2018 Front-Line Staff Outstanding Performance Award Winners

Karl Anderson

Alaska Native Medical CenterKarl Andersen is recognized for his diligence to patient safety during his 18 years with the Alaska Native Tribal Health Consortium. He currently serves as a Lead Security Offi cer on the evening shift and as a Crisis Prevention Intervention trainer. Karl has shown leadership in training staff on patient safety tools including Heartsaver and Basic Life support. He has led many fi rst responder teams reducing the time to respond to emergencies. He has championed the use and availability of Automated External Defi brillators (AEDs) and Ambu bags throughout the campus. Karl is a champion for patients and is dedicated to ensuring their safety.

2 South Nursing Team

Foundation Health Partners

The 2 South Nursing Team at Fairbanks Memorial Hospital has demonstrated excellence in process improvement work over the past two years to reduce patient falls. The team worked closely with the campus fall team and the quality department utilizing the Institute for Healthcare Improvement model for improvement. They implemented new daily practices and team rewards for achieving goals. The team is learning how to incorporate storytelling into their process improvement activities to bring about eff ective and lasting change and is working to spread the new approaches to other acute and long-term care units in the hospital.

Under the leadership of Mandy Mitchell, Manager of Medical Oncology Services, Alaska Regional Hospital developed an innovative program to serve patients with medical disorders related to their addictions, improving outcomes for patients while supporting staff in creative ways to prevent burn-out. Master’s level clinicians in the psychology residency program provide individualized emotional support for both patients and staff while receiving valuable hands-on experience. Solid clinical care combined with a supportive, non-judgmental environment supports patients on the road to recovery.

The Remote Camera Project was developed to improve basic radiology services in remote clinics served by Southeast Alaska Regional Health Consortium. Beginning in 2015, Dr. Loren Laybourn (Chief of Radiology at SEARHC) and Eva Marie Popelka (radiology technician) explored ways to address the challenge. Mounting a tiny camera on the x-ray tube itself along with multiple remote cameras and a hands-free phone headset allows real-time monitoring of patient positioning, x-ray tube alignment, and cassette placement. Providing better communication and support to staff in rural locations has measurably improved diagnostic quality, decreased unnecessary radiation exposure and allowed for improvements to the overall process of taking x-rays.

Quyanna Care Center staff are dedicated to improving the quality of life and safety for residents. In recent months this dedication has been recognized through the CMS star ranking; they achieved a 5-star overall rating. Staff has worked to blend traditional culture and modern innovation by utilizing assistive devices such as Pocket Talkers and iPads. Residents “come to life” when hearing familiar voices, music, and movies clearly, often for the fi rst time in years. Quyanna Care Center is committed to working with “Nome-grown” certifi ed nursing assistants to stabilize staffi ng and provide continuity for the elders in their care.

The nurse leadership team at Prestige Care has undergone a transformational change that allows for more dynamic and rapid problem resolution. This shift in leadership focus has resulted in measurably improved outcomes in resident safety, infection rates, and compliance. Communication between leaders led to increased regulatory readiness, greater collaboration, and overall patient safety. The result has been increased patient safety and continuity of care, and greater collaboration across the care team.

Persevering in the Pursuit of Zero Patient Harm Award recognizes a facility that has shown exemplary initiative and commitment to the development of comprehensive systems and processes that lead to a better, safer

and more reliable health care system. These changes involve the leadership’s commitment to achieving zero patient harm, a functional culture of safety throughout the organization, and the widespread deployment of highly eff ective process improvement tools. Facilities must demonstrate that changes are “hard-wired” into the organization, displaying a culture of safety across all disciplines that values the input of all employees no matter their title or role.

2018 High Reliability Award Winner

Providence Valdez Medical Center

In 2016 Providence Valdez Medical Center embarked on the journey to become a high-reliability organization. The objective was to train all staff to become obsessed with safety and to utilize tools to empower staff to speak up for safety. The administrative team became the champions of the safety movement; Caring Reliably. The reporting of all events or near miss events was identifi ed to be the number one objective indicator of how well they were developing their sensitivities to safety concerns. Providence Valdez Medical Center has now gone more than 725 days since the last serious safety event.

2018 Oustanding Patient Safety Winner

Utilizing the IHI model for rapid cycles of change (plan, do, study, act), Providence Alaska Medical Center has dramatically reduced the number and rate of people dying from sepsis. A team led by physician and nurse champions analyzed their approach to sepsis from all angles; recognizing presenting symptoms, hardwiring treatment bundles, improving education for patient care technicians, nurses and physicians, early risk recognition and intervention, implementing nurse-initiated orders, reinstating ED sepsis alerts, and fi nally developing a Sepsis RN program. This award recognized a team eff ort that has resulted in sustained life-saving interventions.

The 2018 Quality and Patient Safety Awards are intended to encourage, recognize, and reward those qualities.
