Michael Diaz, MD President, Florida Society of Clinical Oncology Executive Board and Director of Patient Advocacy, Florida Cancer Specialists Board of Directors, Community Oncology Alliance (COA) BIOGRAPHY:

Michael Diaz, MD joined Florida Cancer Specialists & Research Institute (FCS) in 2011 and currently serves on the FCS Executive Board and as Director of Patient Advocacy for the practice. Dr. Diaz also provides leadership on the Board of Directors for Community Oncology Alliance (COA), serving on the Payment Reform Task Force working to champion Medical Oncology Health Care Reform. In addition, he serves on the Board of Directors and as President for the Florida Society of Clinical Oncology (FLASCO). He is active on the American Society of Clinical Oncology (ASCO) Payment Reform Work Group and was recently appointed to ASCO's Clinical Practice Committee.

Dr. Diaz donates many hours as an active Medical Missionary and has participated in numerous medical missions to Kingston, Jamaica.

Throughout the year FLASCO offers the opportunity for FLASCO member Oncology Focused Trainees to submit applications for Travel Awards. Thank you for your desire to learn! Susmitha Apuri, MD Moffitt Cancer Center

BIOGRAPHY: Dr. Apuri is a second year hematology/oncology fellow at University of South Florida/Moffitt cancer center. She attended medical school at N.T.R University of Health Sciences, Vijayawada, India and completed her internal medicine residency at University of Miami regional campus in West Palm Beach. She worked as an assistant professor/hospitalist at

Moffitt Cancer Center prior to pursuing fellowship in hematology/oncology.

Jennifer Crozier, MD Mayo Clinic Florida

BIOGRAPHY: Jennifer A. Crozier, MD, is a compassionate physician who is focused on providing patients with high-quality care. Incorporating her background in research along with clear communication, Dr. Crozier educates her patients on their diagnosis and individualized care

plans by utilizing the latest therapies and clinical trials. She works with a team of multispecialty health care providers in order to provide her patients with high-quality comprehensive care. Her goal is to educate each patient about their disease and answer any questions that may arise during such a life-changing diagnosis as cancer. She has a strong internationally recognized research background and stays up-to-date on the latest cancer research so she can utilize the best novel therapies and clinical trials to provide treatment options. Her areas of expertise include breast cancer and clinical trials.

Bhagirathbhai Dholaria, MBBS Mayo Clinic Florida

BIOGRAPHY: Bhagirathbhai (Bhagi) Dholaria, MBBS is a final year fellow- hematology & oncology, at Mayo Clinic, Jacksonville, FL. He is originally from a small costal town in Western India. He graduated from B. J. Medical College, Ahmedabad, India in 2009 and finished his

Internal Medicine residency from University of Arkansas for Medical Sciences (UAMS) in Little Rock, AR in 2014 and joined Mayo Clinic for his hematology & oncology fellowship. He intends to pursue a carrier in malignant hematology.

Sharon Robertson, MD Moffitt Cancer Center

BIOGRAPHY: Dr. Robertson is a PGY 6 in the Gynecologic Oncology program at the University of South Florida, College of Health. She is a married mother who spent 6 years in Portland, Oregon, before moving to Florida to complete her Oncology Fellowship.

Erica Takimoto, DO Florida Hospital Cancer Center

STATEMENT: My name is Erica Takimoto and I was the fortunate recipient of the FLASCO travel award in the Spring 2016 prior to completing my fellowship at Florida Hospital in Gynecologic Oncology in July of 2016. The funds provided by the FLASCO travel award

allowed me to attend our National Society of Gynecologic Oncology Annual meeting as well as the Business of Oncology Summit at Gaylord Palms. This afforded me the opportunity to present my research poster at the national SGO meeting, as well as network and connect with our oncology colleagues throughout the state at the FLASCO meeting, where I learned a great deal regarding the business side of oncology. The presentations outlined current changes in business practice, and also what we can expect to be implemented with our ongoing healthcare climate. At the completion of the session, I felt better prepared for the changes to come. There was a wide variety of exhibitors at each Expo center who were excellent resources for information on everything from new drugs and symptom management, to patient clinical registries, better equipping up to take the best possible care of our patients. I am thankful for the opportunity presented by FLASCO, and I am happy to see the ongoing support for young oncology fellows from the state of Florida.

