There has been significant press in the last

couple of years about the deficiency of Vitamin

D plaguing our society, and some scholars

claim we have reached epidemic levels of

deficiency.

Unfortunately, many American medical

organizations negate the seriousness

of Vitamin D deficiency and are

equally opposed to Vitamin D as

an enhancement to the treatment

of chronic disease, including heart

disease and cancer, regardless of the

present literature available.

This opposition by conventional

medicine is nothing new. History has

shown that it takes about 25 years

for a clinically efficacious nutritional

treatment to catch on in medicine.

This was evident in the case of the

utilization of folic acid during pregnancy.

In 1975 there was ample clinical evidence

that women supplementing with folic acid

during pregnancy could prevent certain birth

defects in their children(1). During the following

two decades it took medicine to accept folic acid

during pregnancy as public policy, thousands upon

thousands of children were born with severe birth

defects that could have been prevented.

On the other hand, it is interesting that the FDA will

approve a synthetic chemical as a safe medication in less

than a decade, while nutrient acceptance can take 2.5 times

as long.

I hope to provide enough clarity, research and interest

in Vitamin D in this short article to encourage you to

take action before public policy catches up with the

overwhelming clinical evidence.

low Vitamin D levels suffered 2.42 times more

heart attacks(2) than those with adequate levels.

To show you the magnitude of financial liability

that results from maintaining a Vitamin D

deficient society, we will extrapolate average

2008 study noted above. Improving Vitamin

D status would reduce health care costs by

approximately $89.4 billion in regards to

heart disease alone. This deficiency clearly has

a profound effect on our society in regards

to overall health and economic strain. This

does not include the significant effects

Over 5,000 research studies have been conducted concerning

Vitamin D, including its effect on seventeen different cancers.

Research has correlated Vitamin D

deficiencies to numerous health

conditions including:

The research being done on the

conditions noted above and their

excerpts of research should provide you

• Diabetes occurs more commonly in Vitamin D deficient

individuals. Vitamin D has been shown to stabilize blood

sugar when at optimal levels (4,5).

• Vitamin D has been shown to significantly lower systolic

blood pressure due to its effects on the blood pressure-

related hormone rennin (6).

• Cancer outcomes are improved with proper Vitamin D levels

through the following four mechanisms(7):

1. Promotes cell differentiation

2. Regulates programmed cell death (Apoptosis)

3. Limits metastatic spread of cancer

4. Limits growth of tumor blood supplies

are yOu DeFICIeNT?

There are numerous ways to establish deficiency states of Vitamin

D. The most common and accurate method is to do a blood test

for 25(OH)D. Clinical radiological findings can also be very useful

because radiological changes after supplementation in Vitamin D

can be seen as early as one week after supplementation(8) .

The lower acceptable limits of 25(OH)D levels vary within the

20-37 nml/L range and optimal levels range from 60-75 nml/L. It

is important to retest after 90 days of supplementation to ensure

proper dosing.

According to the Vitamin D Council, it is recommended that

adults take a minimum of 5,000 IU per day with retesting done in

90 days (9).

According to Dr. Carnell at the Council on Vitamin D,

Infants and children under the age of one should obtain a

total of 1,000 IU (25 mcg) per day from their formula, sun

exposure, or supplements. Children over the age of 1 year

should take 1,000 IU per every 25 pounds of body weight

per day, depending on latitude of residence, skin pigmentation,

and sun exposure (10).

FaCING reaLITy

Cutting edge research on Vitamin D has recently helped over turn

guilty verdicts in child abuse cases where parents were convicted

of physically abusing a child due to the presence

of multiple fractures. In these particular cases, the

fractures were not caused from parental abuse but

the child’s deficiency of Vitamin D, creating brittle

bones more prone to fracture. The profound reality

of those court cases highlights the extent of medical

ignorance regarding Vitamin D deficiency.

I would like to leave you with a very important clinical

consideration on the topic of Vitamin D: the growing

number of Vitamin D deficient children. In addition to

taking note of the previous dose recommendations

for children from Dr. Carnell, it is important to be

aware of the cascade which sets the stage for early

developmental deficiencies in Vitamin D.

First and foremost, mom is usually covered up

from the sun because showing her skin is the last

thing she wants to do after gaining weight during

pregnancy. Therefore, she does not get the natural

dose of Vitamin D from the

sun to pass on to her

growing baby.

Although 92% of

pregnant women take

prenatal vitamins,

a shocking 40% of

white women and an

overwhelming 54%

of black women have

insufficient levels of Vitamin D during

pregnancy(11).

When the child is born he/she is also “protected” from receiving sun

exposure during the

first couple of years

of life further robbing them of Vitamin D. When there is chance of exposure, the child is usually dipped in

Case Report:Effectiveness of Human Chorionic

Gonadotropin (HCG) and Hormone

Balancing in Weight Reduction

Case Description:BackgroundThe patient is a 28-

year old female with a

relatively unremarkable

medical history. She was

diagnosed with asthma

and seasonal allergies

in early childhood and

a benign adenoma of

the anterior pituitary

in later adolescence;

however, the patient

does not suffer from

any serious, ongoing

medical conditions.

