AIMS OF THE SURVEY
• Analizzare la tempistica diagnostico/terapeutica dei pazienti riferiti all’Emodinamica dell’Ospedale di Udine (HUB) per PCIp
• Confrontare i tempi di intervento tra area vasta (Centri SPOKE) e realtà urbana e suburbana (HUB) ed individuare criticità migliorabili
• Individuare eventuali indicatori utili al confronto futuro in un ottica di monitoraggio continuo del percorso diagnostico/terapeutico intrapreso
Valutazione prospettica dei pazienti con STEMI inviati all’emodinamica
di Udine per coronarografia finalizzata ad eventuale PCI in un periodo temporale compreso tra tra il 1.01.2007 ed il 31.12.2007
Background
L’angioplastica primaria (PCIp) rappresenta oggi lo standard di trattamento dell’infarto
acuto del miocardio con sopraslivellamento del tratto ST (STEMI)
…purchè venga eseguita entro 90 minuti dal primo contatto medico con il paziente
(door to balloon time <90 minuti)

ECG teletransmission from the Ambulance to the
nearest CCU is an ongoing project, applied in 2007
in a small subset of patients (25/180 , 13,8%) is
increasing in 2008 (14/41, 34%)
CONCLUSIONS
• I tempi di intervento globali non si discostano globalmente da quelli dei grossi trials e Registri
• La risposta del sistema 118 è del tutto adeguata per quanto riguarda la tempistica di intervento mentre è migliorabile il sistema della trasmissione dati per via telemetrica e la centralizzazione del paziente
Le criticità riscontrate nella nostra rete sono per lo più legate a modelli organizzativi - in parte superati - che si rendono più evidenti nei passaggi nei quali si interfacciano le diverse competenze (trasporto, passaggio tra il PS del centro Hub ed il cardiologo)
• Il miglioramento dei tempi di intervento potrebbe allora essere ottenuto
– Incrementando la teletrasmissione degli ECG
– Riducendo i tempi di permanenza dei pazienti nei centri spoke per quelli che arrivano con mezzi propri
– Concordando un protocollo FAST track di gestione che preveda la centralizzazione diretta del paziente in una visione assimilabile a quella in uso per il Trauma.
– Attivazione diretta dell’Emodinamica da parte del medico dell’Area Di Emergenza (ADE)