Kamran Ahmed, MD Moffitt Cancer Center

BIOGRAPHY: Kamran Ahmed is a PGY-V radiation oncology resident at Moffitt Cancer Center. He is originally from Houston, TX and then went on to attend Mayo Medical School. After completing his training at Mayo, he went on to complete an internship at Baylor

College of Medicine before beginning his training at Moffitt. He has published numerous peer-reviewed publications during residency with a number of oral presentations at national meetings.

Sarah Cevallos, MBA Florida Cancer Specialists

BIOGRAPHY: As Chief Revenue Cycle Officer, Sarah Cevallos is responsible for providing strategic leadership and direction to all Revenue Cycle responsibilities for the Florida Cancer Specialists' physician network. In this role, she supports FCS through excellence in

patient financial services, billing, timely collection, and precise payment posting, while delivering a positive patient experience. Sarah is also responsible for developing our payer partnerships. Since joining FCS in 2008, she has held several finance-related positions of increasing responsibility. Sarah holds a Bachelor of Science degree in Business Management from Nova Southeastern University and an MBA degree from Western Governors University with a concentration in Healthcare Management. In addition to her work with FCS, Sarah serves as Clinical Practice Committee Member for the Florida Association of Clinical Oncology (FLASCO).

ASCO MACRA Training Margarita Wiegandt, MBA Memorial Healthcare Systems

BIOGRAPHY: Maggie Wiegandt has been a healthcare administrator for the past 13 years. In her current role as Vice President of Oncology Services, she is responsible for providing programmatic, operational and strategic leadership to further the growth, improved

outcomes and integration of cancer services within the Memorial Healthcare System in Broward County, the 2nd largest public healthcare system in the nation.

Maggie’s community commitments include Gilda’s Club of South Florida Executive Committee Board Member, Women’s Breast and Heart Initiative Board Member, American Cancer Society Board Member, Pink Angels of the Memorial Foundation Executive Committee Member, and Past Chair of the Board for Miramar/Pembroke Pines Chamber of Commerce. Awards include 1,000 Plus Woman of the Year, Hispanic Woman of Distinction and South Florida Business Journal Up and Comers.

Maggie earned her Bachelor of Science Florida State University and a combined Master of Healthcare Administration and Master in Business Administration from the University of Houston.

Aixa E. Soyano Muller, MD Mayo Clinic Florida

BIOGRAPHY: Dr. Soyano Muller was born and raised in Caracas, Venezuela in a medically focused family. Both her parents and older brother are physicians, thus she has been interested

in medicine since she was very young. She started her medical education in October 2002 by attending the Luis Razetti School of Medicine at Universidad Central de Venezuela. During Medical school she did a 3-month away rotation in the indigenous community of Wonken, near the Amazon jungle, helping the most needed communities. After graduation she worked as a rural physician as well as a physician assistant in a private hematology practice in Caracas. All these experiences have been enriching and have given her the opportunity to learn to adapt to different environments.

In 2012 she got married and came to the US to start her Internal Medicine Residency Program at St. Elizabeth’s Medical Center in Boston, MA. She graduated in 2015 and moved to Jacksonville to start her Hem/Onc fellowship in July 2015 at Mayo Clinic Florida. In September 2015, she was blessed with having a beautiful baby boy. This last year has been exciting and challenging. She has given love and devotion to her family while at the same time she has been enthusiastic and dedicated to learning and to giving the best medical and humane experience possible to her patients.

Amrita Desai, MD, MPH University of Miami

BIOGRAPHY: Dr. Amrita Desai is currently a third year fellow in the research track in the Department of Hematology/Oncology at the Sylvester Comprehensive Cancer Center, University of Miami. She has a background in Clinical Pharmacology from India and an MPH from Harvard T. H. Chan School of Public Health. Her research is focused in understating the

immunological aspects of hematopoietic stem cell transplant and identifies ways to improve the GVL effects while ameliorating GVHD. Her long long-term career goal is to be a translational researcher in the field of hematopoietic stem cell transplantation.

