To determine the root cause

To take corrective action to prevent it from happening again

To save potential dollars in lawsuits, insurance claims etc…

To remain in compliance with DOT recordkeeping regulations

WHO? – All parties involved. Including damaged property owners. ~Make sure you get contact information~

WHAT? – What happened? Just the facts here, not opinions.

WHEN? – What time did the incident occur?

WHERE? – Where did the incident occur? Be as specific as possible.

And now the WHY? This is where you dig for a root cause. This is not where you share your opinion about the other driver.

All accidents / incidents have a root cause. The root cause is the underlying events or circumstances that lead to the final result.

It’s like a little kid asking “why” to everything you say.

A root cause is not determined by someone’s opinion or past driving record.

A root cause cannot be listed as “the reason this accident occurred is because that moron doesn’t know how to drive and ran into my truck”

A root cause CAN be listed as “the other driver stated that his right front tire was severely worn causing it to blow out which resulted in a loss of control of his vehicle”

Most times, a root cause is not determined at the time of the incident. Just as a detective does, once all the FACTS are gathered, they are reviewed by the investigating person(s).

A key player in determination of a root cause can be photos. Pictures capture the scene as it was, along with ALL the details that may otherwise be forgot.

• All vehicles involved

• Damage to all vehicles and property

• Traffic signs/signals/flow

• Nearby businesses /addresses

• Debris scattered on roadway

• Road conditions

• ANYTHING AND EVERYTHING!!!

All Plant Managers and Area Managers should know by now what forms to use and where to find them.

Most of the information on the reporting form can be filled out by a direct manager. Many areas of the form have pre-loaded information so it’s as easy as point and click!

Reports should be submitted to the safety department ASAP so they can be reviewed and submitted to Titan America within 24 hours.