She experienced significant weight

gain of 40 pounds

and extreme difficulty in weight

reduction after having three children and, later,

discontinuing oral

contraceptives after nearly 15 years of use. In February of 2010, the patient presented with complaints of chronic fatigue and malaise,

widespread musculoskeletal pain, hair loss, abnormal

weight gain, nausea with severe headaches related

to monthly cycles, and frequent exacerbation of

asthmatic symptoms. Further investigation and workup

revealed significant hyperestrogenism, hypoadrenalism,

hypothyroidism, and evidence of systemic inflammation.

Medical InterventionIn early March, 2010, the patient was placed on

a medication regimen consisting of bioidentical

thyroid hormone (specifically, T3), adrenal support,

transdermal progesterone and testosterone therapy,

oral pregnenolone, and DHEA supplementation.

Approximately 6 weeks later, she was recommended

for a 40-day trial of injectable human chorionic

gonadotropin (HCG) coupled with a very low calorie

diet (VLCD) for the purpose of weight loss. Regular

office visits were maintained throughout the course

of treatment, and frequent laboratory workups were

performed to monitor thyroid hormone levels, serum

progesterone and testosterone, and C-reactive protein

(CRP).

OutcomeDuring the course of treatment, the patient

experienced a total weight loss of 50 pounds, and

her body fat percentage had been reduced by

over 50%. Additionally, many of her presenting

symptoms albeit disappeared; the malaise,

widespread musculoskeletal pain, nausea,

and asthmatic problems began to subside

approximately 8 weeks into treatment. Toward

the end of the program, the patient also

reported a significant reduction in chronic

fatigue and previously-noticed hair loss and a

marked increase in energy and activity levels

and overall quality of life. No negative side

effects were reported during treatment.

In terms of objective measurements, the

patient’s thyroid levels, specifically T3,

began to normalize, progesterone and

testosterone began to move toward

optimal levels, and CRP decreased

significantly – from 7.1 to 1.7 in a matter

of only 8 weeks.

Case Discussion:Trends During TreatmentAs described in much of the current HCG literature, the patient noticed a significant loss in weight each day during the treatment (varying from 0.5 pounds to 2 pounds per day). She also reported a marked increase in energy levels, motivation for physical activity, and quality of sleep. Because the patient was in need of additional medical intervention for the aforementioned conditions, it is important to note some interesting trends reported during the 40-day course of injectable HCG.

• When thyroid levels began to fall below optimum, weight loss slowed significantly. Additionally, the patient noticed discernible fatigue and cravings for sugars and starches. • As the menstrual cycle approached or

progesterone/estrogen levels were imbalanced, weight gain stalled (sometimes reversing) for several days, and obvious fatigue, sugar cravings, and edema were present.

• Sleep disturbances and increased stress seemed to have a direct correlation with sugar cravings, severe hunger, fatigue, and a decrease in weight loss.

• Vigorous exercise (i.e., running, speed walking, weight training) decreased successful weight loss throughout the program; whereas, light exercise (i.e., yoga, walking at a comfortable pace) several times per week improved success.

One of our menopausal patients, Francis, was in tears

as she shared this embarrassing secret. We reassured her

that she wasn’t alone in having these thoughts —nearly 50

percent of menopausal women privately confess to similar

feelings. Yet, this decrease in sex drive is rarely brought out

in the open to be discussed or treated.

Where Did My Sex Drive Go?

The average age for menopause is 51. Often, women

will first notice an irregularity to the menstrual cycle—

skipping periods, lighter flow. During this stage, called

perimenopause, the production of hormones—estrogen,

progesterone and testosterone—by the ovary declines

significantly. In fact, testosterone production actually starts

to decline after age 30.

Women in this phase of life

can experience hot flashes,

night sweats, sleep disturbances,

vaginal thinning, depression

and decreased libido. It is during

this erratic and unpredictable

transition from normal hormone

levels to low hormonal levels

that the symptoms are the most pronounced. The severity

of symptoms varies from woman to woman, but nearly 80

percent will experience them to some degree.

Stress Factors

Adding to the complexity of this issue is that the time

of the menopause often coincides with several other major

transitions in a woman’s life. Her children move away from

home, she experiences changes in career, or she takes on the

role of caring for aging parents. Stress levels go through the

roof, also resulting in a lack of interest or desire. Metabolism

slows 10 to 15 percent at this time of life as well, and the

resulting weight gain and lack of energy can make sex the last thing on

a woman’s mind, compounding the decrease in libido. The symptoms of

menopause itself—insomnia, hot flashes and depression—also contribute to

libido decline. Vaginal thinning can lead to painful intercourse, which then

causes tension and fear, and subsequently less desire.

Isn’t It Normal to Lose Interest?

While there is a clearly defined physical response to arousal, there

is also a large psychological component. Many of my patients assume it

is “normal” to lose interest in sex as they go through menopause and,

therefore, do not seek out help in this area. They attribute the lack of libido

to strain within the relationship or familiarity with their partner—being

bored. They feel like they can take it or leave it, and unless their partner is

pushing them, they may never broach the subject with anyone. We tell them

it is common, but it doesn’t have to be mandatory.

