An Incident Report provides a description and analysis of each equipment failure.

The following demonstrates the Manoeuvring Module. The same procedures apply to

8.1 Creating an Incident Report

Maneuvering

• To create a new report Click on Create.

8.1.1 First Report Title Screen

Manoeuvring

• Select Create First Report option.

• Click on Next to continue or Cancel to return to the previous screen.

Note: When a first report has been created any number of intermediate

reports can be created.

Click on the First Report.

8.1.2 Entering Data in an Incident Report

Manoeuvring

• Ship Name, Date and SI.No: These fields are automatically generated.

• Voyage No.: Enter the Voyage Number.

• Service Line: Enter the departure and arrival details of the voyage selected.

• Failure Date: This will default to the current date. Click on the symbol to

select date and view calendar. Failure date should not be greater than the

report created date.

• Department: Select from three options. Click on symbol to display list

and select the required option

o Deck

o Electrical

o Machinery.

• Place of Failure: Enter the place where the incident occurred.

• Type Of Failure: Click on symbol and select Maneuvering from the list.

• Equipment/Machinery: Click on symbol and select the required

equipment/machinery type form the options listed in the dropdown list.

• Manufacturer: Enter the name of the equipment manufacturer.

• Type: Enter the Equipment’s serial number.

• Ship’s Condition: Click on symbol to select the required option.

• Mode of Discovery: Click on symbol to select the required option.

Maneuvering Sub Menu displays various methods of maneuvering.

• Effect on Services: Select the required option by clicking on symbol

o Affected

o Unaffected

• Effect on Time: Click on Effect on Time field and select the time in Hours,

Minutes and Seconds (this is service affected time). In this small window,

select (hours) MM (Minutes) SS (Seconds). And Click on OK.

• When the service is Unaffected, Effect on Time field will not be enabled.

• Master: Enter the Master’s name.

• CO/CE: Enter CO/CE’s name.

• Phenomenon or Condition of Failure: This is a text field. Enter a brief

description of the failure.

• Final or Subsequent Treatment: If either final or subsequent treatment is

required enter a brief description of the treatment.

• Direct Cause of Failure: Enter the direct cause of failure.

• Presumed Reason: Enter the assumed reasons for the failure.

• Remarks: Enter the general remarks.

Attachment is optional. If the report requires any external file to be attached then

attach the file before saving the report as Active. Files can be attached in a draft

report.

Browse Screen

• Select the required file and Click on Open.

Click on Attachments button

• To save Click on Save. A pop up screen will appear asking if the report is to

be Saved as Draft or Approve and Send.

Note:

• The Cancel button will take you back to the summary list screen without

saving any of the details entered in the report screen.

cannot be transferred to Office.

• Select Create Intermediate Report option.

• Click on Next to continue or Cancel to return to the previous screen.

Note:

Any number of intermediate reports can be created.

The first intermediate Report can be created only when the First Report is

saved as active.

The Second Intermediate Report can be created only when the First

Intermediate Report is an Active Report.

Click on the Intermediate Report.

A Final Report can be created only when the last intermediate report is an

Active Report. Only one Final Report can be created.
