Appendix D – Entry Blanks 1. General. This appendix is intended to provide examples of entry blanks for various tournament formats. Use for format only. These entry blanks have not been checked against the sanction checklist. 2. The Foothills XII Open – Gold Dollar Upset. Page D-3

ELIGIBILITY: Open to all members of the USATT. Non-members may join the USATT by including the membership fee with their entry fee (see entry form for dues). Juniors are players born on or after Jan 1, 93. Players who have never been USATT members may buy a tournament pass for $10 (one time only).
EVENTS : Giant Round Robin - Only 48 players will be accepted.Limitation: Only 8 players under 1000 rating will be accepted.Handicap –Players must have an official USATT rating over 1000 and be entered in the Giant RR. Limit 16 playersEVENT & USATT FEES: Round Robin $50 includes the USATT $7 processing fee; Handicap $5; DEADLINE: Saturday October 30. Entry fees must accompany entry form. $5 discount for AGTTA club members. $5 charge for each entry not paid in advance.

FORMAT: Giant RR - Play will be in three stages. Preliminary: 8 groups of 6 players. Stage 2: Based on playing records in the Preliminary groups, players will be placed into Championship, Class “A”, “B”, “C” , “D”, or “E” round robin events. Each event will have 2 groups of 4 players. Stage 3 is single elimination of group winners & runners-up.
Handicap – Match is one 31 point game; single elimination. To be played between Preliminary and Event stages.
PROCEDURES: The highest rated player in each preliminary group will be responsible for recording match results and keeping the group on schedule.
Matches: All games (except Handicap) will be to 11 points; Championship event best of 7 from semi-finals; all other matches best of 5 games

RULES: The Laws of Table Tennis as published by the ITTF and all USATT regulations will be enforced.
EQUIPMENT: Only USATT approved equipment will be used: 10 Donic and 4 Stiga Optimum 30 tables; Donic and Stiga nets; white Butterfly !!! 40mm balls; Wooden floor. NOTE: RACKETS: must be USATT approved; one side of the racket must be bright red, the other side black
CLOTHING: The USATT Dress Code applies. A wide variety of colors and designs is now permitted. The main color of the body of the shirt or shorts may not be white. Please dress in appropriate athletic attire. Designs or lettering that are distracting to an opponent or are offensive will not be permitted. Socks and rubber-soled, non-marking, shoes must be worn.
REPORTING AND STARTING TIMES: Players must register at the desk not later than 8:45 a.m. Play will begin promptly at 9 a.m. Players not on time may be defaulted for the entire event (and replaced by an alternate) at the discretion of the referee.
AWARDS: Championship: 1st $300; 2nd $150; 3 rd & 4 th $75 each. New: The winner of one other event, selected at random, will receive $50 in addition to a trophy.
Handicap 1st $30. Trophies to the winner and runner-up in other events.
PRACTICE: The gym will be open for practice at 8:30 a.m. Saturday.
UMPIRES: Umpires will be provided on request.
UMPIRE EXAM: Umpire exams will be available @ $15 each. We will grade the exam and evaluate umpiring.
CANCELLATION: The committee reserves the right to cancel the tournament or modify the format based on the number of entries.
31 Point Handicap EventBetween preliminary and event rounds
Limited to players over 1000 rating
Racket Control. 1. No complete re-gluing will be permitted unless it is with a new sheet of rubber.
2. Partial re-gluing may be done with the permission and under the supervision of the referee.

2012 Alabama Closed Team Championships 2-star DATE: Saturday, March 31, 2012
SITE: Anniston Army Depot 256-235-6385 ELIGIBILITY: Open to residents of Alabama, USATT membership required (available on this form) for team play. A one-time tournament pass is available for non-members who have never played in a USATT or USTTA tournament. Military personnel and college students in Alabama are eligible. COMPETITION FORMAT: Groups of four teams (2-3 players) in round robin with all teams advancing to single-elimination playoffs. Field will be Limited to 20 Teams; the top 12 rated teams will play in the upper tier; bottom 8 in the lower tier. Upper tier advances the top 2 teams from each RR group to championship. The remaining teams will advance to Class A. Lower tier advances top 2 teams from each RR group to Class B. The remaining teams will advance to Class C. The championship winning team is declared the
2012 Alabama Team Champion. TEAM FORMAT: Matches will be played in the following order:

The captain need not name the doubles pair until after the 2nd singles match SCHEDULE: Team play starts at 9 a.m. Team captains must register no later than 8:30 a.m. Players/Teams not on time may be defaulted. The playing site will be open for practice 6-9 p.m. Friday, March 30 and at 8 a.m. on Saturday.
There will be lunch on site the day of the tournament for all players. All players will receive a free t -shirt and a chance to win a door prize. EVENTS: All teams ($80 per Team) AWARDS: Upper tier team champion (based on 12 teams in division): $450. 2nd place: $225. Class A: 1st: place Plaques & 2nd place Plaques. Lower tier team champion (based on 8 teams in Class B). $300. 2nd place: $150. Class C: 1st place Plaques & 2nd place Plaques. NOTE: The directors reserve the right to modify any event (due to insufficient entries, etc.). EQUIPMENT: Only USATT approved equipment will be used. Butterfly Centerfold tables, white Butterfly (40 mm) compliments of BumperNets. RACKETS: USATT approved red and black. CLOTHING: The USATT Dress Code applies.
