That’s what our KMHS staff do every day, with and for the 6,800 children, families and adults living in Kitsap County who choose our services. Across our spectrum of services, from inpatient treatment, to residential services and supported housing, and our Outpatient Care Teams, our staff hold hope because they know from experience and research, its true - outcomes for people with serious mental illnesses and serious emotional disturbances show treatment works, and that with treatment, recovery happens. Every day mental health crises occur for someone in our community. Our 24/7 Crisis Clinic of the Peninsulas staff and volunteers listen, refer, and support over 8,000 callers, and another 13,000 2-1-1 callers seek community services information. The Crisis Response Team of Designated Mental Health Professionals (DMHPs) are available 24/7 to evaluate if a person is in imminent danger of harm to themselves or to others and meets the legal criteria to be admitted for involuntary inpatient treatment. Because mental illnesses are not independent of the body, we strive to provide health care that supports the whole person with mental health and substance use treatment, coordination with primary care providers, and recently with PCHS, Dental care. Clinicians in our South Kitsap office, our North Kitsap Team, our Bremerton locations, and staff working in co-located partnerships in primary care offices, schools, social service agencies, jail, or who make visits in the home, nursing homes and elsewhere do this work each day because they KNOW treatment works and recovery happens. In the last three years since Medicaid expansion, access to community mental health services is up over 30%, representing a large increase in people seeking and receiving first time services at KMHS. The bad news is community mental health in Washington remains underfunded, and the current health care work force insufficient to meet the increase in people now eligible for services. Yet our community has never been more concerned about preventing and providing interventions and treatment for persons with serious mental illnesses and serious emotional disturbances than it has been these last few years. The filling of service gaps through the Kitsap County Behavioral Health Strategic Plan and local Mental Health and Chemical Dependency Sales Tax has resulted in stronger connections between the community mental health system of care, law and justice, primary care, housing and other service providers. For these reasons, in this Season’s newsletter, we provide a map of what the KMHS integrated system of health care looks like at this time for persons with serious mental illnesses and serious emotional disturbances. It includes a diagram of our services, and a brief description of important partnerships that are weaving behavioral health services into a far more comprehensive system of care than ever before. And for this too we hold hope that our community and the people who live in it are healthier because of our shared commitment. To shape the future of mental health through state of the science service delivery, community partnerships and advocacy.

AND NOW IN OTHER NEWS!New! WISe - Wrap Around, Intensive Supports for Children and Families “WISe” or Wraparound Intensive Services are now available to all Medicaid eligible youth between the ages of 0 – 21 and their families. Four WISe Teams, each consisting of Care Coordinator, Therapist, and Peer Support Specialist, providing services 24/7, and together with the family, seek out formal and informal supports from the community. WISe Teams build a supportive structure around and with the family, teaching self-advocacy skills and using this supportive structure to help families become self-sustaining and self-advocating. WISe Teams currently serve 32 families.

Peer Specialists Core Members of Outpatient Teams KMHS Teams now include 16 Peer Specialists! Each Outpatient Team is assigned a Specialist, and five specialists are trained as Peer Navigators specifically to help clients improve their skills and confidence to “navigate” the physical health care system, working side by side with clients using a method called “for them, with them, by them.” The Navigator coaches clients to prepare for healthcare appointments, and to set and achieve their health goals, eventually fading away as the client learns to navigate the healthcare system on their own. New Washington State funding has resulted in hiring of two Peer Specialists for the “Housing and Recovery through Peer Services” or “HARPS” program. The HARPS Team, led by a Behavioral Health Professional, is co-located at Kitsap Community Resources and offers services aimed at securing permanent supportive housing, primarily for adults discharging from inpatient psychiatric and chemical dependency facilities. Another new program - Peer Bridgers - provides two Peer Specialists offering 90 days of intensive supports to adults exiting Western State Hospital plus 120 days support post-discharge to the community.

PCHS DENTAL CLINIC NOW LOCATED AT KMHS!Peninsula Community Health Services, in partnership with KMHS, now offers dental services at the KMHS Almira campus for KMHS clients who are also patients of PCHS. PCHS offers both children and adult dental care to Apple Health/Medicaid members. PCHS also offers emergency services to non-PCHS clients or uninsured adults.

MEDICALLY AFFILIATED PARTNERSHIPS Community Primary Care Providers For mild to moderate mental health and/or chemical dependency concerns, primary care is often the first place people turn to for help. If serious mental health concerns are noted, outpatient or inpatient treatment is often referred to a specialty care setting, with the patient returning to primary care when their illness is stabilized or can be managed by their primary care provider. KMHS also provides a Behavioral Health Professional at 5 Harrison HealthPartner clinics, creating an easy flow for patients between the two systems of care.

Primary Care Services At KMHSBoth Harrison HealthPartners and Peninsula Community Health Services offer primary care services to KMHS clients, with offices located at the KMHS Bremerton Campus.

New PCHS Dental ServicesMeeting a longstanding need for dental care, PCHS has opened a four chair dental clinic on the KMHS Bremerton Campus, providing services for both children and adults.

Nursing Home LiaisonPsychiatric ARNP and Geriatric Mental Health Professional provide consultation to nursing home staff regarding patient behavioral health concerns.

Behavioral Health Network Health HomesKMHS Behavioral Health Professional at request of various Health Plans, works in the community to conduct assessment & care coordination for managed care affiliated adults who have mild to moderate behavioral health needs.

