As a result of long-term, continuous research into biomaterial – body environment in-

teractions, LASAK was the first manufacturer of implant system on the market that hasbeen able to offer unique hydrophilic, nanostructured, bioactive surface treatment. The

invention of the BIO-surface has given LASAK a leading global position in the develop-ment of implant surface modifications. LASAK's unique BIO-surface modification speeds up

the formation of a functional bone-implant interface, thus improving the implant’s secondary

stability in the early healing phase. Thanks to the BIO-surface, the stability dip (often observed innonbioactive surfaces) is eliminated. The outstanding performance of LASAK BIO-surface implants has been

documented for even the most demanding indications.

THE BIONIQ® DENTAL IMPLANT SYSTEM

The BioniQ® implant system includes tapered implants offering easy insertion and

high primary stability in soft bone as well as straight implants for easy positioningin dense bone. The horizontal and vertical set-off of the implant-abutment con-

nection from the bone level, together with the implant mini-threads, contribute to the stability of the marginal bone and soft tissues surrounding the abutment and

provide improved restoration esthetics. The state-of-the-art implant constructionenables safe and precise insertion and optimized load distribution in the bone tis-

sue. Implants are available with a unique hydrophilic, nanostructured, bioactivesurface (BIO). A single system organizer provides instruments for the insertion of

implants with both tapered and straight design.

CONNECTION

A unique combination of 4 stabilizing components:

• Deep cone ensures stability and tight seal of the connection• Solid hexagon as anti-rotation element

• Internal conical connection for single tooth restorations• External platform for BioCam bridge restorations

Q-Lock connection

The stress of implant and the abutment is examined through the use of strength test with kinematics of chewing. The test consists

of 5 million cycles, where strength of the connection implant-abutment-screw is checked. This amount of cycles corresponds toabout 20 years of the implant lifetime. The given force is likely the maximum of stress force providing the infinite lifetime of the

implant.

Marking and packaging of the products

The implants are supplied in two blisters and an outer paper box. The transparent blister allows the visual checking of the product beforeits use. There is a label on the back of the blister with implant information and labels which should be fixed onto the patient documentation.

The paper box has a label, too (see the picture below).The instruments and prosthetic components are supplied decontaminated but not sterile. For an example of the label, see the picture below.

BioCam technology represents a unique production system of precise superstructures which allows direct fixation in dental implants

without the use of abutments or other connecting elements. This reduces the number of gaps, between the implant and the superstructure,

which can be a potential source of bacterial contamination.

LASAK Ltd. offers a twenty-year guarantee on all BioniQ® system implants. In the case of implant loss or failure within twenty years of the implantation date, LASAK Ltd. will replace the implant, including cover screw, free of charge, on condition that: the implant was

inserted with the use of original BioniQ® system components, and in accordance with the manufacturer's recommendations, instructions and manuals.

GUARANTEE TERMS AND CONDITIONS

Entitlement to guarantee:

Claims on the guarantee will be honoredproviding original surgical and prostheticcomponents of the BioniQ® system wereused and the implantation performed in ac-cordance with commonly accepted medicalpractice and adhering to the manufacturer'sinstructions and recommendations as pub-

lished in the manuals and leaflets of LASAKLtd. Implantations with contraindications,as described in the instructions and manu-als of LASAK Ltd., are not covered by the guarantee. The guarantee can be claimedsolely by the medical entity that undertook the implantation, it cannot be claimed by the patient or by any other person. Anymedical entity that is financially in debt toLASAK Ltd. for delivered goods or servicesis, however, excluded from this guarantee.

Exclusions:

This guarantee does not cover implants that are lost:• due to a patient's insufficient oral hygieneand/or due to infections,• as a result of a personal accident or a patient's inappropriate behavior,• due to overloading.

This guarantee does not cover any provi-sional implants.

Changes to and termination of this

guarantee:

LASAK Ltd. reserves the right to makechanges to, or to terminate, this guarantee, without prior notice.

How to make a claim under this guar-

antee:

To make a claim under this guarantee, acompleted form "Record of Failed Implant"should be sent, along with the sterilizedimplant and other components used, to the business address of LASAK Ltd. within30 days of the implant failure.

The conditions stated above are generaland may vary slightly in different countries.The valid conditions of the guarantee for a given country will be provided by the rep-resentative of LASAK Ltd. in each country.

LASAK Ltd. maintains the right to modify, terminate, change specifications or prices without prior notice.

GENERAL BUSINESS TERMS

PRICING – All the above prices are ex-works(EXW) Prague, Czech Republic, Incoterms2000, and do not include any commission, VATor other duties, nor transport or packing costs.The seller reserves the right to change theprices without prior notice.

ORDERING – Orders may be receivedby writing, Internet, telephone or fax. Anorder is deemed accepted upon confirma- tion by the seller, or upon delivery of the

products, whichever is earlier. In every or-der must be clearly stated the full statutoryname of the buyer, delivery address, speci-fication required, delivery date, the pre-ferred mode of transportation and contactperson with their phone number.

DELIVERY TIME – The delivery time de-pends upon the ordered quantity and has to be agreed individually in advance. Gener-

ally, orders are dispatched by the first avail-able carrier within three working days afterreceipt of the order or payment.

PACKAGING – Implants are supplied insterile packing. Other components of theimplant system are not supplied sterile, butdecontaminated in sealed transparent flatbags.

PAYMENT TERMS – All listed prices are

net at the account of the seller. Payment terms are payment in advance or confirmed,irrevocable, documentary L/C. If paymentis late the maximum statutory interest rate will be applied to the late amount. Furtherdeliveries may be suspended until fullpayment for any previous unpaid shipmenthas been received. All products remain in the ownership of the seller until the fullinvoiced price is settled.

TRANSPORTATION – An individuallyagreed mode of transportation is used, usu-ally standard mail to the specified addressof the customer. The transportation costsare charged separately and are not includedin the listed prices.

DELIVERY TERMS AND INSURANCE

The listed prices are ex-works (EXW)Prague, Czech Republic, Incoterms 2000delivery terms. We are prepared to ship

the requested material to the specified ad-dress based on DDU, CIP or other agreeddelivery terms upon request. All costs thusincurred are charged over and above thelist price of the goods.
