• Meals Data: • 260,000 home delivered or to-go meals were served in the month of

January. The demand for home delivered or to-go meals remains high.

• The Great Plates 2.0 program ended on January 31st. Our staff conducted outreach to those participants to determine eligibility and enroll as many as possible to the full Great Plates Delivered program. A flyer with other food resources was also shared with all 2.0 participants. The full Great Plates Delivered (1.0) program will continue at least through February.

• COVID-19 Vaccine Update by K. Smith: • Formal and informal caregivers, even under 65 years of age, are now

eligible to receive the vaccine. Documentation is required. • Seniors aged 75 and older with no internet access can schedule a

vaccine appointment by dialing 2-1-1. • The County website launched a new interactive map showing all

County-partnered vaccination sites. More information can be found at www.vaccinationsuperstationsd.com

• Currently working with 2-1-1 and mobile doctor providers for access to homebound and semi-homebound seniors.

c. Legislative Update: • On January 26th, the Board of Supervisors adopted a revised 2021 Legislative

Program for the County of San Diego which includes abbreviated guiding principles and key legislative initiatives and priorities. This revised version grants more flexibility to influence the very targeted areas we most want to impact.

• Several of the identified priority areas align with our Aging Roadmap, such as transportation, affordable housing, homelessness, and ensuring safe communities.

THE ALZHEIMER’S PROJECT 2021 ANNUAL REPORT (DISTRICTS: ALL) ..Body

OVERVIEW Approximately 100,000 San Diego County residents, along with their families and caregivers, are affected by Alzheimer’s disease and related dementias (ADRD). The burden of ADRD, for which there is no known cure, is immense for these families, and it is taking a large and growing economic toll on our region. It is estimated that 84,405 San Diegans age 55 and older were living with ADRD in 2015. The estimate grew to 98,610 in 2020 and is expected to reach 115,000 by 2030, which would represent a 36.2% increase since 2015. While ADRD is the sixth-leading cause of death in the United States, it is the third-leading cause of death in San Diego County. On May 6, 2014 (5), the Board of Supervisors (Board) launched The Alzheimer’s Project, an unprecedented initiative to develop a regional roadmap to address this devastating disease. The Cure, Care, Clinical, and Public Awareness and Education roundtables were formed and charged with developing a countywide plan to address the epidemic. “The Alzheimer’s Project: A CALL TO ARMS” was presented at a Board conference on December 2, 2014 (1). The Board unanimously adopted the report’s recommendations, and staff worked with the roundtable groups to craft an implementation plan. On March 17, 2015 (3), the Board approved the implementation plan and directed the Chief Administrative Officer to provide an annual update for the Board. The first annual update on The Alzheimer’s Project was presented to the Board on March 1, 2016 (4), along with The Alzheimer’s Project 2016 Annual Report. The second annual update was presented on March 21, 2017 (1), with The Alzheimer’s Project 2017 Annual Report. The third annual update was presented on April 24, 2018 (2), with The Alzheimer’s Project 2018 Annual Report. The fourth annual update was presented on April 9, 2019 (4), with The Alzheimer’s Project 2019 Annual Report. Due to COVID-19, the fifth annual update was submitted on September 11, 2020 to the Board via a memo, with The Alzheimer’s Project 2020 Annual Report. Today’s action supports the County’s Live Well San Diego vision by continuing services that meet the needs of persons with ADRD and their caregivers. The Alzheimer’s Project 2021 Annual Report describes these activities to support a region that is building better health, living safely, and thriving.

ADVISORY BOARD STATEMENT The Aging & Independence Services Advisory Council reviewed this item at their regular meeting on [date] and recommended approval. BACKGROUND Alzheimer’s disease and related dementias (ADRD) is taking a growing toll on families and caregivers, both personally and financially. It is estimated that 84,405 San Diegans 55 and older were living with ADRD in 2015. The estimate grew to 98,610 in 2020 and is expected to reach 115,000 by 2030. That would represent a 36.2% increase since 2015. While ADRD is the sixth-leading cause of death in the United States, it is the third-leading cause of death in San Diego County. The costs associated with the care for individuals with ADRD are significant and have a sizeable financial burden on the region. In 2020, there were more than 250,000 San Diegans providing unpaid care for those with ADRD in the county. That number of unpaid caregivers, often loved ones or friends, is projected to increase to 292,556 by 2030. On May 6, 2014 (5), the Board of Supervisors (Board) launched a broad and unprecedented initiative to develop a regional roadmap to address the disease and the impacts on individuals, their caregivers, and our community. The Alzheimer’s Project began with the formation of four roundtables (stakeholder groups) that were charged with developing a countywide Alzheimer’s plan to address this epidemic. “The Alzheimer’s Project: A CALL TO ARMS” was presented at a Board conference on December 2, 2014 (1). The Board unanimously adopted the report’s recommendations and directed the Chief Administrative Officer to work with the roundtables to develop an implementation plan and report back in 90 days. The implementation plan was approved by the Board on March 17, 2015 (3), with the direction that staff would return to the Board annually with an update. The first annual update on The Alzheimer’s Project was presented to the Board on March 1, 2016 (4), the second annual update was presented on March 21, 2017 (1), the third annual report was presented on April 24, 2018 (2), and the fourth annual update was presented on April 9, 2019 (4). Due to COVID-19, the fifth annual update was submitted on September 11, 2020 to the Board via a memo. On September 24, 2019 (4), the Board recommended implementation of the Aging Roadmap. The Aging Roadmap takes into account the future needs of the growing older adult population locally and provides a guide that allows for innovation, flexibility, and coordination to best meet these needs. It represents the County of San Diego’s comprehensive system of care, including person-centered and community-wide efforts. It is also comprised of ten focus areas: Health & Community

