CHAPTER 1: COURSE INFORMATION

Course Overview Three credits is equal to 120 hours of practice experience. The course may be completed on part-time or full-time basis as supervision and practice site permits. Development and approval of proposal is discussed below. As a first step, students must identify their own Rotation Supervisor (see Course Instructor section below) for this elective and have this pharmacist approved by the Course Coordinator prior to embarking on the creation of a proposal. The Course coordinator must also approve the proposal prior to enrolling you in the course. Up to 25% of the rotation may be spent on a project within the specialty area (examples include scholarly activity related to specialty area (e.g. case report, review article), patient care program development (e.g. patient education material), etc). A final course presentation is required. This course may be repeated once (with a different rotation supervisor). This is only permitted in separate terms and the spring/summer term is considered one term.

Course Instructors A student wishing to enrol in Pharmacy 455 will be responsible for identifying a Faculty member with a clinical practice or a practicing pharmacist * to be a Rotation Supervisor for his/her elective. Students must let the course coordinator know preceptors they want to appraoch before doing so. Students are encouraged to contact preceptors who have a practice in which they are interested, to explore their potential interest in the area. It is the student’s responsibility to demonstrate to the course coordinator how a preceptor will help them meet their goals. Taking this course is contingent on rotation supervisor availability. * Practicing pharmacists can be approved to be a Rotation Supervisor if they meet the qualifications, requirements and responsibilities outlined in Appendix 1. Students should also be familiar with this document to guide their selection of a Rotation Supervisor, and should share this document with prospective Rotation Supervisors to initially assess interest and eligibility. Before students approach practicing pharmacists to be their Rotation Supervisor, they must first notify the course coordinator before proceeding to invite that pharmacist to participate in the course. Of note, if the desired Rotation Supervisor is a front-line AHS/Covenant staff pharmacist**, the Rotation Supervisor’s scheduling manager needs to be additionally contacted by the student/course coordinator (course coordinator will advise) prior to approaching the potential Rotation Supervisor to ensure scheduling feasibility. Students need to provide the course coordinator with confirmation that their chosen Rotation Supervisor does or will meet the qualifications, requirements and responsibilities in addition to a brief description (100-200 words) of the practice area for which they are seeking approval. A form to guide the collection of this information is in Appendix 2.

Proposal Development and Registration Prior to being enrollied in the course (by the course coordinator), a 1-2 page rotation proposal (maximum 2 pages using 11 point Calibri font) outlining your learning objectives (see Course Objectives section below) will be developed and signed/dated by both the Rotation Supervisor and the student, and then submitted to the course coordinator ([email protected]). For students working with a Rotation Supervisor from outside Edmonton, email notification of approval of the proposal is acceptable. The proposal will then be reviewed and approved by the course coordinator. If the course coordinator has comments regarding the proposal, it will be sent back to the student for revision prior to final approval. The approved proposal will serve as the contract for PHARM 455 and form the basis for the student’s evaluation. The student needs to create their assessment forms (both midpoint and final) based on their proposal. Each objective needs to be assessed. Once approved, the course coordinator will enrol the student in the course. The proposal must also include the following elements at the beginning of the document:

1. Student name, email and ID number 2. Location of rotation and area of specialty 3. Name and contact information (including email) of rotation supervisor 4. Start and end date of rotation and when the time requirement will be fulfilled

The initial proposal for PHARM 455 is due 2 months prior to the start of the term. A final proposal is due 1 month prior to the start of the term. (see Class Schedule section). Please submit the proposal in Word format so that the Course Coorinator can provide comments within the proposal itself.

**NOTE – practice site requirements: For students in the Class of 2014 completing Pharm 455 rotations in AHS/Covenant facilities, please be aware that a police information check/security clearance will be required which is dated within 90 days of starting the rotation. AHS/Covenant also requires a record of your immunizations and N95 Mask fit test certificate. If you are in the Class of 2015, the faculty will be collecting and filing this information, and you will not need to take these documents to the site itself. Additionally, Netcare access will need to be arranged in advance of the rotation if required (see next section).

Netcare Access (if required) If your rotation is in a clinical setting where you will be providing patient care, you will require Netcare access. Initial set-up will be facilitated by the course coordinator who will send your name and email address to the Faculty Liaison Netcare Coordinator, who will then email you with the relevant forms and instructions that need to be completed by you and the Netcare Access Administrator at the practice site. This process needs to be started 6 weeks prior to your rotation starting in order to have Netcare access active when you arrive at your practice site. Please work with your rotation supervisor to identify the Netcare Access Administrator for their practice site, and complete the required forms/process in a timely manner.

