This kit explains each part of the program: what it is, how to
access it, and how you earn Health Account credits as part
of the new Health Account Plan (HAP).

Beginning October 1, 2012, PG&E and Provant are offering the programs below to Management and Administrative & Technical employees as part of their Health Account Plan.

In-network, out-of-network, a new diagnosis, multiple treatment
options, deductibles, co-insurance, Health Account credits: the
health care system can be difficult to navigate.
That’s why PG&E has teamed up with Provant to provide a one-stop
shop for all your health care questions. Advocates help you find
in-network providers and access the quality care you need. They
are available if you are concerned about a treatment or diagnosis,
if you are not clear on what to do next, or if you do not have all of
the information that you feel you need. Advocates also help with the
paperwork, such as explaining claims or resolving billing issues.
With just one number, you and your family have the health answers
you need. Advocates are available Monday through Friday, 5 a.m. to
4 p.m. Pacific time, at (866) 271-8144. You are also able to access
them on the Health & Wellness portal at https://pge.provantonline.
com. See page 13 for more services available through the portal.

Health screenings give you important information about your health numbers. Numbers outside the ideal range can indicate that you are at a greater risk for heart disease, diabetes and other conditions before you show any symptoms. Screenings can help you to identify your health risks and set health goals that may prevent these conditions. Fasting is not required for health screenings.
If you are enrolled in PG&E’s medical plan and participate in the annual health screenings and the tobacco-free program, you can earn additional Health Account credits which, when coupled with your automatic deposit, can reduce your medical plan deductible to zero every year.
What do the health screenings include?
Health screenings provided by Provant include a non-fasting finger stick to check your cholesterol and glucose, a blood pressure screening, a Body Mass Index (BMI) screening measured by height and weight, and an optional tobacco screening done by mouth swab to check cotinine levels.
Health screenings provided by your health care provider cover the cholesterol and glucose, blood pressure, and BMI screenings. Tobacco screening will need to be done separately at an onsite event.
If you participate in the health screening, you earn $250/single and $500/family in your Health Account.

Option 1: Onsite Health Screening Event
Where: Onsite events are scheduled at worksites where a volunteer site contact can guarantee 16 or more participants. If you don’t see a screening scheduled at your worksite, email [email protected] to volunteer to coordinate an event at your site. Events can be scheduled 7 days a week, 24 hours a day.
How: Go to https://pge.provantonline.com and check the Health Screening Schedule Blog to find an event near you along with how to register. You must schedule an appointment online at least 24 hours ahead of time. The screening takes approximately 25-30 minutes per participant.
Results: Provided immediately onsite by Provant’s health coach. Tobacco results available online 2-3 weeks after event.
Option 2: Concierge Service Health Screening
Where: Individual screenings can be scheduled at your home or office and can also be coordinated for smaller groups (2-15 participants) at your worksite. Available Monday through Friday, 6 a.m. to 6 p.m. Pacific time. How: Call Provant at (866) 271-8144 at least two weeks in advance to schedule an appointment. The screening takes approximately 25-30 minutes.
Results: Available online 2-3 weeks after event.
Option 3: Your Health Care Provider Conducts a Screening
Where: Get the screening from your health care provider during an office visit. Tobacco screening will need to be done separately at an onsite event.
How: Download form from https://pge.provantonline.com and bring to your health care provider to complete and fax to Provant at (401) 398-1708. Please check with your health care provider on how to prepare for your screening. For a tobacco screening, go to https://pge.provantonline.com and check the Health Screening Schedule Blog to find an event near you. You can get the tobacco screening on a walk-in basis, no registration required.

Tobacco use, including chewing tobacco and snuff, is the second leading cause of death and is the leading cause of preventable disease and premature death in the United States. On average, tobacco users die 13 to 14 years earlier than non-tobacco users.
Testing for tobacco use is an optional test offered during our onsite or concierge health screenings. If you test negative for tobacco use, you can earn $250/single or $500/family for your Health Account.
If you test positive or you opt out of the screening because you currently use tobacco, you can still earn the $250/single or $500/family for your Health Account by completing Provant’s tobacco cessation program.
The five-week telephonic comprehensive tobacco cessation program is conducted by a certified tobacco cessation specialist who helps you improve your health by teaching you the necessary behavior skills required to achieve a tobacco free life. Nicotine replacement therapy is available to compliment the program.
To receive your 2013 Health Account credits in January, enroll in the Provant tobacco cessation program no later than October 31, 2012.
For your Health Account credits:Complete the health screening and test negative for tobacco or complete the Provant tobacco cessation program by December 19, 2012, and you will receive your Health Account credits by January 1, 2013. However, screening events will be held from January through September of 2013 for you to participate and earn credits for 2013. In the fourth quarter of 2013, you can participate in a screening and earn your credits for 2014.