Support, Housing, Social Participation, Transportation, Dementia-Friendly Environments, Caregiver Support, Safety, Preparedness & Response, Silver Economy, and Medical & Social Services System. Together, the Aging Roadmap and The Alzheimer’s Project serve as a foundation for serving the growing older adult population of San Diego County including those living with ADRD. Since its establishment by the Board in 2014, The Alzheimer’s Project has instituted significant advances in several areas, including clinical standards, laboratory research and improving the care and safety of those with the disease. Participants in the initiative include the region’s top political leadership, scientists, healthcare systems, public universities, caregiver organizations, and others. Providing Better Care The Alzheimer’s Project Care Roundtable brings together providers, experts in dementia, family caregivers, residential and day care providers, public safety representatives, and community organizations to collaborate and discuss strategies to help improve the system of care for individuals living with ADRD and their caregivers. Improvements instituted through the Care Roundtable and related entities include: a) expansion of the San Diego County Sheriff’s Department’s Take Me Home program and the region’s missing persons alert system to help address incidents of wandering and to reunite families with those who have wandered off; b) creation of the County’s Respite Care Voucher Program to assist family caregivers who need an occasional break; c) establishment of Choose Well, an online directory and rating system to help families find quality residential care for their elderly loved ones; and d) the launch of the Alzheimer’s Response Team (ART) to ensure those with dementia, and their caregivers, get the right kind of help in a crisis. Through the ART program, first responders refer to ART for crisis stabilization as well as follow-up support and care management. The team consists of a County Adult Protective Services worker and a dementia specialist from Alzheimer’s San Diego. Clinical Improvements The Clinical Roundtable, which has been co-chaired by Dr. Michael Lobatz of Scripps Health and Dr. Nick Yphantides, Chief Medical Officer of the Health and Human Services Agency, is comprised of neurologists, gerontologists, neuropsychologists, and psychiatrists from San Diego County’s major health systems, community clinics, and private practice physicians, as well as community-based organizations. Improvements instituted through the Clinical Roundtable and related entities include: a) the creation of the region’s first clinical guidelines for the screening, evaluation, and management of Alzheimer’s disease and other types of dementia; b) training by Clinical Roundtable members of more than 2,800 local front-line doctors and other providers on the use of the guidelines; c) enhancing training for the next generation of nurses and other health professionals on how to recognize and treat memory loss; and d) setting a goal to become the first region in the nation to have all of its emergency departments accredited as geriatric-friendly. To date, all 18 participating San Diego Emergency Departments have applied for accreditation, with seven already approved. Finding a Cure

Collaboration4Cure (C4C) is comprised of researchers, physicians, and biotechnology representatives from the San Diego region who have established a regional fund to harvest promising local research ideas and put them on a path to help fund a cure for ADRD. C4C is led by Alzheimer’s San Diego, in partnership with the County of San Diego, the City of San Diego, and philanthropist Darlene Shiley, as well as some of the region’s finest research institutes: The Salk Institute; Sanford Burnham Prebys Medical Discovery Institute; Scripps Research; UC San Diego; and the J. Craig Venter Institute. C4C was created to fill a gap in early-stage research funding when it is critical to secure funds but difficult to do so. It provides early support to promising local projects to enable them to reach the point where they could receive federal grants. Projects supported by C4C have gone on to receive a total of $8.73 million from the federal National Institute on Aging, including $7.46 million in 2020. These research efforts include exploring new ways to block the effects of the corticotropin-releasing factor, a protein hormone which studies show may be linked to Alzheimer’s, and a search for compounds that might provide the starting points for the development of safer and more effective drugs to control ephA4, a receptor that allows brain cells to communicate and is linked to neurodegenerative processes. Public Awareness A network of community organizations and Aging & Independence Services collaborate to educate the public about Alzheimer’s disease and other types of dementia. The goal is to increase both public and professional awareness of dementia and to equip the community, caregivers, and stakeholders with the resources and skills to support San Diegans living with these diseases. Recent public awareness initiatives conducted include: the County of San Diego joined the global Dementia Friends movement to educate San Diegans about dementia and how to better interact with those living with the condition; the Caregiver Coalition of San Diego teamed up with the local chapter of the National Alliance on Mental Illness in late 2020 to host a virtual conference on improving mental health during COVID-19 and holidays, an issue with added significance for families dealing with dementia; and the County entered into the fifth year of its Age Well San Diego initiative, which is a community effort to make the region more age-friendly and dementia-friendly. Looking Ahead While significant progress has been made to address ADRD in San Diego County, the number of San Diegans with the disease continues to surge and major challenges remain. Planned improvements include: a) translating the Caregiver Coalition’s Caregiver Handbook into additional languages; b) exploring ways to improve dementia care through telehealth; c) the creation of medical exam room posters in multiple languages that explain the benefits of early diagnosis and prompt patients to talk with their doctor about their brain health; and d) bringing together the Care and Clinical Roundtables to increase awareness among healthcare providers of the wide variety of local caregiver resources.