Course Objectives Course objectives will be developed by the student and approved by the Rotation Supervisor.

Tips on Creating Learning Objectives for your Proposal:

1. The AFPC Educational Outcomes document may be used as a guide and are located at: http://www.afpc.info/downloads/1/AFPC_Education_Outcomes_AGM_June_2010.pdf

2. Learning objectives must include professionalism, and this will be evaluated as outlined in Appendix 4.

3. Learning objectives need to be “action-oriented” and include what you will do, not just what you will observe or discuss. As example of a learning objective is: Effectively communicate through documentation.

4. They should be concise and focus on the main skills and knowledge you would like to develop.

5. The main knowledge areas to be developed should be specified. The student will also be required to provide an oral presentation at the end of the course detailing what they learned and accomplished during the course. This is arranged by the course coordinator and is scheduled at the end of the term. It is expected that you will attend the ½ day arranged for this event so that you can present and also view your classmates’ presentations.

Assignments and Portfolio Assignments will be determined by the student and their Rotation Supervisor. A portfolio is required to be submitted to the course coordinator. The portfolio contents are:

1. Rotation Supervisor Assessment of Student Form (template included in Appendix 3 which student must customize based on their proposal; please create both a midpoint and final using the Word template of the evaluation which is posted on the website). This must include a Rotation Supervisor Assessment of Professionalism (Appendix 4). [NOTE: A midpoint evaluation form of preceptor is also included for student use. This does not need to be submitted with the portfolio but is intended for use during the rotation.]

2. Written description of what activities were completed 3. Artifacts of any projects completed 4. Written reflection essay (maximum 500 words) describing what was learned during the

rotation. 5. Final Student Evaluation of Rotation Supervisor and Site

The portfolio is to be submitted to the course coordinator by the student on the day they complete their term-end presentation.

Evaluations The course is Credit/No-Credit.

An evaluation form will be created based on the learning objectives pre-determined by the student (see template, Appendix 3), and approved by the Rotation Supervisor (proposal). The student will be evaluated on each of these components, and must receive a minimum grade of “Satisfactory Progress” for each objective. The student will also be evaluated on Professionalism (Appendix 4) and must receive a grade of “Acceptable” in all areas.

Conditions for pass:

1. Professionalism (Mandatory): A mark for professionalism will be assigned by the Rotation Supervisor. Professionalism will be assessed for 4 areas as outlined in Appendix 4. They are: (1) ethical behaviour, (2) appearance and attire, (3) timeliness and commitment and (4) initiative. This must be completed by your preceptor at the midpoint and final evaluation, and by final evaluation, must have an Acceptable rating in each area.

2. Rotation Deliverables (Mandatory): Deliverables (based on proposal) must be completed to

the satisfaction of your Rotation Supervisor. You and your Rotation Supervisor will need to determine what evidence needs to be submitted for review and successful completion. In order for the course coordinator to evaluate that objectives are completed in an acceptable manner, the Rotation Supervisor Assessment of Student form (template in Appendix 3) must be formatted by you, completed by your Rotation Supervisor and submitted to the course coordinator. An acceptable mark (Satisfactory Progress or Achieved) must be received in each area in order to be awarded a pass for the course. The Professionalism Performance Evaluation (Appendix 4) must also be completed by your Rotation Supervisor. Both these evaluations need to be included in your portfolio.

3. Completion of Final Student’s Evaluation of Preceptor and Site form (Appendix 5).

4. Presentation*: An oral presentation must be completed at the end of the term on date that is

pre-determined by the course coordinator (note: Spring term students will provide their presentation at the end of Summer term). The course coordinator or designate will evaluate the oral presentation based on the rubric in Appendix 6.

* Presentation format: 20 minutes long, with 5 minutes for questions. It should include the following elements: an overview of your rotation objectives that you determined prior to starting the rotation, an overview of the patient care area where you provided care and your role, and an outline of any projects you completed as part of the rotation. If you completed a project, you should describe the project, what steps you took for completing it, and the results of the project (what did the final product from the project look like, if applicable). Finally, you should include a reflection, which should highlight what you learned, areas of strength you developed from the rotation, areas for continued growth clinically, and your opinion of the overall experience. Optional suggestion: outline a case where you provided care, and how your involvement helped you meet your patient care course objectives.

