Welcome to your 2017 Wellness Program! All Summit L.I.F.E. enrollees will have opportunities to participate in various wellness activities that can reward you with a premium discount in 2018. Your new Wellness Program details are outlined throughout this brochure.

Overview Complete Steps 1-3 by the applicable deadlines to be eligible for a premium discount in 2018.

STEP 1 – Complete the Know Your Number (KYN) Assessment starting January 1, 2017 through August 31, 2017 and Biometrics/Labs during the month of August 2017

STEP 2 – Attain the five (5) Healthy Lifestyle Targets through your labs and biometrics taken during the month of August 2017 OR complete the Alternative Standard Program, if applicable

STEP 3 – Complete the Physical Activity Requirement by November 30, 2017

Wellness Portal Your Wellworks For You account has been generated for you. Please follow the steps below to login to your Wellworks For You account. In accordance with HIPAA confidentiality laws, your individual data is accessible only to you and the third party vendor, Wellworks For You.

LOGIN TO YOUR WELLNESS PORTAL ACCOUNT 1. Go to www.wellworksforyoulogin.com 2. Enter your Username: The username will be your company email

address ([email protected]) 3. Enter your Password: The temporary default will be your date of

birth in YYYYMMDD format to include all leading zeros (19750125) 4. Accept the terms of the Consent Form 5. Enter the required Personal Information

Complete the KYN Assessment with Biometrics and Labs, attain the Healthy Lifestyle Targets, or alternative standard, if applicable, and the Physical Activity minutes to be eligible for a premium discount beginning January 1, 2018.

PROGRAM STEPS

STEP 1 – KNOW YOUR NUMBER ASSESSMENT (KYN), BIOMETRICS, AND LABS All employees will receive credit for Step 1 by completing the KYN Assessment between January 1, 2017 and August 31, 2017, and having their biometrics taken and labs drawn at their site in August 2017. The health metric data from the onsite screenings (height, weight, waist circumference, blood pressure, and lab results) will be supplied from Summit Medical Group directly to Wellworks For You and will be uploaded into your KYN Assessment. The KYN Assessment is located on the Wellness Portal under My Health Assessments>Know Your Number Assessment.

If you see that your screening results are entered, complete all questions and click Finish to submit your responses.

If you see that your screening results are not yet entered, complete all other questions and click Save. The assessment will be completed once your metrics are received by Wellworks For You.

The Know Your Number Assessment, biometrics, and labs must be completed by August 31, 2017.

STEP 2 – ATTAIN THE HEALTHY LIFESTYLE TARGETS OR COMPLETE THE ALTERNATIVE STANDARD PROGRAM

Summit Medical Group has set five (5) Healthy Lifestyle Targets for their employees. They are: 1. Tobacco/Nicotine Free 2. LDL of <160 3. Blood Pressure Level of <140/90 4. Healthy weight with BMI less than 30 (but not less than 18.5) or waist circumference less than or equal

to 35 for a woman and 40 for a man 5. A1C level of less than 5.7 or less than 7.0 for diabetics under diabetic management

These targets have been set as a standard for all employees and will be calculated from results of the onsite screenings. The screenings are free to employees and will be held during the month of August. More details about the screenings and labs will be communicated via email at a later date. If you fall outside the Healthy Lifestyle Targets, an Alternative Standard Program will be offered to you. This will satisfy Step 2 of the 2017 Wellness Program steps. If you do not meet the Healthy Lifestyle Targets, you will receive an email from Wellworks For You with more instructions on how to complete the Alternative Standard Program in September 2017.

STEP 3 – TRACK YOUR PHYSICAL ACTIVITY You should complete an average of 100 minutes of exercise per week, for a total of 4,800 minutes by November 30, 2017. You can track your gym visits, fitness classes, and any other physical activity by syncing your device or app on the Wellness Portal under Devices/Apps. Follow the instructions to link your device or tracking app. If you do not own a tracking device or app, you can manually track your physical activity on the Wellness Portal under Fitness and Nutrition>WellBalance>Track Steps. More instructions on how to sync your device to the Wellness Portal and how to manually track your physical activity on the website are located on the Portal under My Profile>Programs and Events.

Incentives Complete Steps 1, 2, and 3 and the Alternative Standard Program, if applicable, to be eligible to receive the health insurance discount effective January 1, 2018. Please note: If you do not complete Steps 1 and 2 by August 31, 2017, and Step 3 and/or the Alternative Standard Program by November 30, 2017, you will not be eligible for the January 1, 2018 effective date for the premium discount.