Learn to Live with It?

Most of our menopausal patients have learned

to live with it. They essentially have given up on that

aspect of their lives, and their relationships take on new

dynamics. But, when asked, most of our patients say they

would love to have fulfilling, fun, passionate sex lives with

their partners. So, often in desperation, they ask what can

be done.

Remember, there is no quick fix for loss of sex drive, but there are things

that can help. First and foremost is speaking honestly with your partner

about your feelings. Next, talk to your doctor.

• Do Something Nice for Yourself

Getting into the right mind-set can do

wonders. This may mean exercising,

getting a good night’s sleep and making

time for intimacy. Try clearing a special space

for intimacy away from the stresses

of everyday life.

• Review Your Health History

and MedicationsSome of the most commonly prescribed

medications in the United States have

decreased sex drive as a side effect. These

include common antidepressants like

Prozac and Zoloft, as well as blood pressure

medications such as beta-blockers. In

addition, corticosteroids and oral estrogen

can decrease the levels of testosterone in

the blood. If you are taking any of these

medications, talk to your doctor about

alternatives.

• Consider TestosteroneThere have been more than 20 prospective

studies that have shown that testosterone

improves sex drive. Interestingly, though,

the FDA has not approved any medications

for this use in women. For this reason,

patients must get the testosterone from

compounding pharmacies or use medications

designed for men in lower doses. These are

available in pill, patch and cream forms. The

main risks associated with testosterone use in

women are increased hair growth and acne.

However, these risks are quite small and are

reversible.

• Vaginal EstrogenVaginal dryness and subsequent painful

intercourse can be treated with vaginal

estrogen preparations. These can return the

vaginal tissue to its premenopausal state,

which is more elastic and less dry. They come

in the form of creams or inserted tablets.

It usually takes about six weeks before

maximum benefit is obtained.

• Couples TherapyOpen dialogue with your partner, often

guided by a therapist, can bring to

the surface other issues that might be

contributing to the problem, including

stress within the relationship, self-image

problems and guilt. A trained sex therapist

can help with relaxation and communication

techniques as well.

Morgentaler noted. Blood levels are an important part of the diagnostic process too—but it isn’t entirely black and white. “While we currently don’t recommend that every man undergo regular testosterone checks as a screening test,” Morgentaler noted, “We do need to get primary care doctors thinking about the diagnosis in an increasingly broad way. Men who complain of fatigue, poor focus, and low energy levels, in addition to those with purely sexual complaints, should have their T levels checked. Not all will have low T, of course, but we know that we’re missing a very large number of people at present.”

As with any other blood test, it’s important to know what the “normal” levels are, and how they apply to an individual. “According to the FDA, “a ‘normal’ total T level is 300 ng/dL,” says Morgentaler. “Strictly interpreted that would mean that a man with a level of 299 is low, while a man with a level of 301 is normal. That’s simply not a sensible approach. The Endocrine Society, a very august group, has produced guidelines that mirror the FDA, though they do point out that no single number should be used and that symptoms should guide the diagnosis.16 The International Society for the Study of the Aging Male17 uses a higher number, 348. The point is that we need to interpret these numbers in the context of a man’s signs and symptoms.” In other words, if a man’s symptoms are consistent with low T and his level is anywhere near the low end of these scales, he and his physician should consider a trial of T replacement therapy.

According to Dr. Morgentaler, “Which tests are done, where they are done, and how they are reported also matter. Most primary care physicians use local laboratories, which can be inaccurate and arbitrary in their ‘normal’ reporting range. We pulled 25 hospital laboratories’ reference ranges at random, and found 17 different low and high ‘normal’ values.” Worse, none of these values were based on symptoms—they were based simply on standard deviation from average levels in the population. “In a population with 10% of men with truly low T levels, this approach means you’d miss three-quarters of them with standard testing,” Morgentaler points out. This can have significant impact when a primary care physician, appropriately concerned about the possibility of low T, gets back a “normal” result and falsely concludes that the problem lies elsewhere.

“A more rational approach, and one we’ve taken with other blood tests such as cholesterol, glucose, and PSA, is to establish healthy target levels based on the literature,” continued Morgentaler. “That way we have an objective measure that actually says something about a person’s risk for a particular outcome.”

Physicians may also need some reminding about which test to order for testosterone. Like other sex hormones, testosterone is found in the blood largely bound to a protein called sex hormonebinding globulin, or SHBG.10 But it’s the “free” testosterone that actually affects risk and performance, and standard testing measures only total T (free plus bound). “Physicians need to order both total and free T levels when they do their testing,” says Morgentaler. “If either one is low or in the ‘low normal’ range in a man with symptoms consistent with low T, then treatment should be considered.” Do doctors need to send this test out to specialized labs? “Not at all,” responded Dr. Morgentaler. “Most labs can do both tests—physicians just have to know to order them.”

Fortunately, once the diagnosis has been made on the basis of concerning symptoms and low or low-normal T levels, treatment is fairly straightforward. “We have many options for treatment of men with low T,” Dr. Morgentaler observed. “There’s no recommended pill available in the US—there are some old ones still around that are not very effective
