OUR MISSION

KPCA is charged with promoting the mutual interests of our members, with the mission to promote access to comprehensive, community-oriented primary health care services for the underserved.

KENTUCKY PRIMARY CARE ASSOCIATIONThe Kentucky Primary Care Association (KPCA) was founded in 1976 as a not-for-profit 501(c)(3) corporation of community health centers, rural health clinics, primary care centers and other organizations and individuals concerned about access to health care services for the Commonwealth’s underserved rural and urban populations.

The KPCA is charged with promoting the mutual interests of our members, with a mission to promote access to comprehensive, community-oriented primary health care services for the underserved. Association members are providers of primary care – first contact, broadly trained physicians, physician assistants, nurse practitioners, behavioral health providers, dental providers and other professionals delivering whole-person health care.

We support the development of primary care services throughout the Commonwealth by working with federal, state and local legislators, regulatory agencies, health policy organizations, foundations, and payers to build a stronger primary care system in Kentucky.

A MESSAGE FROM THE CEOWith the continued support of our Congressional Delegation and our State Partners, 2018 was a year of expansion of services, sites and programmatic efforts for the primary care safety network in Kentucky. An integrated approach to care in rural and inner-city underserved areas is happening as the communities and clinics, along with various partners, work to improve the health of the patients the clinics serve and the communities where they live. Kentucky Health Centers are in the forefront addressing the opioid crisis, maternal and child health and social disparities, among other things.

The Health Centers in Kentucky are growing with 356 sites and a substantial increase of the number of individual people served. As we move into 2019, we are addressing the changes in healthcare payment and modeling. The clinics and the Association are developing systems and programs to address the cost and quality of health care provided across the Commonwealth.

WHAT ARE HEALTH CENTERS?* Health centers are consumer-driven and patient-centered organizations that serve as a comprehensive and cost effective primary health care option for America’s most underserved communities. Health centers serve everyone regardless of ability to pay or insurance status. They increase access to health care and provide integrated care services based on the unique needs of the communities they serve.

Health centers deliver value to and have a significant impact on America’s healthcare system. They:

• serve as the health care home for more than 28 million patients in nearly 11,000 communities across the country;

• reduce health care costs and produce savings – on average, health centers save 24% per Medicaid patient when compared to other providers;

• integrate critical medical and social services such as oral health, mental health, substance abuse, case management, and translation, under one roof;

• employ nearly 190,000 people and generate more than $45 billion in total economic activity in some of the nation’s most distressed communities;

• serve on the front lines of public health crises such as the Zika virus and the opioid epidemic; and

• provide care to nearly 1.3 million homeless patients and more than 355,000 veterans.

Delivering High-Quality Primary Care Studies have found that the quality of care provided at health centers is equal to or greater than the quality of care provided elsewhere. Moreover, 99% of surveyed patients report they were satisfied with the care they receive at health centers.* Kentucky health centers’ effective management of chronic disease have led to improved health outcomes for their patients, as well as lowered the cost of treating patients with chronic diseases. The majority of the health centers in Kentucky are patient-centered medical home (PCMH) recognized through the National Committee for Quality Assurance (NCQA). PCMH is integral in building not simply a “home” for patients to receive care, but rather a model of the organization of primary care that delivers the core functions of primary health care. This model focuses on treating the whole patient by delivering quality, coordinated, comprehensive care, which is patient-centered, accessible and safe.

Investing in Kentucky’s Health Centers* Expanding and strengthening the health centers program would further reduce health disparities, increase access to high quality and regular care, and boost more local economies. As a result, fewer Kentuckians would rely on costly sources of care, such as the emergency room, thereby reducing the burden on taxpayers and making the overall health care system more efficient. Health centers are therefore good public investments that generate substantial benefits for patients, communities, insurers, and governments. Health center expansion that is coupled with expansions in insurance coverage and the primary care workforce would most effectively achieve improved health outcomes for the entire population.

G L O S S A R Y O F W O R D SClinic Location- (service Site) Locations where a health center either directly or through a subrecipient or contractual arrangement provides services and where all of the following conditions are met: Health center encounters are generated by documenting in the patients’ records face- to-face contacts between patients and providers; Providers exercise independent judgment in the provision of services to the patient; Services are provided directly by or on behalf of the health center, whose governing board retains control and authority over the provision of the services at the location; and Services are provided on a regularly scheduled basis

Delivery site- service delivery site identified in health center’s scope of project (grant or designation Form 5B)

Uniform Data System (UDS)- The UDS is a core set of information appropriate for reviewing the operation and performance of health centers. The UDS annually collects a variety of information, including patient demographics, services provided, staffing, clinical indicators, utilization rates, costs, and revenues. *All health center data from this book is sumitted by health centers PRELIMINARY 2018 UDS reports and subject to alter before publishing*.