ASH continued Andrew Kuykendall, MD Moffitt Cancer Center

BIOGRAPHY: Andrew Kuykendall grew up in Tampa, Florida and completed his undergraduate training with a major in economics at Wake Forest University in Winston-Salem, North Carolina. He returned to the state to obtain his medical degree from the University of

Florida in Gainesville followed by a residency in Internal Medicine at the University of South Florida in Tampa. Currently, he is a second year Hematology and Oncology fellow at USF at Moffitt Cancer Center. Dr. Kuykendall is actively involved in research focusing on myeloid diseases, specifically myelofibrosis and other myeloproliferative neoplasms. Throughout his training, he has been actively involved in organized medicine and has served in local leadership roles as well as on national committees for the Florida Medical Association, American Medical Association, and American College of Physicians.

Arjun Mittra, MBBS Mayo Clinic Florida

BIOGRAPHY: Arjun Mittra, MD is currently a fellow in Hematology/ Oncology at the Mayo Clinic in Jacksonville, Florida. After completing medical school in Mumbai, India, Dr. Mittra

pursued postdoctoral fellowships in gastrointestinal and breast cancers at Memorial Sloan Kettering Cancer Center and the University of Southern California. He then trained in Internal Medicine at Presbyterian Hospital Dallas before joining the Mayo Clinic.

In an effort to increase engagement among physicians,

clinicians and administration, the FLASCO Board of

Cancer Center is a state-of-the-art facility in Broward County (South Florida) that provides their

physicians with access to national resources to treat all types of cancer, including but not limited to breast

cancer, colon cancer, lung cancer, pancreatic cancer and prostate cancer.

Suncoast Cancer Institute is an oncology and hematology practice in Sarasota, Florida,

specializing in the diagnosis and treatment of cancer and blood disorders. Their mission is to improve the

lives of those in their community that are impacted by cancer and blood diseases by providing evidence

based, compassionate, personalized, state of the art care.

Sylvester Comprehensive Cancer Center, University of Miami is a

university-based cancer center. Sylvester physicians and scientists apply research breakthroughs from the

laboratory to the patient’s bedside. Sylvester closely evaluates the effectiveness of new treatments, and

shares clinical insights with laboratory investigators, leading to improvements that benefit patient care.

This type of translational research is the cornerstone of our comprehensive cancer center, providing us the

ability to accurately treat a wide range of cancers across 15 site diseases and from stage one to stage four.

Sylvester patients also have the opportunity to participate in groundbreaking clinical trials that offer new

treatment approaches and help the scientific community answer questions about new ways of using

BIOGRAPHY: Having entertained over a million people live and on television, Diana Jordan came to realize that by making people laugh, she helped them deal with life's stresses, motivated them, and taught them what she’d learned over the past 25 years about how great laughter is in nourishing yourself and changing your outlook on life.

Diana has toured the country many times over as an extremely successful stand up comedian and successful author. She is also a cancer survivor and speaks of her journey to many cancer organizations and events nationwide. In Diana's Keynote

address, motivational messages ring loud and true about how we can make the choice to be happy, find the funny in your life, taking responsibility for your own health, ways to bust stress and in doing just these four things can make a happier, healthier, and more productive you! And it's funny too!

