RECOVERY PROGRAM STEPS – Fall 2017
1. Complete a Probation Intake Session and your Self-Assessment as soon as possible (for more information on how to complete this session log on to success.unl.edu/current/probation)
2. Identify your Academic Recovery Guide and schedule a meeting in MyPlan (see list below) 3. Meet with your Academic Recovery Guide and create an Academic Recovery Plan before September 18th, 2017 4. Complete Academic Recovery Plan by October 13th, 2017 to have your hold lifted before Priority Registration begins

Academic Probation Self-Assessment
A student who receives a semester grade point average (GPA) of less than 2.00 or ends a semester with a
cumulative GPA below 2.00 will be placed (or will continue) on probation. The student will remain on
probation until a semester is completed with both a semester and cumulative GPA at or above 2.00 or until
the student is dismissed.
Please complete the assessment and be prepared to discuss the topics with your Academic Recovery Guide.
You MUST bring this form to the meeting with your Academic Recovery Guide to develop your Academic Recovery Plan.