Textbooks There are no assigned textbooks for this course; however, the AFPC Educational Outcomes will be a useful resource for creating learning objectives for the proposal. Additionally, for patient care rotations, the Patient Care Process document will provide appropriate guidance. Information for Rotation Supervisors Pharm 455 is a specialty elective that allows students to gain experience in a specialty practice setting and create a course proposal that allows them to tailor the learning to their interests. Students desire rotation supervisors who can guide their learning and integrate them into their practice. Rotation supervisors are encouraged to get to know students who approach them for this elective course so that you can determine their interests, and your ability to meet their learning needs. Of note, this course can be taken on a part-time or full-time basis within a term depending on the mutally agreeable schedule of the rotation supervisor and student. Students cannot be paid while participating in this course. Rotation supervisors have the following responsibilities:

1. If new to supervising this elective course, review the course syllabus including Appendix 1 entitled “Preceptor qualifications, requirements and responsibilities” to ensure you can meet the requirements. Please contact the course coordinator to discuss any questions you may have if you are considering participation in the course.

2. If you are interested in participating in the course, in conjunction with the student, determine dates and hours for the rotation. Ensure you choose dates that work for you and your practice site.

3. The student will confirm with us that you can supervise the course, and for new preceptors, we will be in contact with you to set you up in our database. Existing preceptors may also have to complete a Profile Form to update our database. We will let you know which forms need to be completed.

4. If not already done, complete the Preceptor and Site Enrolment/Profile Form, which can be found on our website at: http://www.pharm.ualberta.ca/preceptors/how-to-become-a-preceptor. Please submit this form to Alix Peterka ([email protected]). If you have already completed this, and not changed practice sites since last completed, this does not need to be re-submitted.

5. Complete preceptor training as required. Requirements for preceptor training are outlined in the Preceptor and Site Enrolment/Profile Form. Also review the Preceptor Guide located at: http://www.pharm.ualberta.ca/preceptors/training-and-resources/preceptor-guide. This has lots of helpful information regarding our program and precepting.

6. Review the student’s learning objectives as presented in their proposal and make any suggestions required. The proposal needs to be signed by the Rotation Supervisor, either in person, or by e-mailing the course coordinator agreeing to the learning objectives set forth. [NOTE: The course coordinator will also review the proposal and provide feedback (if required) after you have reviewed.]

7. IT/access requirements: Determine requirements for having the student at your practice site and convey these to the student. For example, if Netcare access is required, this will need to be arranged 4-6 weeks in advance of the rotation by the student, with your help. Students will have access to, and can complete a Netcare registration form, which will need to be signed by your site's Netcare authorizer or Access Administrator, and submitted to the

appropriate help desk for processing. The student will need your direction as to who or where to direct their form. Students are then expected to confirm that their form has been received by the site authorizer/Access Administrator. Any questions or issues concerning Netcare Portal access can be directed to: Netcare Enrollment and Information line at 1-866-756-2647 or 780-642-4082 (Edmonton), Mon - Fri 8:30 am to 4:30 pm.

8. Prepare for your student! Review the orientation checklist in the Preceptor Guide, Appendix B, located at: http://www.pharm.ualberta.ca/preceptors/training-and-resources/preceptor-guide. If there are pre-readings you would like your student to review, direct them to these in advance of the rotation.

9. Review the Preceptor Faculty Appointments document located at: http://www.pharm.ualberta.ca/preceptors/preceptor-faculty and please apply for one of the designations.

10. Contact the Course Coordinator (Ann Thompson, [email protected]) if any concerns or questions arise before, during or after the rotation. Midpoint assessments of the student do not need to be submitted to the course coordinator, however, if concerns are present, please contact the course coordinator at any time to discuss.

CHAPTER 2: COURSE POLICIES & PROCEDURES

Faculty You are strongly encouraged to contact the coordinator as soon as difficulties or concerns arise regarding the course. This will greatly assist in providing you with a timely resolution to specific problems and preventing further difficulties.

E-mail Communication When e-mailing a lecturer or a course coordinator it is necessary to include – PHARM 455 in the subject line. This will ensure that your email is not deleted by the screening process. Students who fail to identify themselves will not receive a response. Please use your University of Alberta email address.

Employment/Payment Students may not receive any form of payment for their experiences pertaining to this elective.

Your Responsibility It is your responsibility to work professionally with your Rotation Supervisor to work toward successful completion of the specialty elective based on the proposal created before being enrolled in the course. It is also your responsibility to contact the course coordinator immediately if you are having difficulty with the course or if you have any concerns. You are expected to act in a professional manner, for example:

Acting professionally and adhering to site policies while at the practice setting

Providing and accepting constructive feedback

Being accountable for your words and actions

Taking an active role in your learning

Immunizations Students require an adequate immunization status before registering in the course. Please obtain a copy of your immunization record to take with you to your rotation site as required. Failure to comply may result in cancellation of your rotation.