Diana has been nominated by the American Comedy Awards as one of the Top Five Female Comedians in the country. She's been seen on Showtime, Showtime Specials, HBO, ComedyCentral, VH1, and many more. Diana has also used her talent as a writer in a hilarious best selling book which was featured on Oprah called, "A Wife's Little Instruction Book, Your Survival Guide To Marriage Without Bloodshed!" which has sold over 500,000 copies. Motivational speaking was a natural transition for Diana. She realized her gift and talent for comedy was much needed around the country. For the last two decades Diana has also headlined America's top comedy clubs and entertained our troops overseas. For over a year she performed her widely popular one-woman-show, "Female, Fertile, and Frustrated!" which she Wrote, Produced, Directed, and Starred in at the world famous Ice House Comedy Club in Pasadena, California . "Female, Fertile, and Frustrated!" is still the longest running one person show in the forty year history of The Ice House. No longer FERTILE, Diana makes light of getting older and using humor to cope with being a breast cancer survivor. 'When my Reconstruction Surgeon ask me if I wanted to go up a cup size or two I told him, 'let me put it this way, when I fly I want to be the flotation device!"

Diana studied Improv comedy many years in "Off the wall" classes with classmate Robin Williams (whom she also dated) and has been under contract to the CBS Network and Twentieth Century Fox for her own sit-com which she also wrote and a member of SAG, AFTRA, The National Speakers Association, and The Writers Guild of America. She has opened in concert around the country for major stars as well as at The Kennedy Center, Garden State Arts Center, and Radio City Music Hall. She has also been Master of Ceremonies for many gala events with such prestigious Speakers as Former Secretary of State, General Colin Powell and Yolanda King, daughter of Dr. Martin Luther King.

Diana can also be seen in her co-starring role in "Loretta's Suprise' which was at the Sundance Film Festival and Director Cameron Crowe hand picked her to be in the divorced women's group in the Oscar nominated movie 'Jerry Macguire' with Tom Cruise. You'll see her sitting next to Cruise during the now famous "You complete me" and "You had me at hello" scenes! Cruise says, "Diana is funny and smart, she 'had me at her first joke!"

A world traveler, Diana has lived in Greece, Hawaii, Chicago, Atlanta, Las Vegas, and even ‘starved’ while working as a waitress in New York to get a job on Broadway. She never gives up!

BIOGRAPHY: Luis E. Raez, MD, FACP, FCCP; is the Medical Director of Memorial Cancer Institute (MCI), he is also the Oncology Research Director of Memorial Health Care System (MHS) and Director of the Thoracic Oncology Program. As well as the Clinical Associate Professor of Medicine at Florida International University (FIU) and Visiting Professor of Medicine at Cayetano Heredia University in Peru. Formerly, Dr. Raez was an Associate Professor of Medicine, Epidemiology and Public Health, and a Co-Director of Thoracic Oncology at Sylvester Cancer Center/University of Miami for 10 years (2001-2011). Dr. Raez has expertise in medical oncology in the

areas of lung cancer, and head and neck cancer. Currently Dr. Raez designs phase I-III clinical trials with new chemotherapeutic agents and combinations. Dr. Raez does translational research in the areas of cancer vaccines and has been funded by NCI and the pharma industry. Dr. Raez has given oral presentations and lectures in national and international meetings in the US, Europe, Latinamerica and Asia. He is American Board Certified in: Internal Medicine, Medical Oncology and Geriatric Medicine. He is board elegible in Hematology. Dr. Raez is a member of AACR, ESMO, ASCO, IASLC, ALLIANCE, NCCTG, ACCP, ACP, ACSG, FLASCO among other institutions.

OBJECTIVES:

1. To gain knowledge of the circulating tumor cell (CTC) assays. 2. To acquire a basic understanding of the role of exosomes.

BIOGRAPHY:

Jhanelle E. Gray, MD, is a Medical Oncologist in the Department of Thoracic Oncology and the Director of Thoracic Clinical Research at H. Lee Moffitt Cancer Center and Research Institute in Tampa, Florida. She received her undergraduate degree from the University of Florida and her medical degree from Cornell University Medical College in New York City. Following her internship and residency in Internal Medicine at Cornell, Dr. Gray returned to Florida and completed her Medical Oncology Fellowship at Moffitt. She is a faculty member in the Department of Thoracic Oncology at Moffitt Cancer Center where she has been practicing since 2007.

