A Guide to Completing the Annual Well Status (Form 3) and Annual Production Report (Form 8) Under the Oil, Gas and Salt Resources Act

Background Section 13 of the ‘Oil, Gas and Salt Resources of Ontario Provincial Operating Standards’ (Operating Standards) mandates that all commercial operators are required to submit an Annual Production Report(s) (Form 8) and an Annual Well Status Report (Form 3) by the 15th of February each year, for the previous calendar year’s production. These reports are to be submitted to the Ministry of Natural Resources and Forestry (MNRF) at 659 Exeter Road, London, Ontario N6E 1L3. These forms must be completed in duplicate with all required information accurately recorded. Failure to do so could result in further compliance action for submission of inaccurate or incomplete documents and/or obstruction of an inspector under the Oil, Gas and Salt Resources Act (OGSRA), 1990 Section 4. (4)
Definitions Condensate: a low-density, high-API gravity liquid hydrocarbon phase1. Commingled: the production of oil and gas from two or more wells through a common well-bore or flow line without separate measurement of the production of each well. The combined wells must meet the following stipulations:
a) production from a pool averages less than 1m3/d oil per well and 0.3 103m3/d gas per well and all the oil gas interests subject to the commingling have been unitized;
b) where production recovered at surface originates from multiple deviated, horizontal or lateral wells drilled from the wells at surface and all the oil and gas interests affected by the commingling have been unitized, and
c) production of occurs from gas wells located in Lake Erie (Provincial Operating Standards section 6.2.1) Shut-in Pressure: the annual shut-in pressure measurement shall be:
(a) taken with a dead-weight gauge or other equipment acceptable to the Ministry; (b) taken after sufficient shut-in time has passed for reservoir stabilization or 24 hours shut-in time whichever is the lesser; and (c) reported as gauge pressure on Form 8 (Provincial Operating Standards section 6.12.1)

Form 8: Annual Report of Monthly Oil and Gas Production This form is used to report the production of a single well or a group of wells that are commingled together. All gasses and fluids produced from these wells are to be recorded on these forms. Operators must use the current approved version of this form which can be found at the Oil, Gas and Salt Resources Library (OGSRL) website through the following links:

The Form 8 is an official document of the Ontario Government and must be used for the purpose of reporting monthly production. Upon completion of the form, it must be submitted, in duplicate, and received by the MNRF Petroleum Operation Section (POS) at 659 Exeter Road, London, Ontario, N6E 1L3 no later than February 15th of each year.

1. Single Well Report or Commingled Well Report: For a single well, select the “Single Well Report” box. For multiple commingled wells, select the
“Group/Commingled Well Report” box. 2. Production Volumes: All volumes of Oil/Gas/Condensate and Water need to be tabulated for the entire year and then recorded in the correct field based
on month produced. “Water” includes all forms of water. Some examples are (but not limited to) brine/salt water, sulphur water, brackish water). If there was no volume produced for any month, enter this value as 0.
3. Pressure Data: The well needs to be shut-in for a period of time, once per year, in order to determine the shut-in pressure on the well head. The pressure (in kPag), the shut in time (in hours) and the date it was shut in all need to be reported on the Form 8 (Provincial Operating Standards section 6.12.1).
4. Time On Production: The number of days the well was producing each month.

1. Used As Fuel: If any of the oil, gas or condensate produced from the well was used as a fuel onsite, this information needs to be reported in Part B, along with any dollar values associated with it being used as a fuel.
2. Sales: All volumes of oil, gas, condensate and water sold must be documented in Part B, along with the dollar values associated with those transactions. 3. Disposal: All volumes of oil, gas, condensate and water disposed of must be documented in Part B along with the dollar values associated with each

Total Production (1000m3) gas (m3) oil and condensate: Sum the values reported in “Sales” ”, “Disposal”, “Secondary Recovery”, or “Other” from Part B (see page 8) and enter them into the correct field based on if it was oil, gas or condensate. Note: Do not include the volume that has been entered in the field “Used As Fuel” in your calculation. This volume is exempt from the annual well licence fee.
2. Licence Fee Payable (A)x(B): Use the volume documented in column (A) in Part C and multiply it with the corresponding licence fee found in column (B) in Part C. Note: The licence fee is based on the volume produced during the reporting year. Volumes stored from previous reporting years and sold during the current reporting year should not be included in the calculation of the licence fee payable.

Form 3: Annual Well Status Report A Form 3 is used to report the status of a well at the end of the production/calendar year, any purchases, and/or sales of wells, and to calculate the annual well licence fee using the information collected on the Form 8s. All wells (with the exception of wells that were plugged the year previous to the current reporting year) must be listed on this form regardless of the production status. Well Status -Type: This field is used to describe what type of well it is. The terminology to be used is listed on the Form 3 under the table. 9. Well Status – Mode: This field is used to report the operating status of the well as of December 31st of that reporting year. The terminology to be used is