If you complete the Alternative Standard Program after November 30, 2017, you will be refunded back to January 1, 2018 the month following completion. The Alternative Standard Program will be open until December 31, 2018.

All forms are located on the Wellness Portal under My Profile>Wellness Toolbox. Participants can download and print all PDF forms for completion.

VIEW YOUR PARTICIPATION Wondering if you have completed the steps for your Wellness Program?

DEVICE AND APP TECHNOLOGY Our Wellness Portal is compatible with pedometers and tracking systems including all Fitbit Pedometers, Garmin, Jawbone, Misfit, My Fitness Pal, and more! Simply sync your device on the Wellness Portal by selecting Device/App Connect. As a wellness partner, Summit Medical Group employees are eligible to receive a discount on the purchase of pedometers. Simply print the Pedometer Order Form located in your Wellness Toolbox to order your device today!

SMARTPHONE APPLICATION The Wellworks For You Portal App includes all of your favorite features from the Portal including Weekly Recipes and Grocery Lists, the ResultsNow incentive tracker, Wellness Challenges Dashboard, Pedometer Tracking, and more! Simply search Wellworks For You in the App or Google Play store to download our free app!

NOTICE FOR EMPLOYER-SPONSORED WELLNESS PROGRAMS Summit L.I.F.E is a voluntary wellness program available to all employees. The program is administered according to federal rules permitting employer-sponsored wellness programs that seek to improve employee health or prevent disease, including the Americans with Disabilities Act of 1990, the Genetic Information Nondiscrimination Act of 2008, and the Health Insurance Portability and Accountability Act, as applicable, among others. If you choose to participate in the wellness program you will be asked to complete a voluntary health risk assessment or "Know Youre Number -KYN" that asks a series of questions about your health-related activities and behaviors and whether you have or had certain medical conditions (e.g., cancer, diabetes, or heart disease). You will also be asked to complete a biometric screening, which will include a blood test for Cholesterol, LDL, HDL, Glucose, and Triglycerides, A1C and GGT, as well as attain the Healthy Lifestyle Targets and meet the Physical Activity requirement to complete your enrollment.

Employees who choose to participate in the wellness program and meet all of the requirements will receive an incentive of a premium discount for health insurance in 2018. Only employees who complete all of the enrollment requirements will receive the premium discount.

If you are unable to participate in any of the health-related activities or achieve any of the health outcomes required to earn an incentive, you may be entitled to a reasonable accommodation or an alternative standard. You may request a reasonable accommodation or an alternative standard by contacting Wellworks For You at 800-425-4657.

The information from your KYN and the results from your biometric screening will be used to provide you with information to help you understand your current health and potential risks, and may also be used to offer you services through the wellness program, such as Wellness Challenges. You also are encouraged to share your results or concerns with your own doctor.

PROTECTIONS FROM DISCLOSURE OF MEDICAL INFORMATION

We are required by law to maintain the privacy and security of your personally identifiable health information. Although the wellness program and Summit L.I.F.E may use aggregate information it collects to design a program based on identified health risks in the workplace, Wellworks For You will never disclose any of your personal information either publicly or to the employer, except as necessary to respond to a request from you for a reasonable accommodation needed to participate in the wellness program, or as expressly permitted by law. Medical information that personally identifies you that is provided in connection with the wellness program will not be provided to your supervisors or managers and may never be used to make decisions regarding your employment.

Your health information will not be sold, exchanged, transferred, or otherwise disclosed except to the extent permitted by law to carry out specific activities related to the wellness program, and you will not be asked or required to waive the confidentiality of your health information as a condition of participating in the wellness program or receiving an incentive. Anyone who receives your information for purposes of providing you services as part of the wellness program will abide by the same confidentiality requirements. The only individual(s) who will receive your personally identifiable health information are the Wellworks For You team and Health Coaches in order to provide you with services under the wellness program The authorization and release of information you signed at the time your HRA was performed allows Summit to receive and review certain information. This is to inform you that clinical information from participants is received and reviewed by Summit as part of the HRA process and will be used solely for administration of the program.

In addition, all medical information obtained through the wellness program will be maintained separate from your personnel records, information stored electronically will be encrypted, and no information you provide as part of the wellness program will be used in making any employment decision. Appropriate precautions will be taken to avoid any data breach, and in the event a data breach occurs involving information you provide in connection with the wellness program, we will notify you immediately.

You may not be discriminated against in employment because of the medical information you provide as part of participating in the wellness program, nor may you be subjected to retaliation if you choose not to participate.