Although Dr. Gray focuses her clinical attentions on lung cancer, she is also deeply involved in clinical research. She is the lead investigator on many thoracic clinical trials especially research investigating novel drug combinations and personalized treatment to advance the fight against lung cancer. As the Director of Thoracic Clinical Research in the Department of Thoracic Oncology, Dr. Gray directly supervises and manages 21 team members, (including 10 clinical research coordinators, 7 research data specialists, and 2 regulatory specialists, a clinical research manager and a clinical research navigator). Dr. Gray leads the restructuring and development of our clinical trial portfolio and assignment (which currently includes 25 clinical trials with active patients and 15 planned clinical trials currently undergoing regulatory and contracts review). In this role, she fosters the clinical trial development through intra- and extramural collaborations between our medical, surgical, and radiation oncologists, research scientists, pathologists, and epidemiologists in the Department of Thoracic Oncology. In addition to treating patients and serving as the lead investigator on many thoracic clinical trials, Dr. Gray is an active member of a number of committees at Moffitt, ranging from evaluation of minority accruals in clinical trials to integration of translational research in Phase I and Phase II clinical trials.

“The Role of Liquid Biopsy in the Diagnosis & Management of Lung Cancer”

OBJECTIVES:

1. Identify different technologies for liquid biopsy. 2. Understand the role of liquid biopsy in identifying molecular targets in lung cancer.

BIOGRAPHY:

Tarek Mekhail, MD, a surgeon turned thoracic oncologist, uses his unique background to improve the care he provides to patients at Florida Hospital. By reviewing each case from both a surgical and therapeutic perspective, Dr. Mekhail determines the best treatment approach for each patient’s individual condition. In fact, Dr. Mekhail embraces his role as both doctor and counselor, getting to know each patient personally to develop a lifelong relationship. Through this relationship, he speaks openly and directly about treatment options and life-long care plans.

After attending medical school at Cairo University, he began his medical career as a surgeon in the United Kingdom. Then, Dr. Mekhail completed his medical oncology fellowship at the Cleveland Clinic in Cleveland, Ohio. His transition to oncology took his focus from performing surgery to providing comprehensive care for patients, a duty he takes seriously. In fact, he’ll tell you that the most rewarding difference is seeing the impact his care makes in each patient’s life. Today, Dr. Mekhail leads the Thoracic Oncology Program at Florida Hospital Cancer Institute as medical director and works as an assistant professor at the University of Central Florida. He specializes in treating lung cancer (thoracic cancer), kidney cancer, head/neck cancer and aero-digestive malignancies. BIOGRAPHY: Edgardo S. Santos, MD, FACP, is a well-established thoracic surgeon with an impressive academic career and a remarkably diversified professional journey. Educated in Panama and the United States, he is currently performing at an outstanding level of quality in several key roles across Florida. Dr. Santos is the Medical Director of Cancer Research and Co-leader of the Thoracic and Head & Neck Cancer programs at The Eugene M. & Christine E. Lynn Cancer Institute of Boca Raton Regional Hospital in Boca Raton, Florida. Furthermore, he is a board certified oncologist with the Center for Hematology-Oncology in Boca Raton, Florida, and serves on staff of several local

hospitals, including Bethesda Hospital in Boynton Beach, Florida, and Delray Medical Center in Delray Beach, Florida. Established in 1995, The Center for Hematology-Oncology is on a mission to provide high quality, state-of-the-art, medical care to patients with cancer and blood disorders. Edgardo S. Santos, MD, commenced his foray in medicine in 1994 when he acquired his doctorate of medicine from the University of Panama School of Medicine in Panama City, Panama, his native country. He additionally completed his first year of internship at Complejo Hospitalario Metropolitano in Panama City, second year of his internship at Santiago Regional Hospital in Veraguas, Panama, and his internal medicine residency at Complejo Hospitalario Metropolitano. In 1998, Dr. Santos relocated to the United States where he served additional internal medicine internship and residency training at Jackson Memorial Medical Center in Miami, Florida. He also completed fellowship training in hematology and medical oncology at the University of Miami & Jackson Memorial Medical Center, Sylvester Comprehensive Cancer Center. Dr. Edgardo S. Santos is certified in internal medicine and holds sub-specialty certifications in hematology and medical oncology from the American Board of Internal Medicine. A distinguished member of the International Association for the Study of Lung Cancer, he is also a prolific author with many of his works published. Dr. Santos attributes his success to his unwavering dedication and hard work. Dr. Neil Abrahams is a board-certified doctor of clinical and anatomic pathology. He earned his medical degree from the Universidad de Salamanca Facultad de Medicina in Spain in 1995. He completed his residency at the Akron City Hospital/Summa Health System in Akron, Ohio and at The Cleveland Clinic Foundation in Cleveland in 2002. He completed his fellowship in genitourinary pathology at the University of Texas M.D. Anderson Cancer Center in 2003. Prior to joining Memorial, he served as assistant professor of pathology at the University of Nebraska Medical Center from 2003 to 2006.

