Leak in crossover flange• HiPo for injury if the high-pressure flange had popped off and

hit personnel within the limited high pressure area

Diesel leak• 800 ltr spilled out of the reservoir tank and into the engine

enclosure, secondary containment; 40-80 ltr ended up on deck, but

no spill to drain or sea

Job stopped due to diesel leak.

• Job schedule changed several times:- Initially planned equipment was assigned to other operations and was not available

• Once job was called off, insufficient time was given to- Sub-contractor to prepare rented equipment

• Misconceived sense of urgency in communication from Asset/ResDev
