• Eliminate Homelessness

✓ Residential Multi-Service Center (RMSC) Housing for Reentry Population

✓ ACPD Dedicated Transitional Housing

✓ Assist families experiencing housing instability by utilizing the Breaking Barriers program to provide supportive relief services

• Healthcare for All

✓ Establish a clinical treatment model within the Department’s CORE (TDRC) to provide mental health services to clients.

✓ Ensure every youth detained at Juvenile Hall and their families have access to healthcare services

✓ At Camp Wilmont Sweeney ensure all youth have health insurance, are connected to clinics for healthcare, mental health and dental services in their own community

✓ Develop specialized mental health caseloads

✓ Include clinicians in multidisciplinary team meetings for all moderate and high-risk clients

✓ Reduce rates of recidivism and increase family engagement

✓ Create linkages to supportive services following release beginning on the first day of detention and continuing through transition, reintegration, and aftercare in the community

✓ Establish Caseload Management Standards for Adult Field Services

✓ Collaborated with Superior Court and criminal justice partners to implement a comprehensive Pretrial Pilot Program

✓ Assess a youth’s risk and needs utilizing a multi-disciplinary approach

✓ Develop a case plan that focuses on education and outlines the resources that will be provided to the youth from a strength-based perspective

✓ Participation in the Collaborative Court process, with individualized mental health treatment services, educational and vocational opportunity and other community support

✓ Ensure the continuity of care by identifying appropriate individualized, and continuous wraparound services prior to discharge

✓ Host six-week Summer Enrichment Academy for youth in Probation Supervision

✓ Provide a service delivery model that offers multiple opportunities to enhance the efficacy of the delivery of programs and services

to nutritious foods and reduce the amount of food going into landfills

through the Probation Youth Employment Program