In Provant’s health coaching program, you’ll be teamed up with a
dedicated health coach and you’ll start by setting goals based on your
health risks, needs and interests.
To participate in health coaching, call Provant at (866) 271-8144 to
schedule your first health coaching call. Health coaching appointments
are available Monday through Friday from 7 a.m. to 7 p.m., all time
zones.
During your first appointment, you and your health coach will
determine the number of follow-up appointments needed to meet
your health goals and you will be able to choose to continue by phone
or with scheduled secure messaging appointments online.
After your first appointment, your coach will follow up with you at
regularly scheduled appointments to check your progress on meeting
your goals, answer your questions, provide suggestions and help you
overcome any obstacles.

participate in or enter into the portal is completely confidential.

health risk questionnaireThe Health Risk Questionnaire offered by Provant is a totally private,
15-minute survey that asks about your current health, diet, fitness,
safety and general lifestyle. If you have completed your health
screening, the results will be pre-populated to ensure that your
results are based on the most accurate data.
The results of the Provant Health Risk Questionnaire are
completely confidential.
Completing this survey will help you and your health coach
understand your current health and provide you with a
comprehensive report and recommended resources and workshops
to address your health risks.
To take the Provant Health Risk Questionnaire, log in to the Health
& Wellness portal at https://pge.provantonline.com and complete
the registration. After you have logged in, on the left side, find the
“Health” heading and click on “Health Risk Questionnaire.” Once
complete, you will immediately receive your personal outcomes
report with active links to portal tools and features to help you
address any health risks you may have.

Who is eligible to participate in the wellness program?All PG&E Management and A&T employees and their spouses or domestic partners are eligible to participate in the program, however, health screenings and tobacco-free tests are only available to employees to earn credits for a Health Account.
Do I have to participate in the wellness program?No, participation is voluntary. However, in order to receive additional Health Account credits, you must participate in a Health Screening (additional $250 for single coverage, $500 for family coverage) and test tobacco free or complete Provant’s tobacco cessation program (additional $250 for single coverage, $500 for family coverage).
How is employee privacy and confidentiality protected in the wellness program?Provant’s clinicians will administer the health screenings and track the data with the utmost privacy and in full accordance with all state and federal regulations, such as the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
Will PG&E receive a copy of my results?No. PG&E will not receive any individual data – all data is provided on an aggregate level. All individual results are strictly confidential. Provant adheres to HIPAA regulations.
Will my results affect my eligibility for any benefits?No, your results will not affect your eligibility for any type of PG&E-sponsored benefit.

Who is eligible to participate in this program?All PG&E Management and A&T employees and their families are eligible to participate in health advocacy.
What services does the Health Advocacy program include?There are many benefits to participants. These include, but are not limited to, the following:
Easy access to valuable health information, education and personal support to help you make changes that can positively influence your health; assistance locating quality doctors, hospitals and centers of excellence covered by your health care plan; and timely resolutions of health care billing and claims disputes, followed up by reports of the results and answers to any further questions you may have.

What will be measured at the onsite health screening events?The onsite health screening will measure cholesterol, glucose (blood sugar), height, weight, body mass index (BMI), and blood pressure. Provant’s team will only collect a few drops of blood from your finger to test your cholesterol and glucose. Fasting is not required.
What types of health risks do these screenings predict? • Total cholesterol is one of the major risk factors for heart disease.• HDL is the “good” cholesterol, the type of lipoprotein that removes the
“bad” cholesterol (LDL) from your body.• Total cholesterol cardiac risk ratio is the calculation of the amount of
good cholesterol (HDL) to total cholesterol (TC). Understanding your cholesterol level is an important tool in evaluating whether you are at risk to develop coronary artery disease.
• Glucose level indicates your risk for developing diabetes.
Do I need to fast for the health screening? No, you may eat as usual, but please drink plenty of water as being properly hydrated will help with the screening.
How long will the screening take? The screening will take approximately 25 to 30 minutes per participant.
Can my spouse participate in the screening? Dependents and spouses don’t have access to the free onsite and concierge screenings but they can get free annual physicals and free preventive services under the PG&E-sponsored health plan through their primary health care provider. Your participation in the health screenings earns credits for the entire family.
Do I have to pass the health screening to get my Health Account credits?No, there is no “pass or fail” for the cholesterol, glucose, blood pressure, BMI, and body fat screenings. You’ll earn Health Account credits simply for participating in the health screening.
What will I receive at the onsite screening event? You will receive your personal screening results as well as the normal ranges for each screening. In addition, you will receive a detailed educational brochure about each of the screenings and how to interpret your results. You will be encouraged to enroll with a health coach. Your health coach will provide a one-on-one, confidential coaching session that will include guidance for creating a personal health action plan, with actionable items that you may begin working on immediately. The health coach identifies health risks that may require follow-up with your primary health care provider; encourages participation in follow-up wellness programming; and provides you with a road map for improving or maintaining your current health. How do I register for a health screening provided by Provant?Visit the Health Screening Schedule Blog for more information on events and how to register. For a concierge service health screening, call Provant at (866) 271-8144 to schedule an appointment.
If I have recently been to my health care provider, do I have to participate in a health screening? If you have been to your health care provider and lab work was completed, you may submit a Health Screening Results form in lieu of completing a screening. Obtain the form from the portal at https://pge.provantonline.com. Have your health care provider complete and return to Provant by December 19 to receive your Health Account credits in January for 2013. You are responsible for any charges incurred by electing this method of screening. Tobacco screening will need to be done separately at an onsite event.
Can I just walk in to an onsite screening?No, appointments are required for the screenings, however, tobacco-only screenings are available on a walk-in basis.
How do I earn credits?You earn Health Account credits by participating in a health screening and testing negative for tobacco or completing the Provant tobacco cessation program. See pages 7 - 10 for more information.
Can I still earn my Health Account credits if I use tobacco?Yes. If you complete the Provant tobacco cessation program, you can earn your Health Account credits.
