Excia® specifically addresses three aspects of amodern hip stem.
The prosthesis design, implantation technique, and implant surfaceform the foundation for success in primary hip replacement surgery.
Excia® is implanted using a newly designed rasp that works forboth the cementless stem, which is coated with a calciumphosphate layer, and the cemented stem with a distal centralizer.
Another distinguishing feature of Excia® is the narrow 8/10 mmtrunnion, which increases the range of motion by up to 15% greaterthan the standard 12/14 mm trunnion.
The lateral offset of the Excia® stem increases with stem size, further enhancing the stability of the joint.
Excia® was also developed for computer navigation and supportsless invasive surgical techniques, thus making it well-equipped forthe future of hip replacement surgery.

Design
The Excia® straight stem is designed for implant longevitywith or without bone cement.
Cementless designStem design with distal fitand proximal flanges for mechanical stability. Proximalfixation with the Plasmapore®
µ-CaP calcium phosphatesurface.
Cemented designWingless stem design preservesbone near the trochanter.Flanges ensure a good proximalfit within the cement mantle.
Central stem alignment withthe distal centralizer.
The narrow 8/10 mm trunnionallows a wide range ofmotion: up to 140° (32 mmheads) compared to 130° witha standard 12/14 mmtrunnion.

Lateral wing
Cementless and cementedExcia® stems differ by material, surface, andespecially by the lateralwing.
The stem rasps are so precisely machined that the trialreduction mimics the final implant perfectly.
Rasps
The wing rasp is used in the final step of the cementless implantation only.

Opening the medullary canal
The medullary canal is openedwith a box osteotome, whichis inserted posterolaterallyand determines the implantantetorsion angle.

Excia® cemented
For a cemented implantationthe Excia® stem andcentralizer sizes are selectedaccording to the table below. The distal centralizer sizealways corresponds to thesize of the last rasp used.
Rasps
The medullary canal isprepared with increasingsized rasps until the desireddepth and stability areachieved.

Rasp
With the appropriate trial heads, the rasp is also usedfor trial reduction and jointinspection.

The required thickness of the cement mantle can beadjusted from 1 to 2 mm, depending on the size of the final implanted stem.

Excia® cementless
For cementless implantation,a groove for Excia®´s lateralwing is incised with the wingrasp, which is guided down aslot in the final stem rasp.
The Excia® stem can be implanted after the trial reduction. The size of the cementlessExcia® stem corresponds directly to the final stem raspfor a proper press-fit.
The stem impactor controlsthe rotational alignment during implantation.
4 5Biomechanical concept
The primary stability of theExcia® implant is achieved bya precise fit of the distal stemand good rotational stabilityin the proximal portion.
Secondary stability isachieved by a firm bonyingrowth into the proximalPlasmapore® µ-CaP coating.
