It will take approximately four weeks, or possibly longer, to receive the record, documenting the LAPD officer's visit to the Homeowner's residence. Thus, this record may not be available for the Tuesday, January 10, 2017 LADBS/City Council Hearing. If the record does become available, by January 10,2017, it will be forwarded to the LADBS. Please refer to the Los Angeles Police Department Discovery Section Correspondence Attachment, dated December 6, 2016.     In order to become aware of mail being stolen from the Homeowner's mailbox, Mr. Alp Aslan, the sen of the Homeowner, who has moved into the Homeowner's residence sc as to assist his father in dealing with his progressive symptoms of Dementia (see numbered Paragraph 2), has enrolled in an identity theft alert monitoring service thru Identity Guard. Please refer to the Identity Guard Letter Attachment, dated December 6, 2016.

    2 The Order to Comply Notice, and any other Notices, were addressed to the Homeowner, Mr. Muzaffer Aslan, who has experienced progressive symptoms of Dementia many years prior to his documented history of this neurodegenerative disease in early 2010 Thus, the Homeowner's ability to comply with the payment of a Noncompliance Fee, as stated in any Notices, was compromised due to his severe memory impairment, caused by his Dementia Please refer to the UCLA Health Dementia Diagnosis/History Letter Attachment, dated November 28, 2016, from the Homeowner's primary care physician.

    3. Compliance, to resolve the violation of the Los Angeles Municipal Code, was obtained on May 6, 2010, approximately five weeks after the March 28, 2010 Compliance Date deadline, as written by LADBS Inspector Jack Matillo on the Order to Comply stamped May 11, 2010. Mr. Matillo wrote "Compliance Obtained. Case Closed", and placed his signature, along with the date of May 6, 2010. The written statement by Mr. Matillo stating "Case Closed" -ndicates that there is no outstanding issue to be resolved with the LADBS. Thus, the asking for the payment of a Noncompliance Fee oy the LADBS is not correct and is a lien processing error. Please refer to the City of Los Angeles Order to Comply Attachment, stamped March 11, 2010, and signed and dated by Inspector Jack Matillo.

    4. Since compliance was not obtained until five weeks after the March 28, 2010 Compliance Date deadline, the Homeowner and his son, Mr* Alp Aslan, are willing to pay the current Noncompliance Fee, without having to pay any additional charges, including those charges related to late charges, collection fees and interest.

    It would be greatly appreciated if this case could be resolved before the January 10, 2017 City Council Hearing as the Homeowner's attendance at this meeting, in order to object the confirmation of the proposed lien, would be a burden to the Homeowner, aged 93, due to nis advanced Dementia and to his limited mobility, as he has difficulty walking and must use a cane.

    Dear Sir/Madam:

    I was a victim of identity/credit card theft in approximately August/September 2015-

    Someone used my identity to apply for and receive a new credit card.The new credit card was shipped to my mailing address, and was then stolen from my mailbox when the credit card arrived to my residence.

    I did not file a police report, however, I called the LAPD, and two police officers came to my residence. I do not remember the exact date that the officers came to my home, however, it was sometime in August or September 2015-

    The address that the LAPD officers came to is 1356 Stradella Road, Los Angeles, CA 90077.

    My full name is Mr. Alp Aslan.

    I need a copy of the record showing that the LAPD officers came to my residence.

    When I called the LAPD Call H Service/Discovery Unit, I was told that it may take four weeks to receive the record of the LAPD officers visit to ray residence. However, I need this record with one to two weeks, and no later than Tuesday, December 27,2016.

    I am the primary care physician and geriatrician for Mr. Aslan Muzaffer (DOB: 2/28/1923). I first met him in 2014 and diagnosed him with dementia which is a progressive neurodegenerative disease.     If an appeal or request for slight modification is not filed within 15 days of the compliance date or extensions granted therefrom, the determination of the department to impose and collect a non-compliance fee shall be final. Section 98.0411 L.AMC.