BIOGRAPHY: Lillie D. Shockney has been the administrative director of the Johns Hopkins Breast Center since 1997. A two-time breast cancer survivor, Lillie has worked tirelessly to improve the care of breast cancer patients around the world. She is a registered nurse with a Bachelor of Science degree in health care administration from Saint Joseph's College and a Masters in Administrative Science from the Johns Hopkins University. She has worked at Johns Hopkins since 1983. She is also certified as a breast imaging and breast cancer patient navigator. Beginning the end of 2011, she accepted the inaugural role as director of the Johns Hopkins cancer survivorship programs for the Kimmel Cancer Center at Johns Hopkins.

Lillie is a published author and nationally recognized public speaker on the subject of breast cancer. She has written 14 books and more than 250 articles on this subject. She serves on the medical advisory board of several national breast cancer organizations and is the co-founder and vice president of a national non-profit organization called Mothers Supporting Daughters with Breast Cancer.

Lillie is also editor-in-chief of a new peer-reviewed medicine journal called Journal of Oncology Navigation and Survivorship, which launched in Spring 2011. She also serves as the chairman of the Leadership Council of the Academy of Oncology Nurse Navigators (AONN), a national organization that provides a network for all professionals involved and interested in patient navigation and survivorship care services to better manage the complexities of the cancer care treatment continuum for their patients. This is a professional organization she co-founded in 2009. Beginning in July 2012, she accepted the leadership role of Program Director of AONN.

She is the consultant for breast cancer for national ABC News and Good Morning America, and is also consulted regularly by the Today Show and CNN. She is an active lecturer on various topics related to breast cancer and spoke at the 2010 World Economic Forum, which will featured her lecture, “The Cancer Paradigm Shift: Who Will Take Care of the Breast Cancer Survivors?”.

Lillie serves as “Ask an Expert” for several breast cancer websites, including the Johns Hopkins Breast Center, among others. She also is an active clinical researcher with a focus on quality of life issues for survivors, as well as a strong advocate of the value of humor as a beneficial form of complementary medicine. In 2008, the Johns Hopkins Board of Trustees appointed her to a chair as a University Distinguished Service Assistant Professor of Breast Cancer. This is the first time in the history of the institution that a hospital nurse has been appointed to a distinguished service designation. Ms. Shockney has recently been promoted to full professor in the Johns Hopkins University School of Medicine. She is the first nurse in the country to have a primary appointment in the school of medicine and achieved the highest academic rank of full professor in surgery and oncology and also University Distinguished Service Professor of Breast Cancer.

FLASCO continues to make strides in promoting excellence in care of patients afflicted with cancer and hematologic disorders and leads efforts to respond to policies and legislation that threaten to compromise patient access to quality cancer care. FLASCO is gaining momentum as more professionals use us as the catalyst to develop partnerships, share knowledge, and stand together to demonstrate strength and determination to move forward in the battle against cancer.

FLASCO consists of over 2,100 members that include physicians and their supporting staff, allied healthcare, and industry representatives that represent the oncology professionals of Florida. FLASCO will continue to be recognized as a nationally respected state professional society as we stand together as “the Voice of Oncology in Florida”.