Kitsap ConnectsCollaboration of Kitsap Public Health District, Kitsap Community Resources, Bremerton Housing Authority, Salvation Army, and Kitsap Mental Health Services to connect high utilizers of Emergency Room, Emergency Medical Services, and law enforcement encounters. Team of KPHD Nurse, KCR Housing Specialist, KMHS Behavioral Health Professional offer outreach and engagement to connect 50 people with treatment, services, and housing.

Through partnerships, mix of 100+ units of KMHS permanent supported housing, housing authority affiliated, or private landlord housing, totaling 100+ units for persons with chronic, severe mental illnesses.

ABOUT KITSAP MENTAL HEALTH SERVICES

Inpatient, residential, and outpatient treatment services offer evidence-based treatment, using trauma informed care to help people reach their recovery goals. We offer whole person care through mental health and substance use treatment and care management, close care coordination with on-campus and community-based primary care providers, and with partner PCHS, now offer dental care to KMHS clients and the community.

Many community partners assure people receive behavioral health care in the right place, in the right way, at the right time. Together we are weaving a web that connects emergency room and hospitals, substance abuse treatment, and primary care providers; creates partnerships for early intervention at home and school, supports multiple law enforcement, jail, and justice endeavors, and works to ensure housing, food, and other social supports together with social service agencies and community collaborations.

Salish Behavioral Health Organization provides 90%+ in Medicaid funding. Other funds includes WA State and Medicare funds.

The Crisis Response Team of Designated Mental Health Professionals (DMHPs) are available 24/7 to evaluate if a person is in imminent danger of harm to themselves or to others and meets the legal criteria to be admitted for involuntary inpatient treatment.

Because mental illnesses are not independent of the body, we strive to provide health care that supports the whole person with mental health and substance use treatment, coordination with primary care providers, and recently with PCHS, Dental care. Clinicians in our South Kitsap office, our North Kitsap Team, our Bremerton locations, and staff working in co-located partnerships in primary care offices, schools, social service agencies, jail, or who make visits in the home, nursing homes and elsewhere do this work each day because they KNOW treatment works and recovery happens.

There is good news. In the last three years since Medicaid expansion, access to community mental health services is up over 30%, representing a large increase in people seeking and receiving first time services at KMHS. The bad news is community mental health in Washington remains underfunded, and the current health care work force insufficient to meet the increase in people now eligible for services. Yet our community has never been more concerned about preventing and providing interventions and treatment for persons with serious mental illnesses and serious emotional disturbances than it has been these last few years. The filling of service gaps through the Kitsap County Behavioral Health Strategic Plan and local Mental Health and Chemical Dependency Sales Tax has resulted in stronger connections between the community mental health system of care, law and justice, primary care, housing and other service providers. For these reasons, in this Season’s newsletter, we provide a map of what the KMHS integrated system of health care looks like at this time for persons with serious mental illnesses and serious emotional disturbances. It includes a diagram of our services, and a brief description of important partnerships that are weaving behavioral health services into a far more comprehensive system of care than ever before. And for this too we hold hope that our community and the people who live in it are healthier because of our shared commitment.

AND NOW IN OTHER NEWS!New! WISe - Wrap Around, Intensive Supports for Children and Families “WISe” or Wraparound Intensive Services are now available to all Medicaid eligible youth between the ages of 0 – 21 and their families. Four WISe Teams, each consisting of Care Coordinator, Therapist, and Peer Support Specialist, providing services 24/7, and together with the family, seek out formal and informal supports from the community. WISe Teams build a supportive structure around and with the family, teaching self-advocacy skills and using this supportive structure to help families become self-sustaining and self-advocating. WISe Teams currently serve 32 families.

Peer Specialists Core Members of Outpatient Teams KMHS Teams now include 16 Peer Specialists! Each Outpatient Team is assigned a Specialist, and five specialists are trained as Peer Navigators specifically to help clients improve their skills and confidence to “navigate” the physical health care system, working side by side with clients using a method called “for them, with them, by them.” The Navigator coaches clients to prepare for healthcare appointments, and to set and achieve their health goals, eventually fading away as the client learns to navigate the healthcare system on their own. New Washington State funding has resulted in hiring of two Peer Specialists for the “Housing and Recovery through Peer Services” or “HARPS” program. The HARPS Team, led by a Behavioral Health Professional, is co-located at Kitsap Community Resources and offers services aimed at securing permanent supportive housing, primarily for adults discharging from inpatient psychiatric and chemical dependency facilities. Another new program - Peer Bridgers - provides two Peer Specialists offering 90 days of intensive supports to adults exiting Western State Hospital plus 120 days support post-discharge to the community.

PCHS DENTAL CLINIC NOW LOCATED AT KMHS!Peninsula Community Health Services, in partnership with KMHS, now offers dental services at the KMHS Almira campus for KMHS clients who are also patients of PCHS. PCHS offers both children and adult dental care to Apple Health/Medicaid members. PCHS also offers emergency services to non-PCHS clients or uninsured adults.

Holding Hope. That’s what our KMHS staff do every day, with and for the 6,800 children, families and adults living in Kitsap County who choose our services. Across our spectrum of services, from inpatient treatment, to residential services and supported housing, and our Outpatient Care Teams, our staff hold hope because they know from experience and research, its true - outcomes for people with serious mental illnesses and serious emotional disturbances show treatment works, and that with treatment, recovery happens.

Every day mental health crises occur for someone in our community. Our 24/7 Crisis Clinic of the Peninsulas staff and volunteers listen, refer, and support over 8,000 callers, and another 13,000 2-1-1 callers seek community services information.