Federal Investment- The Health Resources and Services Administration (HRSA), an agency of the U.S. Department of Health and Human Services, is the primary Federal agency for improving health care to people who are geographically isolated, economically or medically vulnerable. The health center program receives federal dollars form HRSA and stated on congressional district pages as “federal investment”

FPL- Federal poverty guidelines: An annual statement of the amount of income below which an individual or family of different sizes are considered to be in poverty

Health Centers- Community-based and patient-directed organizations that deliver comprehensive, culturally competent, high-quality primary health care services. In this document, unless otherwise noted, the term “health center” refers to organizations

that receive grants under the Health Center Program as authorized under section 330 of the Public Health Service Act and look-a like organizations that are recognized by Bureau Primary Health Care as meeting all the Health Center Program requirements.

Homeless Location- For the purposes of health centers receiving a Health Center Program award or designation under section 330(h) of the Public Health Service Act, the population served includes individuals: Who lack housing (without regard to whether the individual is a member of a family); Whose primary residence during the night is a supervised public or private facility that provides temporary living accommodations; Who reside in transitional housing; and/or Who reside in permanent supportive housing or other housing programs that are targeted to homeless populations.

Medicaid- Federal and state-run programs operating under the guidelines of Titles XIX and XXI (as appropriate) of the Social Security Act

Medicare- Federal insurance program for the aged, blind, and disabled (Title XVIII of the Social Security Act)

Private- patients with health insurance provided by commercial and not-for -profit companies (employer sponsored insurance, federal/state exchange purchased plans, insurance purchased for public employees or retirees such as Tricare, Trigon, or the Federal Employees Benefits Program)

School Based Location- A health center located on or near school grounds (including pre-school, kindergarten, and primary through secondary schools) that provides comprehensive preventive and primary health services

Total Jobs- Includes full time, part time and part year staff.

Agricultural worker patients- Report patients whose principal employment is in agriculture on a seasonal basis (e.g., picking fruit during the limited months of a picking season) but who do not establish a temporary home for purposes of employment. Seasonal agricultural workers are usually hired laborers who are paid piecework, hourly, or daily wages. Include patients who have been so employed within 24 months of their last visit and their family members who may be patients of the health center.

Babies delivered or prenatal care - (by encounter) includes prenatal care and deliveries, regardless of outcome, of women who were either part of or not part of the health center’s prenatal care program during the calendar year.

Behavioral- (by encounter) The term behavioral health is synonymous with prevention or treatment of mental health. Mental health services include psychiatric, psychological, psychosocial, or crisis intervention services.

Dental- (by encounter) visits performed by licensed dental provide or paid referral; reported one visit per patient per day, regardless of number of clinicians who provide services or the volume of service

Enabling- (by encounter) Enabling services include a wide range of services that support and assist primary care and facilitate patient access to care, including: case management, transportation, outreach, patient and community education, eligibility assistance, translation/interpretation services, community health workers, other.

Homeless patients- patients who lack housing (without regard to whether the individual is a member of a family) as homeless; patients whose primary residence during the night is a supervised public or private facility that provides temporary living accommodations and patients who reside in transitional housing or permanent supportive housing.

Medical- (by encounter) medical visits performed by any medical provider

School based patients- total number of patients who received primary health care services at the approved school service delivery site(s); patients served at in-scope school-based health centers located on or near school grounds, including pre-school, kindergarten, and primary through secondary schools, that provide on-site comprehensive preventive and primary health services; Services are targeted to the students at the school, but may also be provided to their children, siblings, or parents and may occasionally include persons residing in the immediate vicinity of the school.

Substance - (by encounter) total number patients, by encounter served for substance use disorder

Total patients - A person who has at least one reportable visit in one or more categories of services: medical, dental, mental health, substance use disorder, vision, other professional, and enabling

Veteran patients- total number of patients served who have been discharged from the uniformed services of the United States; only those who affirmatively indicate they are veterans.

Vision- (by encounter) visits performed by licensed vision service provider or paid referral