Attendance Attendance is mandatory for credit to be received. In the case of illness the student should notify the clinical instructor at the site as soon as possible. When more than one day is missed for illness, arrangements must be made to make up the missed time.

Specialized Support and Disability Services (SSDS) Students registered with Specialized Support and Disability Services (SSDS) who will be requiring accommodations are required to provide a "Letter of Introduction" to the course coordinator prior to course enrolment.

Recording Course Materials Audio or video recording of lectures, labs, seminars or any other teaching environment by students is allowed only with the prior written consent of the Rotation Supervisor or as a part of an approved accommodation plan. Recorded material is to be used solely for personal study, and is not to be used or distributed for any other purpose without prior written consent from the Rotation Supervisor.

Appendix 1: Pharm 455: Rotation Supervisor Requirements, Qualifications and

Responsibilities

Experiential education provides students with an opportunity to integrate classroom knowledge, scientific evidence, and ethical judgment through preceptor guidance and expertise. A preceptor is a practitioner who teaches, coaches and supports students to develop, apply, and integrate practical knowledge and skills while caring for patients. Qualifications

Rotation Supervisors must be integrated into the practice environment and practicing at a level of proficiency than enables them to take on the course requirements

Motivated and enthusiastic about teaching

Role model for students demonstrated by having an ethical and patient focused professional practice

Factors identified as assets for Rotation Supervisors taking on this role include:

Advanced scope of practice; which may include additional prescribing authority, authorization to give injections, and collaborative practice environment with both patients and other health care providers

An innovative practice setting

Previous precepting or teaching experience

Demonstrated leadership in the profession

Advanced training or certification such as an Accredited Residency, PharmD, or Certified Diabetes Educator

Requirements The Rotation Supervisor must:

Be licensed to practice in his/her province and must be a member in good standing with the licensing body

Disclose any relationship (familial, personal, contractual or financial) that may create a conflict of interest when developing a rotation or assessing a student

Complete foundational preceptor training Examples of foundational preceptor training include:

Responsibilities Prior to the rotation (after confirmation of approval to be the Rotation Supervisor)

Complete preceptor training as required. If the preceptor is new, he/she will need to complete a Preceptor and Site Enrolment Form located at: http://www.pharm.ualberta.ca/preceptors/how-to-become-a-preceptor. Please send this completed form to Alix Peterka ([email protected]), our Administrative office coordinator.

Preceptors are eligible for 2 different faculty appointments. These are outlined on the website at: http://www.pharm.ualberta.ca/preceptors/preceptor-faculty. We encourage preceptors to let us know what faculty appointment they are eligible for and to take advantage of the benefits available.

Review the course proposal/learning objectives that the student creates for feedback and approval prior to having the Course Coordinator review.

Develop a schedule and plan activities that meet the student-driven objectives of the course. During the rotation

Complete the student’s orientation – see Preceptor Guide for guidance

Communicate performance expectations clearly to the student at the beginning of the rotation and when introducing a new activity

Encourage self directed learning

Meet with the student at regular intervals to discuss the rotation activities

Provide timely positive and constructive feedback verbally or in writing

Follow the evaluation procedures and contact the course coordinator if any issues arise during the rotation

Provide adequate supervision and adjust the level of supervision required based on the student’s performance and the time the rotation (which may be before or after their mandatory 16-week rotation in year 4 of the curriculum).

After the rotation

Attend the student’s presentation at the Faculty about their rotation experience. (optional – subject to preceptor availability)

Debrief with the course coordinator as required.

The professionalism assessment is positioned first in all Experiential Education courses at the Faculty of Pharmacy Pharmaceutical Sciences UofA, to promote its significance. The Faculty of Pharmacy and Pharmaceutical Sciences has ZERO tolerance for unprofessional behaviours such as:

referring to oneself or holding oneself to be more qualified than one is

fails to call/notify preceptor when unable to meet deadlines, complete tasks or arrive on time

breaching confidentiality

violating the Criminal Code

demonstrating infractions of the Human Rights Code

falsifying records

conducting sexual impropriety with a patient, caregiver, their families or colleagues

being impaired with an substance while participating in patient care or equivalent

Should any of the above behaviours occur, please contact the course coordinator immediately. The specific occurrence will be reviewed and appropriate action taken, which could include removal from the placement site. Minimally, there are 5 professionalism outcomes for you to assess with your student at both midpoint and final. Your student may include others with their learning objectives.
