture, dislocation, or laceration.

C R I T I C A L I L L N E S S I N S U R A N C E

As an ASNA employee, you’re eligible to receive Critical Illness coverage through MetLife at no cost to you. The MetLife Critical Illness insurance plan can help with the treatment costs of covered critical illnesses, such as cancer, a heart attack, or a stroke. You receive cash benefits directly (unless otherwise assigned)—giving you the flexibility to help pay bills related to treatment or to help with everyday living expenses.

H O S P I TA L I N D E M N I T Y I N S U R A N C E

As an ASNA employee, you’re eligible to receive Hospital Indemnity coverage through MetLife at no cost to you. Just a few days in the hospital for illness can be costly. Cash benefits with MetLife’s Hospital Indemnity insurance plan may of-fer some financial protection when you’re hospitalized due to a covered accident or illness. Benefits are paid for hospital stays, expenses, admission, confinement and intensive care.
