Open Enrollment elections will be in effect from January 1, 2022 through December 31, 2022 UNLESS you have one of the following status change events:

• Changes to your benefit elections and documentation must be provided within30 DAYS of the event above. Changes must be consistent with the event.

Important Information:

• Newborns must be added to the benefit plans within 30 DAYS of birth, preferably as soon as possible. The verification of birth facts is acceptable.

Biometric Screening and Rally Health Survey

2022 Tasks for 2023 Plan YearCurrent employees and covered spouses:• Download and complete either the Quest lab form or physician form or• Participate in an on-site biometric screening at your campus, AND• Take online Rally Health Survey

Compliance Period:• January 1, 2022 – April 30, 2022 (six-month look-back period available)

Wellness Surcharge:Failure to complete the Biometric Screening and Rally Health Survey will result in a$25 per pay period wellness surcharge from January 1, 2023 – December 31, 2023.

Smart Steps

Rally Well-being Rewards ProgramRally is the state-of-the-art digital component of health and wellness offered by Optum. With Rally, employees are able to engage in their health, using digital capabilities that are personalized and fun – and can be accessed on the go. Rally integrates with Optum data and Optum wellness programs to provide a synchronized wellness experience for all participants.

As part of the Rally Well-being Rewards Program, employees are able to earn incentives by completing the Biometric Screening and Rally Health Survey requirements. In order to qualify for the incentive, you must complete the biometric screening AND Rally health survey.

1Please review the Disclosure Document or Outline of Coverage/Disclosure Document for information on which Covered Condition may be eligible for a Recurrence Benefit. There may be a Benefit Suspension Period between recurrences of the same Covered Condition, as well as occurrences of different Covered Conditions. There may be a limitation on the number of Recurrence Benefits payable per Covered Condition. We will not pay a benefit for a Covered Condition that is subject to a Benefit Suspension Period. This cancer insurance coverage provides a lump sum payment for:

Full benefit cancer – all forms of advanced cancers are covered and may qualify for full benefits (as defined by the group policy or certificate).

Partial benefit cancer – most forms of early-stage cancers are covered and may qualify for partial benefits (as defined by the group policy or certificate).

Opening a ClaimCall 1-800-GET-MET8 –or– go to https://mybenefits.metlife.comto request a claims form via email, fax or mail. To officially open the claim, you need to submit a fully completed claims form, including the physician statement.

Processing a ClaimWithin 3 days after receipt of a complete claims form including the physician statement, a Claims Acknowledgement Letter is mailed to you. An outbound call may be made to you to help collect any missing information.

PaymentWithin 10 business days after receipt of a complete claims form, a “clean” claim is fully processed* and payment is issued to you or the designated beneficiary.
